Interaction as care in advanced dementia: protocol for a qualitative video-based study of routine care practices.

INTRODUCTION: Interaction plays a central role in the delivery of care and in supporting participation and well-being in healthcare. For people living with advanced dementia, cognitive and communicative impairments can restrict participation in everyday care activities while increasing reliance on interaction to obtain support. Although participation is widely promoted in dementia care, empirical evidence on how it is enacted in routine care interactions, particularly in advanced stages of dementia, remains limited. This study aims to examine how healthcare professionals create opportunities f

A Call for Compassionate Care.

The Substance Abuse and Mental Health Services Administration (2014) reported that 1.1% of pregnant women used nonprescription opiates (either heroin or prescription pills) in the last 30 days. Methadone and buprenorphine are the most common treatments for opiate addiction in pregnant women. Reasons for seeking opiate maintenance treatment (OMT) include keeping custody of one's children and the hope of developing a healthy parenting relationship. Yet, healthcare organizations and professionals have mixed opinions regarding health promotion within the maternal OMT population. Proper education r

Right Antibiotic, Right Discharge: Ethical Tensions Between Antimicrobial Stewardship and Social Vulnerability.

In a case of a patient with postoperative septic arthritis and hardware-associated osteomyelitis due to methicillin-susceptible Staphylococcus aureus that is susceptible to multiple oral agents, the clinician must navigate not only a treatment decision between oral and intravenous antibiotics but also the ethical tension between evidence-based practice and the patient's social instability. We examined 3 interrelated ethical conflicts: balancing clinician-patient relationship with organizational priorities, weighing the benefits and harms of competing treatment decisions, and considering how pa

Enhancing advanced clinical practitioners' non-clinical skills.

This article examines the setting up, running and content of a study day for advanced clinical practitioners (ACPs) at the Rotherham NHS Foundation Trust, aimed at enhancing non-clinical competencies in leadership, education and research. Despite a structured national framework, ACPs often report limited support in these areas. The event provided targeted education, encouraged a sense of community, and received positive feedback, showing improvements in participants' knowledge, confidence and professional connections. Recommendations include making this an ongoing initiative to support compreh

A small life in detail.

End-of-life care for extremely low-birth-weight newborns is complex. Newborns cannot be informed or make personal choices. Uncertainty in outcomes as well as defining futility can make decisions about withdrawing or withholding care nebulous. Major ethical, legal and professional problems can arise when clinicians are not skilled in extending end-of-life care to include the family, healthcare team and all involved. The competent, caring and compassionate clinician can often facilitate a positive outcome in end-of-life experiences.

Engaging in health communication can strengthen their ability to translate specialized knowledge into accessible language, a skill that also connects with the competence of continuing medical education. OBJECTIVES: This study aimed to evaluate IVF-related short videos on Douyin and to propose a practi

[Emergency physician expertise in the emergency medical services of tomorrow: requirements in the context of an evolving emergency care system].

The prehospital emergency care system in Germany is undergoing profound structural change, driven by demographic shifts, increasing emergency service responses and the expanding competencies of paramedics as the most highly qualified non-physician Emergency Medical Services (EMS) professionals. Within standardized pre-delegation frameworks, they perform clearly defined physician-prescribed interventions and, in emergency situations, independently carry out medical procedures in accordance with Section 2a of the German Federal Act on the Profession of Emergency Paramedics (Notfallsanitätergeset

Knowledge, affirmativeness, and experience as factors associated with self-efficacy in LGBTQ+-affirmative services for individuals with IDD.

INTRODUCTION: Lesbian, gay, bisexual, transgender, and queer+ (LGBTQ+) individuals with intellectual and developmental disabilities (IDD) experience compounded discrimination stemming from the intersection of disability and sexual and gender minority identities. Direct support and service-providing professionals (DSPs) play a critical role in shaping inclusive service environments; however, perceived lack of competence may contribute to the avoidance of sexual identity-related topics and unmet support needs. Despite the importance of affirmative practice within disability services, limited res

Advocacy and policy leadership training for primary care: a global environmental scan of competency frameworks, interventions, and strategic priorities.

BACKGROUND: Primary health care (PHC) strengthening globally is catalysed by family physicians and primary care professionals equipped with integrated advocacy, policy leadership, and governance competencies. Yet training in these domains remains fragmented and inconsistent across both high-income countries (HICs) and low- and middle-income countries (LMICs), limiting primary care providers' capacity to influence health systems, advance equity, and engage in the policy processes essential to PHC system strengthening and universal health coverage (UHC).
