[Retraining in dento-facial orthopedics. An orthodontist's viewpoint].

In dentofacial orthopedics, rehabilitation concerns the performance of the orofacial functions. Orthodontists will therefore speak of rehabilitation of functions, or functional therapy. Orofacial functions seem to obey a hierarchical system in accordance with their physiological importance; the impact of their disorders on both muscular behavior and facial morphogenesis, proportionally to their physiological importance. Therefore, priority will be given at a very early stage to the rehabilitation of respiration: restoring nasal ventilation is indeed an essential condition to obtain balanced or

Stridor as the major presenting symptom in riboflavin-responsive multiple acyl-CoA dehydrogenation deficiency.

UNLABELLED: Inspiratory stridor of unknown origin was the leading clinical symptom in an 11-month-old boy. The stridor increased over a period of 4 weeks, and assisted ventilation became necessary. Selective urinary screening by gas chromatography/mass spectrometry analysis revealed excretion of ethylmalonic and 3-OH-isovaleric acid and of N-isobutyryl-, N-2-methylbutyryl-, N-isovaleryl-, N-hexanoyl- and N-suberylglycine. Neither hypoglycaemia nor metabolic acidosis were noticed. Treatment with 200 mg of riboflavin per day led to a dramatic clinical improvement with restoration of normal respi

Central sleep apnea-hypopnea syndrome in heart failure: prevalence, impact, and treatment.

Despite recent advances in its treatment, congestive heart failure associated with depressed left ventricular function continues to be associated with excess morbidity and mortality. Multiple factors may contribute to the progressively declining course of heart failure. Nocturnal arterial oxyhemoglobin desaturation caused by sleep-disordered breathing could be a contributing factor, particularly because it has been associated with excess mortality in patients with chronic obstructive pulmonary disease. Cheyne and Stokes were the first to observe periodic breathing in patients with heart failur

[Cerebellar hypoplasia in the newborn: association with respiratory control disorders and mental retardation].

Cerebellum malformations are frequently diagnosed since the advent of the neuroradiological studies (computed tomography, magnetic resonance imaging and ultrasonography). Cerebellar hypoplasia is found in association with a wide variety of neurologic and systemic disorders. Clinical picture in newborn may be different than in other periods of life. Two characteristics are interesting in neonatal period: their relation with abnormal respiratory control and mental retardation. The pathophysiology of cerebellar hypoplasia is uncertain, however experimental studies suggest than an abnormality of t

Sleep apnea and sleep-related breathing disorders in patients with craniofacial synostosis.

Seventeen patients with craniofacial synostosis (CFS) have been treated at the Department of Plastic and Reconstructive Surgery at Osaka Medical College during the past 10 years. Six patients were thoroughly evaluated at the Department of Otolaryngology by polysomnography (PSG), cephalometric X-ray and nasopharyngoscopy during sleep. In 4 of the patients PSG showed obstructive sleep apnea syndromes (OSAS). Heavy snoring without apnea and paradoxical respiration were noted in the other 2 patients. Thus, all of the patients had sleep-related breathing disorders (SRBD). It is assumed that the inc

Clinical applications of cyclic alternating pattern.

Sleep-related disorders are revisited in the light of the physiological modality of NREM sleep expressed by the cyclic alternating pattern (CAP). Owing to its fluctuating properties on vigilance, muscle tone, and vegetative activities, CAP represents a highly favorable condition for the occurrence of interictal generalized and focal lesional EEG discharges, for motor seizures, and for periodic jerks in nocturnal myoclonus. All these manifestations are significantly associated with the components of activation during CAP, i.e., the A phases. On the contrary, the B phases of CAP appear chronolog

Mitochondrial import and processing of an in vitro synthesized human prebranched chain acyltransferase fragment.

A 1.6-kb cDNA for human liver branched-chain acyltransferase [E2b] was placed in a transcription vector under the control of the SP6 promoter. In vitro translation of transcripts from this vector produced a pre-E2b fragment of Mr 39,000. Following import into mitochondria, this protein was processed to a protein with an Mr of 36,000. The processed protein was fully protected from trypsin digestion. Import and processing did not occur in the presence of rhodamine 123 or carbonyl cyanide m-chlorophenyl hydrazone, suggesting that membrane potential and coupled respiration were required. Uptake an

A sleep stage estimation algorithm based on cardiorespiratory signals derived from a suprasternal pressure sensor.

Automatic estimation of sleep structure is an important aspect in moving sleep monitoring from clinical laboratories to people's homes. However, the transition to more portable systems should not happen at the expense of important physiological signals, such as respiration. Here, we propose the use of cardiorespiratory signals obtained by a suprasternal pressure (SSP) sensor to estimate sleep stages. The sensor is already used for diagnosis of sleep-disordered breathing (SDB) conditions, but besides respiratory effort it can detect cardiac vibrations transmitted through the trachea. We collect

The remarkable response to ponatinib therapy in a child with blastic phase of chronic myeloid leukemia.

Myasthenia gravis (MG) is a chronic, autoimmune disease involving neuromuscular junctions. It is frequently associated with symptoms such as loss of muscle strength, difficulty in respiration and swallowing, diplopia and ptosis. All chronic diseases, including MG, may have psychiatric consequences in terms of coping and adaptation. Psychiatric morbidity usually appears as anxiety disorders, such as panic disorder and generalised anxiety disorder, and as depressive disorders. However, there are very few data on the prevalence and aetiology of such psychiatric symptoms in patients with MG, and t

[The assessment of breathing during sleep: a curiosity or clinical necessity?].

The study of sleep, which initially focused on the neurophysiological mechanisms and cardiorespiratory function during the night, has shown the presence of sleep-related breathing disorders that epidemiological, pathophysiological and clinical data have indicated to be associated with increased cardiovascular morbidity and mortality: the obstructive sleep apnea syndrome (OSAS) and the central sleep apnea syndrome (CSAS). OSAS is a condition characterized by repetitive respiratory pauses due to the pharynx wall collapse, with a subsequent obstruction to the airflow. The hemodynamic consequences

Impact of sleep in respiratory failure.

Sleep has a physiological influence on respiration, which can have major adverse effects on gas exchange in patients with respiratory insufficiency. These effects relate largely to a reduction in various stimulant inputs to the brainstem respiratory centre. Conditions that may be associated with sleep-related respiratory insufficiency range from pulmonary disorders (such as chronic obstructive pulmonary disease (COPD)), to central respiratory insufficiency (such as central alveolar hypoventilation), neurological and neuromuscular disorders (such as polio and muscular dystrophy), and thoracic c

Respiratory dysfunction in myotonic dystrophy type 1: A systematic review.

Myotonic dystrophy type 1 (DM1) is one of the most common muscular dystrophies in adults. This review summarises the current literature regarding the natural history of respiratory dysfunction in DM1, the role of central respiratory drive and peripheral respiratory muscle involvement and its significance in respiratory function, and investigates the relationship between genetics (CTG repeat length) and respiratory dysfunction. The review included all articles that reported spirometry on 10 or more myotonic dystrophy patients. The final review included 55 articles between 1964 and 2017. The maj

Sensitivity to carbon dioxide and translational studies of anxiety disorders.

Heightened concentrations of CO2 in inhaled air provoke temporary acidification of the brain, followed by compensatory hyperventilation and increased arousal/anxiety. These responses are likely to map a basic, latent general alarm/avoidance system that is largely shared across mammals, and are sources of individual differences. By showing paroxysmal respiratory and emotional responses to CO2 challenges, humans with panic and separation anxiety disorders lie at one extreme of the distribution for CO2 sensitivity. This is also a developmental trait, sensitive to interference with parental cares.

Assessment of Cardiorespiratory Interactions during Apneic Events in Sleep via Fuzzy Kernel Measures of Information Dynamics.

Apnea and other breathing-related disorders have been linked to the development of hypertension or impairments of the cardiovascular, cognitive or metabolic systems. The combined assessment of multiple physiological signals acquired during sleep is of fundamental importance for providing additional insights about breathing disorder events and the associated impairments. In this work, we apply information-theoretic measures to describe the joint dynamics of cardiorespiratory physiological processes in a large group of patients reporting repeated episodes of hypopneas, apneas (central, obstructi

Ventilatory response to CO2. II. Studies in neurotic psychiatric patients and practitioners of transcendental meditation.

The prominence of respiratory symptoms in patients with neurotic psychiatric disorders is noted and the literature on the control of respiration is reviewed to attempt to explain this finding. A previous study demonstrated a positive correlation between the ventilatory response to CO2 (S) and neurotic personality traits in a group of normal subjects. This study attempted to follow up this finding and hypothesized that a group of neurotically disturbed patients would have a higher S value and a group of individuals who practiced a calming technique such as transcendental meditation (TM) would h

Anti-Inflammatory Properties of Garrya flavescens: Phytochemical Profiling and Mitigation of LPS-Induced Neuroinflammation via ERK Signaling and Mitochondrial Modulation.

Garrya flavescens S. Wats. (GF) has been traditionally used to treat gastrointestinal spasms, yet its bioactivity within the central nervous system remains unexplored. This study aimed to characterize the bioactive constituents of GF and evaluate its anti-inflammatory and metabolic regulatory effects in lipopolysaccharide-activated microglia. Phytochemical profiling using LC-HRMS and HPLC identified rutin as a primary bioactive component, present at an exceptionally high concentration (9309 μg/g). In BV-2 microglial and RAW 264.7 cells, GF treatment significantly suppressed the expression of p

MitoQ attenuates maternal immune activation-induced mitochondrial and ER stress programs and ameliorates psychiatric-like behavioral deficits.

Maternal immune activation (MIA) increases risk for neurodevelopmental and psychiatric disorders, yet the stress programs linking prenatal immune challenge to persistent neuronal dysfunction remain incompletely defined. Here, we show that MIA engages convergent oxidative and ER stress programs accompanied by mitochondrial dysfunction across brain regions. Transcriptomic profiling of offspring hippocampus (HIP) revealed coordinated downregulation of oxidative phosphorylation (OXPHOS) and upregulation of ER protein-processing pathways, consistent with increased neuronal oxidative damage and ER s
