Relationship of plasma growth hormone to slow-wave sleep in African sleeping sickness.

Human African trypanosomiasis (sleeping sickness) is a unique disease model of disrupted circadian rhythms in the sleep-wake cycle and cortisol and prolactin secretion. This study examined the temporal relationship between growth hormone (GH) secretion and the sleep-wake cycle in 8 infected African patients and 6 healthy indigenous African subjects. Twenty-four-hour sleep patterns were recorded by polysomnography and hourly blood samples analyzed for plasma GH. No relationships between the mean normalized plasma GH levels (Z scores) and the sleep stages (wakefulness, sleep stages 1 and 2 ('lig…

Delta sleep-inducing peptide in normal humans and in patients with sleep apnea and narcolepsy.

We measured morning plasma concentrations of delta sleep-inducing-peptide-like-immunoreactivity (DSIP-LI) in 9 sleep apnea patients, 10 narcolepsy patients, and 11 normal controls. Comparisons between the three groups showed no significant differences, although there was a trend toward association with low levels of DSIP-LI in the narcoleptic group, particularly in patients not using medications. No differences were found in the morning or evening plasma DSIP-LI levels in a second group of 11 normal controls and 8 sleep apneics. Our findings do not appear to support a biological marker role of…

Sleep and wakefulness after treatment for craniopharyngioma in childhood; influence on the quality and maturation of sleep.

Craniopharyngiomas are situated in immediate vicinity of sleep regulating structures in the basal forebrain area, and the tumor and its treatment might influence the regulation of sleep and wakefulness. In 10 patients treated for craniopharyngioma nighttime sleep quality and daytime vigilance were examined with polygraphic sleep records and multiple sleep latency tests (MSLT). Two girls and 8 boys, 7.1-22.9 years of age, were studied after a follow-up time of 1.5-16.1 years postoperatively. The results were compared to those of 18 normal children. The regulation of the ultradian sleep rhythm w…

Adjustment disorders of sleep: the sleep effects of a major stressful event and its resolution.

Seventy volunteers had 3 nights of sleep recordings during a period of marital separation, and 61 returned for repeat studies 1 year later. At that time, the divorce was final for 42. Forty of the volunteers were depressed when first screened, and 30 were not. Initially all those undergoing marital separation had less delta sleep than an age-matched married comparison sample. Delta increased at followup for those whose divorce was completed. Rapid eye movement (REM) latency was reduced and REM percent was elevated only in the depressed. Among the not depressed, those whose divorces remained in…

Pharyngeal size and shape during wakefulness and sleep in patients with obstructive sleep apnoea.

Computed tomography has been used to study the pharyngeal airway during tidal breathing in wakefulness and during obstructive apnoeas in Non-REM sleep in patients with obstructive sleep apnoea. In supine subjects, contiguous transverse 10 mm sections were taken perpendicular to the posterior pharyngeal wall with a 2.1 s scan time. Studies during wakefulness showed that the narrowest section of the pharyngeal airspace was in the region posterior to the soft palate and that the minimal airway cross-sectional areas were significantly reduced in the group of patients with obstructive sleep apnoea …

[Some indices of the activity of the brain in "rapid" sleep preceeded or not preceeded by delta-sleep].

In order to study the functional interaction between the delta sleep and the REM sleep some psychophysiological features of REM sleep were examined in REM-onset (without any preceding delta sleep--"early REM period") and in the REM period (REMP) terminating the normal sleep cycle (with the preceding delta sleep) of 92 daytime sleep attacks in 10 narcoleptic patients. Under these conditions the significant differences exist in the characteristics of the dream reports and in subjective estimations of sleep quality and duration. Sleep was evaluated as "superficial" and underestimations of sleep d…

Sleep underestimation and subjective sleep appraisal: Limited value of routine polysomnographic and electroencephalographic features.

OBJECTIVES: Whether routine polysomnographic (PSG) and electroencephalographic (EEG) features distinguish patients who underestimate sleep remains unclear. We examined associations of clinical, subjective, PSG, and EEG features with sleep underestimation and tested whether EEG features added discrimination beyond a parsimonious three-predictor baseline model. METHODS: We analysed 225 adults undergoing clinically indicated in-laboratory PSG in this single-centre retrospective cross-sectional study. The misperception index ([objective - subjective total sleep time]/objective total sleep time) w…

Self-reported sleep health is more than a combination of multiple objective sleep measures.

OBJECTIVES: The self-reported Ru-SATED questionnaire is a psychometrically validated measure of multidimensional sleep health. Ru-SATED is increasingly examined in relation to health outcomes; however, its relationship to behavioral sleep is unknown. This study describes the combinations of bed-measured sleep health features that are associated with the Ru-SATED sleep health score. METHODS: Sleep Number bed users (N = 2674, Mage = 52.4 ± 16.2, 55.2% women) completed the Ru-SATED 4.0 multidimensional sleep health scale and contributed past-month smart bed data (7-28 days). Associations between…

Upper airway stimulation for obstructive sleep apnea: Treatment outcomes by sleep position and sex.

Obstructive sleep apnea (OSA) severity varies by sleep position and sex; however, it is unknown whether these factors affect hypoglossal nerve stimulation (HGNS) treatment effectiveness. Thus, this study evaluates HGNS control of OSA by sleep position and sex. To enhance real-world clinical applicability of results and address concerns regarding appropriate postoperative sleep study criteria to evaluate HGNS outcomes, our study also proposes and utilizes a standardized HGNS titration study scoring protocol to assess apnea-hypopnea index (AHI). A retrospective cohort of adult patients underwent…

Deep learning-based automatic scoring of drug-induced sleep endoscopy in obstructive sleep apnea.

Polysomnography is the standard tool for assessing obstructive sleep apnea (OSA) severity; however, it does not provide information regarding the anatomical site or extent of upper airway obstruction. Drug-induced sleep endoscopy (DISE) serves as a dynamic method to evaluate airway collapse under sleep-like conditions, thereby helping to bridge this gap. However, its clinical utility is limited by inter-observer variability and subjectivity in interpretation. We developed internally and externally validated deep learning models utilizing convolutional neural networks based on EfficientNet-B2 a…

Precision in Pediatric Obstructive Sleep Apnea: Preventing Unnecessary Surgery Using Drug-Induced Sleep Endoscopy.

Pediatric obstructive sleep apnea (OSA) is often multifactorial, and awake airway assessment may fail to accurately identify the sites of upper airway obstruction. Drug-induced sleep endoscopy (DISE) enables dynamic evaluation of the upper airway under sleep-like conditions and may be particularly valuable in children at an increased risk of persistent OSA after adenotonsillectomy. We report the case of a 10-year-old girl with severe OSA, obesity (body mass index: 31.04 kg/m², above the 99th percentile), and neurocognitive impairment, in whom awake assessment suggested adenotonsillar hypertrop…

Sleep architecture and rapid eye movement sleep without atonia in post-COVID-19 insomnia.

STUDY OBJECTIVES: Insomnia associated with COVID-19 infection is a common complaint in long-COVID. Studies to date have predominantly examined post-COVID-19 sleep disturbances with questionnaires. We aimed to investigate whether there are distinctive polysomnographic findings in post-COVID-19 insomnia compared to non-COVID-related chronic insomnia. METHODS: We included 150 patients with chronic insomnia, stratified into three groups: post-COVID-19 insomnia (N = 50), chronic insomnia during the pandemic without a history of COVID-19 infection (N = 50), and pre-pandemic chronic insomnia (N = 50…

Clinical subtypes and sleep-wake evolution pattern in epilepsy manifesting as sleep-related seizures.

OBJECTIVE: Epilepsy manifesting as sleep-related seizures (EpSRS) represents a common but heterogeneous group. This study aimed to identify distinct clinical subtypes of EpSRS and explore alterations in sleep-wake seizure rhythms over the disease course. METHODS: EpSRS is defined by seizures occurring ≥80% during sleep. We conducted a cross-sectional study of 550 patients with EpSRS. Patients were classified as current EpSRS if ≥80% of their seizures occurred during sleep over the past year, or as previous EpSRS if they had a prior history of EpSRS but no longer met this 80% threshold during …

Sleep hygiene and sleep control mediate the relationship between cognitive fusion and insomnia severity.

OBJECTIVES: Insomnia is associated with mental ill health, worry and rumination-the two latter contribute to pre-sleep cognitive arousal. These variables also associate with psychological inflexibility, including cognitive fusion (CF), a literal attachment to unhelpful thoughts, patterns and rules. Good sleepers generally have low CF, good sleep hygiene and a belief in sleep control, but individuals high in CF may have unhelpful beliefs about sleep hygiene and sleep control. METHODS: We investigated associations between CF, sleep hygiene and sleep control, hypothesizing that each would contri…

Sleep Quality in Spanish Adult Women: Associations with Sociodemographic Variables and Pre-Sleep Habits.

Background/Objectives: Sleep health is a key determinant of physical and psychological well-being, yet poor sleep quality remains common, particularly among women. Sociodemographic inequalities and culturally patterned pre-sleep behaviours may contribute to sleep disturbances, but their combined role in Mediterranean populations is still insufficiently understood. This study aimed to examine sleep quality and its associations with sociodemographic characteristics and pre-sleep habits among adult women living in Spain. Methods: A cross-sectional online survey was conducted among 785 Spanish adu…

Home sleep study nasal airflow patterns and epiglottic collapse during drug induced sleep endoscopy.

PURPOSE: Obstructive sleep apnea (OSA) is characterized by collapse of various portions of the pharynx. Epiglottic collapse is difficult to diagnose and may affect treatment tolerance. Previous research shows a distinct nasal airflow pattern during epiglottic obstruction characterized by signal discontinuities. It was hypothesized that primary epiglottic collapse characterized during drug-induced sleep endoscopy (DISE) correlates with nasal airflow discontinuity signals seen on home sleep studies. METHODS: Patients with clinical records of undergoing both DISE and home portable sleep studies …
