An 80-year-old man was seen in urgent neuro-ophthalmology consultation for bilateral vision loss. He had a past medical history of hypertension and metastatic stage IV colorectal adenocarcinoma. Four months prior to presentation, he developed gradual onset, painless blurred vision in his right eye. He underwent cataract surgery in that eye, but his vision continued to decline to the point of no light perception. He developed new onset, painless, blurred vision in his left eye 3 weeks prior to presentation and woke up with no light perception in his left eye one day prior to presentation.

PURPOSE: To describe a standardized orbital resection technique and outcomes for spheno-orbital meningiomas with soft-tissue invasion of the orbit.
METHODS: A retrospective case review of patients with spheno-orbital meningioma that underwent resection utilizing the Alberta Standardized Orbital Technique (ASOT) between 2008 and 2017 was performed.
RESULTS: Twenty patients met the inclusion criteria. Fifteen females and five males, with an average age of 53.4 years (SD ± 13.1 years). Mean follow-up was 57.3 months (SD ± 29.5 months). Eight cases (40%) had attempted resection prior to referral

BACKGROUND: Spontaneous meningoencephaloceles of the lateral sphenoid sinus are rare entities, and their peculiar location represents a surgical challenge due to the importance of a wide exposure and skull base reconstruction. They are thought to arise from the congenital base defect of the lateral sphenoid or in some cases have been postulated to represent a rare manifestation of altered cerebrospinal fluid (CSF) dynamics. We report the first case in the literature of a Chiari malformation type I (CMI) and a lateral sphenoid encephalocele, revising the theoretic etiology and surgical techniqu

INTRODUCTION: Pterygoid hamulus syndrome (PHS) is a rare cause of orofacial and oropharyngeal pain. PHS can be associated with a hamulus hypertrophy or with a bursitis of the palatosalpingeus but it has not always an anatomic cause.
OBSERVATION: A 36-year-old woman was seen for a constant posterior palatal pain spreading towards oropharynx, increasing during swallowing and lasting for more than 6 months. Physical examination showed an erythema of the soft palate, medially to the hamulus. Hamulus palpation was painful and revealed hamulus hypertrophia on both sides. A bilateral PHS was evocate

OBJECTIVES/HYPOTHESIS: This study aimed to determine the incidence of major complications following both primary and revision transsphenoidal pituitary surgery. Major complications included endocrinopathic, skull base, orbital, hemorrhagic and thromboembolic complications, respiratory failure, and death. Secondarily, this study aimed to examine factors associated with the occurrence of complications.
STUDY DESIGN: Retrospective cohort analysis of California and Florida all-payer databases from 2005 to 2008.
METHODS: The major complication rate following both primary and revision transsphenoi

Hydatid cyst disease is a rare parasitic disease caused by the larval stage of Echinococcus. This parasite in the larval stage can thrive in many parts of the body, most frequently in the liver. Head and neck involvement of the disease is rare. Herein, we present a case of primary hydatid cyst occurring in the infratemporal fossa, which is an extremely rare localization.

We report an unusual case of lingual paraesthesia caused by a fracture of the base of the skull involving the foramen ovale. As far as we know, lingual sensory neuropathy associated purely with a fracture of the base of the skull has not been reported before.

OBJECTIVE: To investigate the clinical applied anatomy in the region of anterior clinoid process, and to improve the therapeutic efficacy of clinoidal tumors.
METHODS: Twelve patients with large meningiomas located in clinoid were surgically treated via the extended anterior and middle fossa combined with epidural approach between January 1998 and August 2004. The surgical outcome and follow-up results were reviewed retrospectively. Supraorbital-posterional approach and cranioorbital zygomatic approach were used when tumors involved cavernous sinus. Anterior clinoid process was grinded with h

OBJECTIVE: Dolenc has pioneered the extradural approach to the anterior clinoid process (ACP) in approaching the cavernous sinus, clinoidal space, and orbital apex. A key step is the division of the frontotemporal dural fold (FTDF). Less experienced surgeons may not be as versatile in their three-dimensional understanding of the superior orbital fissure and thus may risk injury to its contents. Through our cadaveric and subsequent clinical experience, we have devised a modification of the approach that permits safer handling of the contents of the superior orbital fissure.
METHODS: In five co

Damage to the lingual nerve, resulting in transient or permanent paraesthesia or anaesthesia, is a common undesirable complication of surgical interventions to the lower third molar region. The anatomy of the nerve, as it travels from its origin high in the infra-temporal fossa, to the floor of the mouth is quite variable. The most critical part of its course is where it enters the sublingual region just alongside the lingual alveolar plate of the lower third molar. A significant number of lingual nerves are located above the alveolar bone in the gingival tissues, or very close to the bone. Re

The CT appearances of three cases with severe nasopharyngeal cavity narrowing are described. In all cases the facial trauma was due to a motor vehicle accident. These cases demonstrate that posterior maxilla and pterygoid plate fractures can cause significant nasopharyngeal soft tissues swelling, most likely due to haemorrhage and/or oedema. Severe nasopharyngeal cavity narrowing could cause difficulty in elective nasogastric tube and endotracheal tube intubation via the nasopharyngeal route.

The transsphenoidal hypophysectomy has become a relatively frequent procedure in recent years, with the otolaryngologist playing a major role in its renewed popularity. This paper reviews the evolution of pituitary surgery, discusses the surgical technique currently used, and reviews the results and complications of 114 cases over a 13-year period. Of the 129 procedures performed at Johns Hopkins Hospital between 1970 and 1983, 114 charts were available for review. Twelve of these were palliative hypophysectomies and 102 were for sella tumors. Sixty-eight patients were treated with surgery alo
