60 patients with relapsing acute or chronic anal fissure underwent lateral internal sphincterotomy between July 1987 and July 1994. 53 of them have been followed up. The results of this treatment are excellent and are in accordance to those of other series. This technique is proposed as a method of choice in the treatment of anal fissure.

Gas in gallstones represents a rare but well described radiological finding. Other causes of gas in gallbladder include biliary-enteric fistula, sphincterotomy, gas forming organisms cholangitis. However, gas in gallbladder raises suspicion of emphysematous cholecystitis which necessitates prompt diagnosis and management due to its rapid clinical course and high mortality rate.

Anorectal abscess and fistula are among the most common diseases encountered in adults. Abscess and fistula should be considered the acute and chronic phase of the same anorectal infection. Abscesses are thought to begin as an infection in the anal glands spreading into adjacent spaces and resulting in fistulas in ~40% of cases. A number of endoscopic interventions have expanded the range of treatment options in symptomatic pancreatic diseases. 30 patients with main pancreatic duct stones were treated by ESWL. In 18 of 22 patients who had not previously undergone endoscopic pancreatic sphincterotomy (EPST) or endoscopic sphincterotomy (EST), the stone fragments disappeared after ESWL. Pancreaticopleural fistula is an extremely rare complication of pancreatitis. We present a case of pancreaticopleural fistula in a patient with recurrent acute pancreatitis who presented with right-sided symptomatic pleural effusion. Pancreatic sphincterotomy and pancreatic duct stenting led to resolution of pleural effusion and withdrawal of the chest tube within 3 weeks.

Two cases of temporary loss of reflex erectile activity after transurethral bladder-neck and external sphincter resection are reported in two tetraplegics. Only recently such occurrences have been reported in the literature associated with external sphincterotomy. In 17 patients with common bile duct calculi, percutaneous transhepatic balloon dilation of the distal common bile duct and the ampulla of Vater (hepaticopancreatic ampulla) was performed. The calculi were pushed into the duodenum with the catheter, and all were removed without significant complications. This method of calculus removal is an effective alternative to surgery or endoscopic sphincterotomy.
