Isolated syndesmosis ankle injury.

Isolated syndesmosis injuries often go unrecognized and are diagnosed as lateral ankle sprains; however, they are more disabling than lateral ankle sprains. The reported incidence of isolated syndesmosis injuries in acute ankle sprains ranges between 1% and 16%. When ankle disability lasts for more than 2 months after an ankle sprain, the incidence increases to 23.6%. Diagnostic workup may include stress radiographs, magnetic resonance imaging, or diagnostic arthroscopy. A simple stress test radiograph may reveal an unstable grade III syndesmosis sprain that may go unrecognized on plain antero

Common peroneal nerve palsy after grade I inversion ankle sprain.

This case report presents a 28-year-old man with foot drop 48 hours after a grade I inversion ankle sprain. Clinical examination and electrodiagnostic studies showed common peroneal nerve palsy. The patient was managed with conservative treatment and rehabilitation and recovered completely 4 months after the injury. Physicians should be aware of the possibility of delayed peroneal nerve injury after grade I ankle sprain. Function of the peroneal nerve should be evaluated in all patients with inversion ankle sprain as part of initial and follow-up evaluations. Early electrodiagnostic studies ar

Effects of soft and semi-rigid ankle orthoses on postural sway in people with and without functional ankle instability.

Some studies have demonstrated improvement of postural control following use of ankle supports, whereas others reported no effect or even negative effects of bracing. The present study aimed to investigate the immediate effects of soft and semi-rigid orthosis on postural sway in patients with functional ankle instability (FAI). Twenty patients with unilateral FAI and twenty matched healthy individuals participated in the study. Balance in single limb stance on force platform with eyes-open was assessed for both legs while wearing no orthosis (control condition), soft or semi-rigid orthosis. Th

Effects of fatigue due to contraction of evertor muscles on the ankle joint position sense in male soccer players.

BACKGROUND: The high incidence of ankle sprains that occur later in matches suggests that fatigue may contribute to altered neuromuscular control of the ankle. Moreover, deficits in ankle joint position sense (JPS) were seen in patients with a history of recurrent ankle sprains. It has been hypothesized that ankle sprains may be related to altered ankle JPS as a consequence of fatigue. PURPOSE: To evaluate if fatiguing contractions of evertor muscles alter the ankle JPS. STUDY DESIGN: Controlled laboratory study. METHODS: Thirty-six soccer players (age, 24.7 +/- 1.3 years; height, 183.7 +/-

Osteochondral lesions: medial versus lateral, persistent pain, cartilage restoration options and indications.

Chronic giving way and ankle dysfunction are common after ankle sprains. In our approach to chronic ankle pain and giving way, one must consider the differential diagnosis before treatment can be directed appropriately. One of the common diagnoses associated with ankle injury is osteochondral lesions of the talus. The advent of MRI has allowed us to make the diagnosis of occult lesions more readily. Arthroscopic and open management of these lesions continues to evolve. This article discusses osteochondral lesions of the talus, treatment options, and resurfacing techniques.

Effects of foot orthoses on patients with chronic ankle instability.

Chronic instability of the ankle can be the result of mechanical and functional deficits. An acute ankle sprain can cause mechanical and functional instability, which may or may not respond to standard rehabilitation programs. Chronic instability results when there is persistent joint laxity of the ankle or when one or more components of neuromuscular control of the ankle are compromised. A loss of balance or postural control seems to be the most consistent finding among athletes with chronic instability of the ankle. Recent research in patients with acute and chronic ankle instability has rev

Tibial and peroneal nerve conduction studies in ankle sprain.

BACKGROUND: Lower limb injuries and ankle sprain occur frequently among football players, due to the high incidence of physical contact. As a result, lower limb nerves are subject to injury. This project aims to evaluate the lower limb nerve conduction among students playing football to investigate the probable relationship between ankle sprain in football and nerve conductivity. MATERIALS AND METHODS: Fifty volunteer students aged between 19 and 25 were studied. They fell into three groups; one comprising of 20 healthy football players whose football experience exceeded three years, the seco

Contrast enhanced ultrasound with second generation contrast agent for the follow-up of lower-extremity muscle-strain-repairing processes in professional athletes.

PURPOSE: In literature, ultrasonographic potentials in traumatic muscle lesions have been codified, whereas the data about this method utility in follow-up are dissonant. The purpose of this work is to evaluate a second-generation ultrasound (US) contrast agent rule for the professional athletes' distractive lesions. MATERIALS AND METHODS: Twenty professional athletes (18 men and two women, aged between 18 and 34 years) affected by different muscle lesions were examined. All the patients were evaluated within 48 h of the trauma by US device Esaote Technos MPX with a high-frequency linear prob

The medial longitudinal arch as a possible risk factor for ankle sprains: a prospective study in 83 female infantry recruits.

BACKGROUND: Ankle sprains are frequent, especially in athletes, soldiers, or others who perform high levels of physical activity. Although prevention is a primary goal, little is known about the risk factors. We evaluated the association of the structure of the medial arch of the foot to the occurrence of acute and recurrent ankle sprains in 83 female infantry recruits. We found no previous studies on ankle sprains in women in the English literature. METHODS: Arch height was quantified using the Chippaux-Smirak index, and each arch was classified as high, normal, or low. Retrospective data we

Relationship between ankle-foot swelling and self-assessed function after ankle sprain.

PURPOSE: Decreasing swelling after ankle sprain is a main focus of treatment, as it is believed that swelling is related to function. The purpose of this study was to evaluate the relationship between ankle-foot swelling and self-assessed ankle function in the early period after ankle sprain. METHODS: Thirty-six individuals were tested for ankle-foot volume and self-assessed ankle function within days of spraining their ankle. Volumetric measurements of the injured and uninjured ankle-foot segment were taken with a foot and ankle volumeter. Stepwise linear regression analysis was performed, w

Plantar pressure distribution during gait in athletes with functional instability of the ankle joint: preliminary report.

This study addressed the hypothesis that gait characteristics differ between normal subjects and athletes suffering from functional instability (FI) of the ankle joint. FI of the ankle joint is one of the most common residual disabilities after an acute ankle sprain. However, the influences of the FI of the ankle joint on the gait characteristics are not well understood. Athletes suffering from FI of the ankle joint and healthy control subjects were examined. Pressure, area, and force during gait were measured using a pressure measuring system. These in vivo plantar pressure measurements were

Ankle function during gait in patients with chronic ankle instability compared to controls.

BACKGROUND: Despite much research, the reasons behind the development of chronic ankle instability in individuals post ankle inversion sprain are unknown. Chronic ankle instability has not previously been investigated dynamically using 3D motion analysis during walking. We hypothesised that chronic ankle instability subjects would exhibit a different kinematic and kinetic pattern during normal walking when compared with a control group. METHODS: Gait analysis was carried out on fifty subjects (25 chronic ankle instability, and 25 age, gender, activity, and gait velocity matched controls) duri

Ankle problems masquerading as sprains.

The most common ankle injury is a ligamentous sprain, usually involving the lateral side. This is usually treated very successfully with conservative management. Those patients who have continued complaints despite such management should be suspected of having something more than a simple sprain. The diagnostic entities discussed herein should always be part of the differential diagnosis of a sprained ankle, so that when one of these injuries is initially missed, the examining physician will be able to re-examine and eventually diagnose and treat these injuries properly. They should be managed

Acute shoulder injuries.

The shoulder is the most mobile joint in the human body. The cost of such versatility is an increased risk of injury. It is important that family physicians understand the anatomy of the shoulder, mechanisms of injury, typical physical and radiologic findings, approach to management of injuries, and indications for referral. Clavicle fractures are among the most common acute shoulder injuries, and more than 80 percent of them can be managed conservatively. Humeral head fractures are less common and usually occur in elderly persons; 85 percent of them can be managed nonoperatively. Common acute

Usefulness of ultrasonography for early recurrent myositis ossificans.

Myositis ossificans is a benign heterotopic ossifying condition of soft tissues. The lesion may cause persistent functional impairment of adjacent joints. Surgical intervention is often needed to regain normal function, even though a high recurrence rate is reported. In the present case, we found that recurrent myositis ossificans could not be demonstrated by radiography during the early stage. However, ultrasonography proved to be an excellent tool for detecting early recurrence of myositis ossificans and for distinguishing the condition from extraosseous sarcomas.
