Clozapine Withdrawal-Induced Malignant Catatonia or Neuroleptic Malignant Syndrome: A Case Report and a Brief Review of the Literature.

In our brief literature review, we discuss the changes in the concept of catatonia as well as its various types and symptoms. We also succinctly review the possible symptoms of clozapine withdrawal. In addition, we analyze the main features of the very few published cases of clozapine withdrawal-induced catatonia and the relationship between neuroleptic malignant syndrome and the malignant subtype of catatonia. Furthermore, we present the case of a 29-year-old male patient with schizophrenia in whom a malignant catatonic episode/neuroleptic malignant syndrome (with negativism, stupor, mutism, …

Signal Detection of Adverse Drug Reactions of Cephalosporins Using Data from a National Pharmacovigilance Database.

This case-non-case study aims to detect signals not currently listed on cephalosporin drug labels. From 2009 to 2018, adverse event (AE) rep … signals. Additionally, six serious AEs that were not listed on drug labels such as cefaclor-induced stupor (ten cases) and cefaclor-induced respiratory depression (four cases) were found. Detecting signals using a national pharmacovigilance database is useful for identifying unknown ADRs. This study identified signals of cephalosporins that warrant furth …

Magnetic resonance imaging in marchiafava-bignami syndrome: a cornerstone in diagnosis and prognosis.

Marchiafava-Bignami syndrome is a rare condition. However, with the advent of MRI, more and more of these cases are being diagnosed. Thus, it becomes essential for a radiologist to be familiar with its imaging features as well as clinical presentation. A 50-year-old chronic alcoholic presented to the emergency room with history of 3 episodes of seizures 2 days earlier. The patient had gait disturbances for the last few days. On examination, the patient was in a state of stupor. No neck rigidity was elicited. On MRI, the corpus callosum appeared diffusely hypointense on T1 weighted images and h…

[A case of psittacosis].

A 67-year-old Japanese housewife, who had been attended the out patient department of medicine, Fukuoka Dental College (FDC) Hospital for paroxysmal atrial fibrillation, was admitted to FDC Hospital because of high fever, exhaustion, anorexia, myalgia and mild stupor. Her ECG finding revealed atrial fibrillation and roentgenologic examination of the chest showed diffuse opacities in the left lung field (S10) without pleural effusion. As she had told her physician that her pet parakeet had been dead recently, she was diagnosed immediately as psittacosis. She was instantly treated with minocycli…

Hypoglycemia in pediatric renal allograft recipients.

Symptomatic hypoglycemia developed 5 to 45 months after transplantation in nine children who had renal transplants before 6 years of age. During hypoglycemia, serum glucose levels ranged from 14 to 39 mg/dl (0.8 to 2.1 mmol/L). Hypoglycemic episodes occurred between 1.7 and 7.5 years of age. Six patients had generalized seizures; the remaining three had diaphoresis with stupor or lethargy. None of the children had serious infections, diabetes, congenital defects of glucose metabolism, or a history of treatment with insulin or oral hypoglycemic agents. Six patients had hypoglycemic symptoms aft…

Cerebral blood flow in sickle cell cerebrovascular disease.

Cerebral blood flow (CBF) has been studied by the xenon-133 (133Xe) inhalation method in 16 children with suspected sickle cell cerebrovascular disease. Abnormalities consisting of decreases in total, hemispheral, or regional CBF were found in 17 of 26 studies. Eleven studies performed immediately after stroke, transient ischemic attack, or depression of state of alertness showed abnormalities. In addition to confirming regional cerebrovascular insufficiency in children with stroke due to major cerebral artery occlusion, the method detected diffuse decrease in CBF in children with stupor, coma…

Brain lesions in alcoholics. A neuropathological study with clinical correlations.

Among 8735 autopsies performed during a 5-year period at Ullevål Hospital in Oslo there were 70 cases of Wernicke's encephalopathy (0.8%) and 152 cases of alcoholic cerebellar atrophy (1.7%). Cerebellar atrophy was found in 26.8% of all examined alcoholics. Twenty-two of the cases with Wernicke's encephalopathy were active (acute and subacute) and 48 were inactive (chronic). Examination of the clinical records showed that stupor and coma were the dominating symptoms in active cases. A pure Korsakoff's psychosis with a selective memory defect was present in about one-third of the cases with ina…

Toxicity and tissue residues from diphenylhydantoin fed to chickens.

Within three experiments with broiler-type crossbred chickens, diphenlyhydantoin (DPH) was administered via the diet at levels ranging from 10 to 2500 mg./kg. Feed intake and body weight gain were adversely affected by levels of 100 mg./kg. or higher. A hemorrhagic syndrome occurred in 16 percent of the chickens fed 100-1000 mg./kg. and 33 percent of those fed 2500 mg./kg. Twenty percent of the chickens fed 1000 mg./kg. were anemic; none of those receiving 250 mg./kg. were anemic. Other toxic manifestations included neurological side effects of a lethargic stupor, excitability to a disturbance…

Effect of tricainemethanesulfonate (MS222), clove oil and electro-anaesthesia on respiratory burst activity in whole blood and serum alternative complement response in rainbow trout (Oncorhynchus mykiss), during the narcosis stage.

There is a little available information on the suppressive effect of anaesthesia on immune response in fish, especially electro-anaesthesia. In the present study, two anaesthetics, MS222 (50 ppm), clove oil (25 ppm), and electro-anaesthesia were tested in rainbow trout (Oncorhynchus mykiss) during the narcosis stage in order to observe their effects on the innate immune system. The results showed that electro-anaesthesia reduces light emission in chemiluminescence assay both 1 and 24 h post anaesthesia. Clove oil and MS222 decreased light emission 24 h post anaesthesia. In addition, clove oil,

Ecotoxicological quantitative structure-activity relationships for pharmaceuticals.

This paper examined active pharmaceutical ingredients (APIs) acute ecotoxicological modes of action (MOA). It was concluded that the vast majority of APIs acute MOA was non-specific narcosis as; 85% out of 59 APIs had an excess toxicity ratio <7; 70% of the APIs ecotoxicity was overestimated based on a narcotic model; and the majority of APIs Log EC(50)-Log K (ow )regression slopes (-0.49 to -0.86) were within the range of the universal narcosis slopes. However, hydrophobicity is likely not the proper descriptor for assessment of pharmacodynamic APIs chronic ecotoxicity, to asses this accurate

Disseminated angiostrongylosis with involvement of the central nervous system as a cause of sudden death in a dog in Germany.

BACKGROUND: The metastrongyloid nematode Angiostrongylus vasorum, known as "French Heartworm", is an emerging concern in canine health in Europe. Foxes are considered the main reservoir hosts, and affected carnivores predominantly show cardiovascular and respiratory signs. However, a disseminated course of infection can have diverse and severe consequences, including fatal outcomes. CASE PRESENTATION: A six-year-old male Labrador Retriever was presented to the clinic with progressive episodes of stupor lasting a few seconds and dyspnea. Radiographic examination revealed multifocal pulmonary c…

Amoebic Encephalitis Caused by Balamuthia mandrillaris.

We present the case of a 71-year-old man who was diagnosed with amoebic encephalitis caused by Balamuthia mandrillaris. He had rheumatic arthritis for 30 years and had undergone continuous treatment with immunosuppressants. First, he complained of partial spasm from the left thigh to the left upper limb. Magnetic resonance imaging revealed multifocal enhancing nodules in the cortical and subcortical area of both cerebral hemispheres, which were suggestive of brain metastases. However, the patient developed fever with stuporous mentality and an open biopsy was performed immediately. Microscopic…
