Antegrade elastic stable intramedullary nail fixation for paediatric distal radius diaphyseal metaphyseal junction fractures: A new operative approach.

BACKGROUND AND PURPOSE: The treatment of paediatric distal radius diaphyseal metaphyseal junction (DRDMJ) fractures is a challenge. The purpose of this study was to introduce a new operative approach at the proximal "safe zone" of the posterior interosseous nerve (PIN) to treat paediatric DRDMJ fractures and analyse the safety and efficacy of antegrade elastic stable intramedullary nail (ESIN) fixation. METHODS: Thirty paediatric patients with unstable and displaced DRDMJ fractures were treated by antegrade ESIN fixation from November 2015 to September 2017. We created the entrance site at th

Anterior corpectomy and fusion to C2 for cervical myelopathy: clinical results and complications.

PURPOSE: Anterior cervical corpectomy and fusion (ACCF) to C2 (ACCF-C2) for multilevel lesions is a challenging procedure that is indicated for massive ossification of the posterior longitudinal ligament (OPLL) extending to C2 or stenosis at the upper cervical region accompanied by kyphosis. However, there is little information on the effectiveness of and complications related to ACCF-C2. The purpose of this study was to investigate the overall surgical results and postoperative complications of ACCF-C2 for cervical myelopathy. METHODS: Sixteen patients who underwent ACCF-C2 for OPLL and cerv

Operative compared with nonoperative treatment of a thoracolumbar burst fracture without neurological deficit. A prospective, randomized study.

BACKGROUND: To our knowledge, a prospective, randomized study comparing operative and nonoperative treatment of a thoracolumbar burst fracture in patients without a neurological deficit has never been performed. Our hypothesis was that operative treatment would lead to superior long-term clinical outcomes. METHODS: From 1994 to 1998, forty-seven consecutive patients (thirty-two men and fifteen women) with a stable thoracolumbar burst fracture and no neurological deficit were randomized to one of two treatment groups: operative (posterior or anterior arthrodesis and instrumentation) or nonoper

[O-arm Versus C-arm: Comparison of the Learning Curves and Accuracy in Transpedicular Screw Fixation of Lumbar Spondylolisthesis].

PURPOSE OF THE STUDY: The annual number of spinal fusion procedures has been increasing and is well documented worldwide. The O-arm is slowly becoming the standard for transpedicular screw insertion. The accuracy and safety of this method have been confirmed by many studies. Therefore, the learning curve of this method and its use by younger surgeons is the focus of our investigation. Longer operative time and radiation exposure to the patient are its only disadvantages. Our aim was to evaluate the learning curve of neurosurgical residents receiving specialist training and to demonstrate the s

What Are the Risks and Functional Outcomes Associated With Bilateral Humeral Lengthening Using a Monolateral External Fixator in Patients With Achondroplasia?

BACKGROUND: Many patients with achondroplasia experience functional impairments because of rhizomelic upper extremities (proximal limb shortening). Bilateral humeral lengthening may overcome these functional limitations, but it is associated with several risks, such as radial nerve palsy and insufficient bone regeneration. Only a few studies have reported on patient satisfaction and functional outcome after humeral lengthening in patients with achondroplasia. Furthermore, the reported numbers of adverse events associated with lengthening procedures using external fixators vary widely. QUESTIO

Combined anterior cruciate ligament and anterolateral ligament reconstruction using an adjustable-loop device provides similar short-term clinical outcomes compared with isolated reconstruction.

INTRODUCTION/OBJECTIVES: Residual rotational laxity remains a relevant clinical concern after anterior cruciate ligament (ACL) reconstruction. Combined ACL and anterolateral ligament (ALL) reconstruction has been proposed to improve control of rotational laxity; however, clinical outcome data supporting techniques using adjustable-loop femoral fixation for combined reconstruction remain limited. This study aimed to compare short-term clinical outcomes and knee laxity between combined ACL + ALL reconstruction and isolated ACL reconstruction. We hypothesized that combined reconstruction would de

Occipitocervical fusion using a contoured rod and wire construct in children: a reappraisal of a vintage technique.

OBJECT: Many methods to stabilize and fuse the craniocervical junction have been described. One of the early designs was a contoured (Luque) rod fixated with wires, the so-called Hartshill-Ransford loop. In this study, the authors report their 20-year experience with this surgical technique in children. METHODS: The authors reviewed the medical records of patients 18 years of age or younger who underwent dorsal occipitocervical fusion procedures between March 1992 and March 2012 at Le Bonheur Children's Hospital using a contoured rod and wire construct. Data on basic patient characteristics,

A keyhole approach for endoscopically assisted pedicle screw fixation in lumbar spine instability.

OBJECTIVE: The standard approach for dorsal transpedicular fixation in lumbar spine instability requires extensive exposure of the vertebral column. This increases the risk of potential complications and possibly destabilizes healthy neighboring segments because of the dissection and denervation of paravertebral muscles. The majority of spinal disorders are currently treated successfully via limited and tailored approaches. Accordingly, a keyhole approach for dorsal fusion of the lumbar spine was developed on the basis of an anatomic study. METHODS: The new endoscopic technique entails the tr

Diagnostic performance of swab PCR as an alternative to tissue culture methods for diagnosing infections associated with fracture fixation devices.

BACKGROUND: Molecular procedures could potentially improve diagnoses of orthopaedic implant-related infections, but are not yet clinically implemented. Analysis of sonication fluid shows the highest sensitivity for diagnosing implant infections in cases of revision surgery with implant removal. However, there remains controversy regarding the best method for obtaining specimens in cases of revision surgery with implant retention. Tissue culture is the most common diagnostic method for pathogen identification in such cases. Here we aimed to assess the diagnostic performance of swab PCR analysis

Repair of pectus deformities: experience and outcome in 317 cases.

BACKGROUND: The most common congenital chest wall deformities are pectus excavatum and pectus carinatum. Various techniques have been described for correction of pectus deformities. We describe our experience with surgical repair of pectus deformity (PD) in adults, including our new technique, which uses a resorbable plaque for fixation of the sternum. METHODS: We reviewed the records of 317 patients who underwent surgical correction of PD between October 1997 and December 2005. RESULTS: All of the patients were male and the median age was 21.3 years (range, 16-32 years). Of 317 patients, th

Lumbosacral dislocation injuries: management and outcomes.

Lumbosacral dislocations are rare high-energy injuries most often reported as isolated cases. Improved outcomes from major trauma means that these cases will likely become more common. We report six cases treated at our institution in the last decade. Nonoperative care in two cases resulted in severe deformity and pain and required late reconstruction. Surgical decompression, reduction, and stabilization with posterior pedicle screw instrumentation resulted in a stable lumbosacral junction, although in one case, implant failure required revision with anterior column reconstruction to achieve t

The incidence and cause of reoperation for complications after volar locking plate fixation of distal radial fractures.

BACKGROUND: Distal radial fractures are the most common upper extremity fractures. Volar locking plate fixation has become the standard surgical treatment, providing stable angular fixation, early rehabilitation, and effective support for comminuted and osteopenic bones. This study aimed to analyze the incidence and causes of major complications requiring secondary surgeries following volar plating for distal radial fractures and to investigate the correlation between demographic factors and postoperative outcomes, including major complications and reoperation. METHODS: This retrospective stu

Intra-osseous and coverage fixation for inferior pole fracture of the patella: a comparative finite element analysis and retrospective cohort study.

OBJECTIVE: Inferior pole fracture of the patella (IPFP) is notoriously difficult to manage. Although multiple surgical options are available, the percutaneous cannulated-screw tension-band (PCST) technique remains the conventional choice, despite its high complication rate. The recently introduced technique of tension-band wiring through a cannulated screw with suture anchors (TACS-SA) offers a promising alternative, though its efficacy requires further validation. This study was conceived to compare the biomechanical behaviour and early clinical results of the TACS-SA construct with PCST fixa

Scedosporium apiospermum infection of the lumbar vertebrae: A case report.

BACKGROUND: Scedosporium apiospermum (S. apiospermum) is a clinically rare and aggressive fungus mainly found in contaminated water, wetlands, decaying plants, stagnant water, and potted plants in hospitals. The lung, bone, joint, eye, brain, skin, and other sites are easily infected, and there is a marked risk of misdiagnosis. There have been few case reports of infection by S. apiospermum of the lumbar vertebrae; most reports have focused on infection of the lung. CASE SUMMARY: An otherwise healthy 60-year-old man presented with a 4-mo history of lumbosacral pain, stooping, and limited walk

Comparison of Automated Titanium Fasteners to Hand-Tied Knots in Open Aortic Valve Replacement.

OBJECTIVE: Aortic cross-clamp and cardiopulmonary bypass times are independent predictors of postoperative morbidity and mortality. Reducing ischemic times with automated titanium fasteners may improve surgical outcomes. This study compared operative times and costs of titanium fasteners versus hand-tied knots for prosthesis securement in open aortic valve replacement. METHODS: A randomized control trial was conducted during a 16-month period at a single university medical center. Patients undergoing elective aortic valve replacement were randomized to the titanium fastener (n = 37) or hand-t
