Loop electrosurgical excision procedure: an effective, inexpensive, and durable teaching model.

The effectiveness of simulation training for enhancing operative skills is well established. Here we describe the construction of a simple, low-cost model for teaching the loop electrosurgical excision procedure. Composed of common materials such as polyvinyl chloride pipe and sausages, the simulation model, shown in the accompanying figure, can be easily reproduced by other training programs. In addition, we also present an instructional video that utilizes this model to review loop electrosurgical excision procedure techniques, highlighting important steps in the procedure and briefly addres…

Teaching NeuroImages: median nerve MRI changes over time in neuralgic amyotrophy.

A 40-year-old woman presented with 2 weeks of left arm pain followed by hand weakness and numbness. Electrodiagnostic testing revealed an isolated median sensorimotor axonopathy localizing proximal to the pronator teres. The working diagnosis of a forme fruste of neuralgic amyotrophy was made. MRI of the left arm was performed to exclude rare structural causes of proximal median neuropathy. The median nerve showed inflammatory features with a diffusely increased signal on short tau inversion recovery sequences that enhanced with gadolinium (figure, A). Two years after onset, symptoms had resol

Teaching neuroimages: swollen T1 nerve root in neurogenic thoracic outlet syndrome.

A 17-year-old girl without any history of considerable trauma or previous disease was admitted with weakness and wasting in the right forearm and hand, more marked on the thenar eminence (figure). EMG revealed unrecordable medial antebrachial cutaneous sensory and low-amplitude median motor responses (recorded from thenar muscles) along with chronic neurogenic changes in the hand muscles. MRI showed a fusiform focal expansion of the right T1 root (figure). Neurogenic thoracic outlet syndrome (NTOS) was diagnosed and surgically decompressed.1

Teaching neuroimages: isolated sphenoiditis: an uncommon cause of abducens nerve palsy.

A 35-year-old woman presented with a headache lasting 2 weeks and complained of recent diplopia. Neurologic examination showed limited left eye abduction. Initial brain CT scan was considered normal (figure, A). Brain MRI showed left sphenoiditis (figure, B). The patient was treated with amoxicillin/clavulanic acid and corticosteroids with rapid symptom improvement which deferred endoscopy that could provide a bacteriologic diagnosis.

Teaching about vulnerable populations: nursing students' experience in a homeless center.

Cultural competence is not limited to ethnicity, religion, or race but is inclusive of vulnerable groups, such as the homeless. The complex health and social issues related to homelessness requires educational instruction that supports students' ability to address and care for the multidimensional elements that surround this group. Exposure to homeless populations provides nursing students with increased awareness of the issues related to health disparities, while promoting introspective reflection on one's values and beliefs. To increase student exposure to working with homeless clients, a se

Teaching video neuroimages: callosal apraxia: a straightforward model of ideomotor apraxia.

A 57-year-old right-handed man complained of difficulty using his hands post-coronary artery bypass graft (figure). Neurologic examination revealed signs of callosal disconnection without hemiparesis. When asked to perform limb gestures like "brush your teeth" or "wave goodbye," the right hand performed flawlessly whereas the left hand was severely apraxic (video on the Neurology Web site at www.neurology.org).

Surgical management of vascular thoracic outlet syndrome: a teaching hospital experience.

OBJECTIVES: Thoracic outlet syndrome (TOS) consists of a group of distinct disorders that are caused by compression of the brachial plexus and/or subclavian artery and vein. The aim of this study was to highlight the different modalities of diagnosing and treating vascular TOS and evaluate outcomes. METHODS: We conducted a retrospective cohort study between 1999 and 2011 using the medical records database from a teaching hospital. RESULTS: During the study period, 54 cases with vascular TOS were identified in 38 patients. Bilateral TOS was in 16 patients. The median age of the patients was 3…

Teaching neuroimages: Pseudohypertrophic cerebral cortex in end-stage Creutzfeldt-Jakob disease.

A 43-year-old woman presented with 1 month of progressive lower limb burning sensation, blurred vision, and gait disturbance. Her mother died of Creutzfeldt-Jakob disease (CJD). Neurologic examination revealed only cerebellar ataxia. EEG revealed periodic generalized discharges. MRI (figure 1) showed FLAIR hyperintensity in caudate and lenticular nuclei. Molecular analysis confirmed genetic CJD (PRNP E200K mutation). One month later, she became comatose. After 10 months, EEG showed diffuse background flattening without periodic abnormalities, and MRI (figure 2) disclosed diffuse pseudohypertro

Stroke among young adults at the LAUTECH Teaching Hospital, Osogbo, Nigeria.

BACKGROUND: Stroke in young adults is relatively rare and there are very few hospital reports about it in Nigeria. The aetiologic mechanisms of stoke among young adults are quite distinct from those of the adults' populations. OBJECTIVE: The purpose of this retrospective study was to determine the frequency, aetiologic mechanisms and prognosis of stroke among young adults at the LAUTECH Teaching Hospital Osogbo Nigeria. METHODS: The study was both retrospective and descriptive. Case notes of stroke patients aged 16-45 years managed at the LAUTECH Teaching Hospital Osogbo from June 2005 till …

Simulation based education - models for teaching surgical skills in general practice.

BACKGROUND: Simulation based education is an accepted method of teaching procedural skills in both undergraduate and postgraduate medical education. There is an increasing need for developing authentic simulation models for use in general practice training. OBJECTIVE: This article describes the preparation of three simulation models to teach general practice registrars basic surgical skills, including excision of a sebaceous cyst and debridement and escharectomy of chronic wounds. DISCUSSION: The role of deliberate practise in improving performance of procedural skills with simulation based …

Teaching tolerance: New approaches to enzyme replacement therapy for Pompe disease.

Sigmodontine rodents (Cricetidae: Sigmodontinae) represent the most diverse extant mammalian subfamily in both generic and species richness. Variation in conservative organ systems, such as the stomach, can provide insight into the morphological basis of their ecological diversification. Here, we examined the gross internal anatomy of 602 stomachs representing 168 species, 74 genera, and 13 tribes-approximately 80% of the subfamily's generic diversity-using sagittal and transverse sections. We propose a refined and standardized classification of stomach morphology based on the distribution of

Homemade ultrasound phantom for teaching identification of superficial soft tissue abscess.

BACKGROUND: Point-of-care ultrasonography (POCUS) is often used to distinguish abscess from cellulitis in superficial soft tissue infections. With the increased use of POCUS in emergency medicine, it is important that training to use POCUS is enhanced by practice using phantom models. OBJECTIVE: To create an easily made, inexpensive, homemade phantom capable of simulating an abscess in superficial soft tissue infection. METHODS: Increasing amounts of Jell-O (Northfield, Illinois, USA) brand gelatin and sugar-free Metamucil (Cincinnati, Ohio, USA) brand psyllium hydrophilic mucilloid fibre we
