Managing ankle injuries in the emergency department.

We review the anatomy and physiology of the ankle joint with attention to the structures most likely to be injured. We discuss the epidemiology of ankle injuries and their physical and radiographic evaluation, including the Ottawa Ankle Rules. Treatment, consultation, and pitfalls are followed by more specific discussions of tendonitis, Achilles tendon injury, tendonous subluxations, tarsal tunnel syndrome, sonovial impingement, and injury to the os trigonum. The references provide a guide for further reading.

Rupture of the short head component of a bifurcated distal biceps tendon.

BACKGROUND: Rupture of the short head component of a bifurcated distal biceps tendon is a rare injury that may be difficult to diagnose and to treat. METHODS: Three cases of patients with selective disruption of the short head of the biceps distal tendon from a single institution are reported. The presenting history, physical examination, imaging studies, operative findings, and treatment strategies are described. RESULTS: In each case, the mechanism of injury was forceful flexion of the involved elbow against an eccentric load. Notable physical examination findings included a palpable tendo

Trends and complications of distal biceps tendon repair among American Board of Orthopaedic Surgery part II oral examination candidates.

BACKGROUND: Surgical fixation of distal biceps tendon ruptures can restore supination strength and minimize biceps fatigue, resulting in high patient satisfaction rates. Surgical approaches can vary (single incision vs. double incision), and the number of fixation techniques has increased in recent years. The reported rate of postoperative complications after surgical repair of distal biceps tendon injuries is high, ranging from 15% to 35%. The purpose of this study was to assess the trends and postoperative complication profile among newly trained surgeons who performed distal biceps tendon r…

Effect of Physical Parameters and Training Load on Patellar Tendon Stiffness in Professional Athletes.

BACKGROUND: Injuries of the patellar tendon commonly occur as a result of mechanical loading of the tendon during physical activity. Shear wave elastography (SWE) is an established technique for assessing tendon stiffness, and has good interindividual reliability. The aim of this study was to investigate the impacts of physical parameters and different sports on patellar tendon stiffness in professional athletes using SWE. METHODS: Standardized patellar tendon SWE was performed in a relaxed supine position with a small roll under the knee (20° flexion) in 60 healthy professional athletes (30

The double armed suture: tendon repair with immediate mobilization of the fingers.

The double armed suture (DAS) method of repairing flexor and extensor tendons allows almost immediate mobilization of the involved digits. The technique protects the tendon juncture from stress during muscle contraction and hand therapy, which is begun a few days after surgery. This technique has been used in 76 patients since 1972. Good or excellent results have been obtained in 90% of the cases over a 16-year period.

[Rupture in the palm of a flexor tendon in a young man with osteogenesis imperfecta].

Flexor tendon rupture in the palm of the hand is very infrequent. It usually occurs in association with an underlying pathological condition. We report a case of hand tendon rupture in a patient with osteogenesis imperfecta (OI). This disease is typically characterised by brittle bones but other tissues rich in collagen, such as tendons, may also be involved. The ruptured tendon was diagnosed one month post-injury and was treated by tendon graft. Six months later, tenolysis was performed and one year post-injury flexion was still limited at the distal interphalangeal joint. Few cases of tendon

A preliminary study on the effects of acellular tissue graft augmentation in acute Achilles tendon ruptures.

Acute Achilles tendon rupture injuries present surgical challenges because of the mechanical forces placed on this tendon. The purpose of this study was to evaluate the effectiveness of an acellular human dermal tissue matrix, GraftJacket Matrix (Wright Medical Technology, Inc., Arlington, TN), as an augmentation material in acute Achilles tendon repair. Eleven consecutive patients with acute tendon ruptures were evaluated and followed up (20-31 months). Primary repair was followed by augmentation with the graft sutured circumferentially around the tendon. Patients were placed in an early func

Complications of flexor tendon repair in the hand: MR imaging assessment.

PURPOSE: To determine if magnetic resonance (MR) imaging enables differentiation of adhesions from tendon rupture after repair of digital flexor tendon injuries. MATERIALS AND METHODS: The reference group comprised eight tendon sutures with a good clinical outcome. Axial and sagittal spin-echo sequences and three-dimensional gradient-echo sequences with curved reconstructions were analyzed in 63 injured fingers. Reoperation was performed in 41 fingers. RESULTS: MR imaging depicted isolated peritendinous adhesions (n = 31), most often with a continuous, uniform tendon (sensitivity 91%, specif…

[Muscular rupture of the extensor pollicis longus].

Closed ruptures of the extensor pollicis longus tendon usually appear as a consequence of fractures of the wrist joint or the carpal bones or ensue from polyarthritic changes or result from a process of degeneration. Mechanical injury of the tendon is quite rare but can be observed after direct trauma or after operative treatment of a distal fracture of the radius. Closed traumatic ruptures of the extensor pollicis longus tendon in the absence of pathological changes are--in spite of the frequency of rotation injuries of the forearm--very rare. The patient in the case described is a 45-year-ol

Transosseous Double-Row Triceps Repair in a Professional Athlete.

Distal triceps rupture is a relatively rare pathology among muscle-tendon injuries and upper extremity trauma. The most common mechanisms include a fall onto an outstretched arm or direct trauma to the extended elbow, both of which can lead to disruption of the distal triceps tendon. The available literature on treatment outcomes remains limited; however, recent reports suggest that surgical management offers superior results compared with conservative approaches. Among surgical techniques, both suture anchors and transosseous suturing are established options. In this report, we describe the u…

Imaging Tendon Disorders in Athletes.

Imaging plays a critical role in evaluating pathology affecting athletes from various fields. Tendon pathology manifests in terms of mechanical, degenerative, enthesitis, neoplastic, and overuse diseases. Tendon pathologies in athletes usually involve injuries to commonly injured tendons such as the tendons involving the ankle, elbow, rotator cuff, hip abductors, patellar tendon, and Achilles tendon. For the purposes of this article, the focus will be on the tendons involving the ankle such as the tibialis posterior and peroneal tendons. The 2 most common imaging modalities used for the evalua

Traumatic patellar tendon ruptures: a follow-up study of primary repair and a neutralization wire.

Thirty-five patients with traumatic patellar tendon ruptures treated with primary repair and with a neutralization wire were retrospectively reviewed. All the ruptures occurred in acute trauma; 12 of the patients (34%) had multiple injuries, and 19 had open wounds around the knee joints. The diagnostic clues included a high-riding patella by palpation or seen on roentgenograms, hemarthrosis, a palpable gap over the tendon, and inability of patients to extend the knee actively. Using the criteria of Siwek and Rao, at follow-up 57% of outcomes were rated as excellent; 28.6% as good; and 14.2% as

Reconstruction of extensor mechanism after trauma and infection by transposition of the Achilles tendon: report of technique and four cases.

High-energy injuries to the knee often involve extensive soft tissue loss as well as fractures. In certain situations, the patellar tendon may be lost due to initial injury or post-injury complications such as infection. Use of the gastrocnemius flap is a popular method of covering the defect as well as controlling infection and promoting healing by increasing local vascularity. We present a method of reconstructing the patellar tendon by extending the use of the gastrocnemius flaps using its Achilles tendon and calcaneal insertion. The distal insertion of the Achilles tendon is fixed to the t

Physiotherapy after thumb extensor tendon repair in Verdan zone III. Case study.

BACKGROUND: Finger tendon injuries represent very common consequences of hand injuries and, if not properly treated, can lead to substantial hand function impairment and interfere with activities of daily living. This article presents the results of rehabilitation of a female patient after thumb extensor tendon injury in Verdan's zone III. MATERIAL AND METHODS: Based on standard diagnostic work-up (assessment of the active range of motion), we calculated the degree of functional hand function impairment according to Swanson's method. The diagnostic procedures also comprised measurements of dy…

Management of anterior thigh injuries in soccer players: practical guide.

Most of the anterior thigh injuries are contusions or strains, however, some of these injuries can be career ending. Early diagnosis and correct treatment are key to successful outcome. Analyzing injury mechanism and adding both clinical and imaging findings, clinicians can make the right treatment decisions already often in the acute phase of the injury. Low grade contusions and muscle strains are treated well with planned rehabilitation, but complete tendon injuries or avulsions can require operative treatment. Also, neglected minor injuries could lead to chronic disabilities and time lost f…
