A Novel Operation Technique for Forearm Deformities in Patients with Brachial Plexus Birth Injury: Retrospective Analysis of 14 Patients.

Background: We aimed to evaluate the effectiveness of our novel operation technique that included radial shaft shortening plus supination producing osteotomy and transfer of the biceps brachii tendon to the brachialis tendon in patients with chronic radial head dislocation secondary to brachial plexus birth injury (BPBI). Methods: Fourteen patients with chronic radial head dislocation resulting from BPBI were included in this study, with a minimum 1-year postoperative follow-up period. All patients underwent the same surgical procedure. The range of motion of affected elbow was measured with a

Blood Flow Restriction Therapy Stimulates Intercellular Mitochondria Transfer and Improves Muscle Regeneration and Shoulder Function in a Murine Rotator Cuff Injury Model.

BACKGROUND: Rotator cuff (RC) tears are among the most common causes of shoulder dysfunction in sports medicine. Muscle atrophy and degeneration are important risk factors for RC tendon retearing and suboptimal recovery of shoulder function after tendon repair. Although blood flow restriction (BFR) can stimulate muscle regeneration after lower extremity trauma and anterior cruciate ligament reconstruction, the mechanisms that underlie BFR remain unknown, and its application to RC tears has not yet been explored. HYPOTHESIS: The authors hypothesized that BFR induces transfer of mitochondria fr

Reanimation of thumb extension after upper extremity degloving injury treated with Integra.

A case of severe degloving injury to the left hand with complete destruction of the extensor apparatus of the thumb and exposure of the IP joint is presented. The damaged area was resurfaced with the use of the Integra template and the abductor pollicis longus tendon, lengthened with a tendon graft provided by the extensor indicis, and used to restore thumb extension. The transfer was performed through a prefabricated sheath under the Integra resurfaced area in a two-staged procedure. The final result was functional and aesthetically satisfying.

Expected outcome for reconstructive hand surgery after spinal cord injuries related to level of injury and sex.

This retrospective, multicentre study maps grip strength and grasp ability after tendon transfer surgery in patients with tetraplegia. Data were analysed in the whole group and in subgroups stratified into the International Classification for Surgery of the Hand in Tetraplegia (ICSHT) and sex. In total, 200 arms (143 patients) had tendon transfer surgery to restore grip and pinch function, 74% were men, 42% were ICSHT group ≤3 and 58% ICSHT were group ≥4. The one-year outcome across all participants for grip strength was 5.2 kg, pinch strength 2.1 kg and a score of 108 for the Grasp and Releas…

[Long-term results of Grice subtalar arthrodesis in children].

Talipes valgus, mostly resulted from poliomyelitis, of 44 feet in 43 children, aged 4 to 12 years, were treated by Grice extra-articular arthrodesis of the subtalar joint, and on 36 of them tendon transfer was done as well at the same time. The result in all of them, after a period of 15.3 years (ranging 5 to 23 yrs.), was found satisfactory in 32 feet (72.7%), and unsatisfactory in 12 (27.3%). Eight of the latter were found to be due to secondary varus deformity, a consequence of imbalance of muscle power following medial transfer of peroneal tendon (s). It is emphasized that in correcting fo…

Microvascular reconstruction for close-range gunshot injuries to the distal forearm.

Three patients with compound defects in the distal forearm resulting from close-range gunshot injuries were treated with a free microvascular composite groin flap. The composite flap, which consisted of groin skin, the iliac crest, and abdominal muscles, was used to cover the exposed nerves, tendons, and vessels in one stage. The iliac crest replaced the segmental radial and ulnar defects, and the attached muscles were placed deep to the tendons to provide a gliding surface and to prevent adhesions to the bone. "Sandwiching" the damaged nerves and tendons between highly vascular soft tissue ha

Reconstruction of chronic tibialis anterior tendon rupture using a degradable synthetic matrix: A surgical technique.

Tibialis anterior tendon (TAT) ruptures are uncommon and may occur acutely or present in a chronic, neglected form. Chronic ruptures, generally defined as those present for more than four weeks, are frequently associated with tendon retraction, degenerative stump tissue, and a segmental defect that precludes primary end-to-end repair. Established reconstructive options include tendon transfer, autograft interposition, and allograft interposition, each carrying trade-offs in donor site morbidity, tissue availability, and cost. We describe reconstruction of a chronic insertional TAT rupture in a…

The effect of tendon excursion velocity on longitudinal median nerve displacement: differences between carpal tunnel syndrome patients and controls.

The subsynovial connective tissue (SSCT) is a viscoelastic structure connecting the median nerve (MN) and the flexor tendons in the carpal tunnel. Increased strain rates increases stiffness in viscoelastic tissues, and thereby its capacity to transfer shear load. Therefore, tendon excursion velocity may impact the MN displacement. In carpal tunnel syndrome (CTS) the SSCT is fibrotic and may be ruptured, and this may affect MN motion. In this study, ultrasonography was performed on 14 wrists of healthy controls and 25 wrists of CTS patients during controlled finger motions performed at three di

Bilateral reversed palmaris longus muscle: a rare anatomical variation.

We report a case of bilateral reversed palmaris longus muscle (PLM). The muscle was tendinous in its upper portion and muscular in its lower portion in both arms. This rare variation has been mentioned only once in the literature as a surgical finding. According to the literature, a reversed PLM may cause a compartment syndrome in the wrist area, carpal tunnel, and Guyon's syndrome. The described variation is also useful to the hand surgeon as a tendon graft, a tendon for transfer, or as an anatomical landmark for operations at this area.

[Thumb hypoplasia].

All the components of the thumb can be involved in congenital hypoplasia. Blauth's classification continues to be helpful for description of the degree of hypoplasia leading to different options of surgical reconstruction. Surgical strategy includes first web deepening, MP ligamentoplasty, thenar augmentation using tendon transfer and correction of associated abnormalities involving the extensor and flexor tendons. For grade IIIB and grade IV, recommended option for reconstruction is index finger pollicisation.

Flexor hallucis longus hypertrophy secondary to Achilles tendon tendinopathy: an MRI-based case-control study.

PURPOSE: The purpose of this study was to outline an indirect sign of advanced Achilles tendinopathy on magnetic resonance imaging (MRI), based on the hypothesis that these patients would present with secondary hypertrophy of the flexor hallucis longus muscle (FHL). METHODS: MRI scans of Achilles tendon were analyzed retrospectively in two cohorts. The study group consisted of consecutive patients presenting with clinical signs of Achilles tendinopathy and no previous surgeries, while the control group were patients that had an MRI due to other reasons and no signs of tendinopathy. Two parame

Tether Creation Between the Second and Third Extensor Digitorum Communis for Third Extensor Tendon Subluxation at the Metacarpophalangeal Joint.

We developed a substitution technique for a sagittal band reconstruction for subluxation of the third extensor tendon at the metacarpophalangeal joint. Through this procedure, we attempted to supply a tether between the second and third extensor digitorum communis (EDC) tendons. A half-slip of the second EDC was interlaced to the third EDC tendon at the level of the third metacarpal head with the metacarpophalangeal joint in full flexion position; this ensured that the interlace of the graft serves as a direct stabilizer against ulnar subluxation. A key advantage of our procedure is the strong

An unusual complication after suspensionplasty with the abductor pollicis longus tendon for osteoarthritis at the carpometacarpal joint of the thumb.

Suspensionplasty was performed using the abductor pollicis longus tendon for osteoarthritis at the carpometacarpal joint of the thumb. The operated wrist had a full range of motion four months after operation; however, there was swelling on the dorsoradial aspect of the wrist and, upon terminal wrist extension, pain was present at the maximum dorsiflexion. Suddenly, one year after operation the patient could not extend the interphalangeal joint of the thumb. Surgical exploration revealed rupture of the extensor pollicis longus tendon. Tenosynovitis at the extensor carpi radialis brevis tendon,

Transposition of the biceps tendon to reduce lateral scapulohumeral luxation in three species of nondomestic ruminant.

Open reduction of lateral luxation of the scapulohumeral joint was performed in a Mhorr gazelle (Gazella dama mhorr), a southern pudu (Pudu puda), and an Alpine ibex (Capra ibex ibex) by transposition of the biceps brachii tendon lateral to the greater tubercle of the humerus. The ibex had a very large greater tubercle that required a second osteotomy to allow successful lateral transfer of the tendon. Although all three animals were non-weight bearing in the first 2-3 wk after surgery, ambulation improved at 3 wk and was almost normal by 6 wk after surgery. Postoperative follow-up of 8 yr, 7

Surgical treatment of the rheumatoid thumb.

Rheumatoid arthritis commonly affects the thumb. Deformity does not require surgical intervention unless pain is present or a functional deficit exists, but if bony erosion develops and surgical treatment may be compromised in the future, earlier intervention may be indicated. When the pathogenesis and pathoanatomy of the impaired rheumatoid thumb are appreciated, and appropriate treatment is selected, surgical intervention is likely to provide a favorable outcome for the patient.

Attritional flexor tendon ruptures due to distal radius fracture and associated with volar displacement of the distal ulna: a case report.

Flexor tendon rupture following distal radius fractures are rare. In this report, a volarly displaced distal ulna that perforated the volar wrist capsule caused delayed flexor tendon ruptures 25 years after a distal radius fracture. The repair with free tendon graft and the excision of the distal ulna produced a successful result.
