Symptom Duration-Dependent Protein Abundance Changes in Symptomatic and Asymptomatic Tendons in Early Stage Unilateral Patellar Tendinopathy.

Tendinopathy is a painful overuse disorder marked by a progressive functional decline. Although chronic disease mechanisms have been described, early molecular changes remain poorly defined. Here, we performed temporal proteomic profiling of human patellar tendon biopsies across one, two, and three months of symptom duration and evaluated proteome differences between symptomatic and nonsymptomatic contralateral tendons. Biopsies were collected from participants with unilateral patellar tendinopathy and symptoms lasting three months or less for analysis by data-independent acquisition liquid ch…

Orthopedic Rehabilitation Following Complex Repair of the Flexor and Extensor Tendons, Artery, and Nerve: A Case Report.

Simultaneous injury of both the flexor and extensor tendons of the hand is a rare condition that can lead to significant functional impairment and is often accompanied by arterial and nerve damage. This study aims to share the clinical outcomes of the rehabilitation process following a complex hand injury. Surgical repair was performed on a patient with injuries to the flexor and extensor tendons, as well as the radial artery and median nerve, followed by an intensive and structured rehabilitation program. The rehabilitation protocol was initiated with early passive movements and later progres…

Collagen denaturation in post-run Achilles tendons and Achilles tendinopathy: In vivo mechanophysiology and magnetic resonance imaging.

Achilles tendinopathy is often attributed to overuse, but its pathophysiology remains poorly understood. Disruption to the molecular structure of collagen is fundamental for the onset and progression of tendinopathy but has mostly been investigated in vitro. Here, we interrogated the in vivo molecular structure changes of collagen in rat Achilles tendons following treadmill running. Unexpectedly, the tendons' collagen molecules were not mechanically unfolded by running but denatured through proteolysis during physiological post-run remodeling. We further revealed that running induces inflammat…

MRI-Based Morphometric Comparison of Lower Leg Muscles and Tendons in Individuals With Medial Tibial Stress Syndrome.

Runners frequently suffer from medial tibial stress syndrome (MTSS), often linked to excessive eccentric muscle contractions causing periosteal traction by the muscles in the deep posterior compartment. However, the effects of MTSS on these muscles and tendons remain underexplored. This study is aimed at investigating changes in muscle and tendon volumes in this compartment, as well as cross-sectional area measurements, using magnetic resonance imaging. Thirty individuals were divided into two groups: MTSS (n = 18; mean age 30.3 ± 12.4) and control (n = 12; age 35.2 ± 9.2). The anterior, deep …

Shear-wave elastography of canine patellar tendons in healthy dogs and the influence of stifle joint angle.

Elastography is a sonographic modality that measures tissue stiffness, a mechanical property of tissues, and a biomarker for disease. Canine … based on the ease of obtaining measurable results and a quality control propagation wave. Patellar tendons were primarily stiff with a red color elastogram. The proximal and middle segments, measured at various angles, had a mean SWE velocity of 7.32 ± 0.90 m/s. Tendon stiffness did not differ along tendon length when measured in greater extension. Howev …

Tendons exhibit greater resistance to tissue and molecular-level damage with increasing strain rate during cyclic fatigue.

Musculoskeletal soft connective tissues are commonly injured due to repetitive use, but the evolution of mechanical damage to the tissue structure during repeated loading is poorly understood. We investigated the strain-rate dependence of mechanical denaturation of collagen as a form of structural microdamage accumulation during creep fatigue loading of rat tail tendon fascicles. We cycled tendons at three strain rates to the same maximum stress relative to their rate-dependent tensile strength. Collagen denaturation at distinct points during the fatigue process was measured by fluorescence qu…

Effect of bite depth of an epitendinous suture on the biomechanical strength of repaired canine flexor tendons.

OBJECTIVE: To determine effects of bite depth for placement of an epitendinous suture on the biomechanical strength and gap formation of repaired canine tendons. SAMPLE: 48 superficial digital flexor tendons (SDFTs) obtained from 24 canine cadavers. PROCEDURES: Tendons were assigned to 3 groups (16 tendons/group). Each SDFT was transected and then repaired with a continuous epitendinous suture placed with a bite depth of 1, 2, or 3 mm for groups 1, 2, and 3, respectively. Specimens were loaded to failure. Failure mode, gap formation, yield force, peak force, and failure force were analyzed. …

Use of Fat-Suppressed T2-Weighted MRI Images to Reduce the Magic Angle Effect in Peroneal Tendons.

INTRODUCTION: Peroneal tendon disorders pose a diagnostic conundrum to the clinician. Magnetic resonance imaging (MRI) is widely used to assess tendon pathology. A recognized artifact of MRI, the magic angle effect (MAE), can lead to spurious results and inappropriate management. The aim of this study is to assess whether T2 fat-suppressed sequences (T2FSs) reduce the frequency of MAE compared with proton density fat-suppressed (PDFS) images. METHODS: MRI scans of 18 patients were prospectively assessed for MAE. The peroneal tendons were assessed at 5 defined levels on PDFS and T2FS images. T…

[Open repair of gluteus medius and minimus tendons tears with double-row technique : Clinical and radiological results].

BACKGROUND: Operative refixation is a new therapeutic option in cases of failed conservative treatment for trochanteric pain syndrome (TPS) … generation. RESULTS: In this study 12 consecutive cases of open reconstruction of the hip abductor tendons were included. There was a significant improvement in the mHHS. In one case the patient showed an atraumatic rupture in the proximal anchor row. The MRI showed a significant improvement in muscle diameter and cross-sectional area for the gluteus med …
