Immediate implant placement: surgical techniques for prevention and management of complications.

Loss of soft and hard tissue is common after tooth extraction. Substantial resorption of alveolar bone compromises esthetics and may result in prosthetic and surgical limitations. Immediate implant placement at the time of tooth extraction is used to maintain alveolar ridge dimensions. Clinical studies support the successful outcome of immediate placement of dental implants in fresh extraction sockets; comparative clinical studies have found that implant survival rates after immediate placement are similar to those after delayed placement. This article addresses surgical techniques for immedia

Adult stem cell therapy for periodontal disease.

Periodontal disease is a major cause of tooth loss and characterized by inflammation of tooth-supporting structures. Recently, the association between periodontal disease and other health problems has been reported, the importance of treating periodontal disease for general health is more emphasized. The ultimate goal of periodontal therapy is regeneration of damaged periodontal tissues. The development of adult stem cell research enables to improve the cell-based tissue engineering for periodontal regeneration. In this review, we present the results of experimental pre-clinical studies and a …

Impact of genetic polymorphisms on the smoking-related risk of periodontal disease: the population-based study SHIP.

Periodontitis is a bacterial inflammatory disease leading to attachment loss with the consequence of tooth loss. There exists a multifactorial risk pattern including bacterial challenge, smoking, age, sex, diabetes, socio-economic and genetic factors. Smoking has the highest impact on the course of the disease modulated by all the other factors. Here, we report the relationship between smoking and the polymorphisms of genetic polymorphisms inflicted in the pathogenesis. In a randomly selected population-based study, 1083 subjects were typed for the polymorphisms of the IL-1 genotype, Fcgamma RI…

The development of archosaurian first-generation teeth in a chicken mutant.

Modern birds do not have teeth. Rather, they develop a specialized keratinized structure, called the rhamphotheca, that covers the mandible, maxillae, and premaxillae. Although recombination studies have shown that the avian epidermis can respond to tooth-inductive cues from mouse or lizard oral mesenchyme and participate in tooth formation, attempts to initiate tooth development de novo in birds have failed. Here, we describe the formation of teeth in the talpid2 chicken mutant, including the developmental processes and early molecular changes associated with the formation of teeth. Additiona

[Problems and risks in the movement of the mandibular anterior teeth].

The key role of the lower anterior incisors as well as the complicated anatomical relationship of this area play a significant part in orthodontic treatment planning. Three fundamental points of view are selected here for discussion: 1. the variability and clinical relevance of the initial periodontal findings, 2. the importance of the symphysis to sagittal and vertical tooth movement, and, 3. the specific problem associated with periodontal bone loss. The few clinical and experimental studies found in the literature indicate that every anterior tooth movement has to be oriented according to t

A phantom study on the evaluation of the YOLOv8 deep learning model in the detection of tooth ankylosis on cone-beam computed tomography images.

This study aimed to assess the performance of a deep learning algorithm for detecting tooth ankylosis on cone-beam computed tomography (CBCT) scans. A dataset of CBCT scans taken from phantoms simulating tooth ankylosis, including 60 teeth and a total of 4971 axial sections, was used to design an ankylosis detection model using the YOLOv8 algorithm. The first dataset (42 teeth, 70%) with 3822 axial sections comprised the training dataset, while the second dataset (18 teeth, 30%) with 1149 axial sections comprised the test dataset. The YOLOv8 algorithm was used to optimize the precise and effic

A Comparison of Volume of Tissue Removed and Biomechanical Analysis of Different Access Cavity Designs in 2-rooted Mandibular First Molars: A Multisample 3-dimensional Finite Element Analysis.

INTRODUCTION: The aim of this study was to compare the biomechanical properties and the amount of coronal tissue removed among the different access cavities with a multisample 3-dimensional finite element analysis in the mandibular first molar. The correlation between the amount of tissue removed and the fracture resistance of the teeth was also analyzed. METHODS: Micro-computed tomography data from 20 2-rooted mandibular first molars were included in this study as 3-dimensional modeling prototypes. Models of untreated molars and molars treated with the traditional access cavity (TradAC), the

Modeling of the buccal and lingual bone walls of fresh extraction sites following implant installation.

OBJECTIVE: To determine whether the reduction of the alveolar ridge that occurs following tooth extraction and implant placement is influenced by the size of the hard tissue walls of the socket. MATERIAL AND METHODS: Six beagle dogs were used. The third premolar and first molar in both quadrants of the mandible were used. Mucoperiostal flaps were elevated and the distal roots were removed. Implants were installed in the fresh extraction socket in one side of the mandible. The flaps were replaced to allow a semi-submerged healing. The procedure was repeated in the contra later side of the mand

Spontaneous exfoliation of teeth following severe elemental mercury poisoning: case report and histological investigation for mechanism.

BACKGROUND: Although the spontaneous exfoliation of teeth and breakdown of oral tissues from severe mercury intoxication have been noted for over a century, there are no published reports investigating the mechanisms of these phenomena. Severe mercury poisoning is rare in modern times, but it does occur. We present a case report and a histopathologic investigation into the mechanism of the associated tooth loss. METHODS: An exfoliated tooth and periodontal and gingival tissues were obtained from a 15-month-old patient who had been severely intoxicated with elemental mercury over a period of m…

A model for decision making regarding periodontal treatment needs.

The aim of this study was to present a tentative model for decision making regarding periodontal treatment needs, based on the amount of remaining periodontal bone support. In the model presented, a maintained alveolar bone height of one third of the root length at the age of 75 years was regarded to be a reasonable goal. The data analyzed were derived from a patient sample of 194 dentate individuals aged 25-70 years from whom 2 full mouth series of intraoral radiographs, approximately 10 years apart, were available. For teeth present at both examinations, the radiographs were evaluated with r

Dental Implants: Adjacent Tooth Proximity and Contact.

During the osteotomy and placement of dental implants, anatomical positions of adjacent natural teeth to a dental implant site may be very close or in actual contact. This can be due to anatomy, the size of the implant site, a misguided osteotomy, drifting, or migration of the adjacent tooth. The implant surgeon needs to be aware of the position of adjacent teeth to avoid root damage from the implant drill or the implant itself. The dimension of 1.5-mm space has been discussed, but it is an arbitrary speculative dimension. There is no agreed-upon "safe" proximity to adjacent tooth roots. Theor

Plakophilin-1, a Novel Wnt Signaling Regulator, Is Critical for Tooth Development and Ameloblast Differentiation.

Tooth morphogenesis is initiated by reciprocal interactions between the ectoderm and neural crest-derived mesenchyme, and the Wnt signaling pathway is involved in this process. We found that Plakophilin (PKP)1, which is associated with diseases such as ectodermal dysplasia/skin fragility syndrome, was highly expressed in teeth and skin, and was upregulated during tooth development. We hypothesized that PKP1 regulates Wnt signaling via its armadillo repeat domain in a manner similar to β-catenin. To determine its role in tooth development, we performed Pkp1 knockdown experiments using ex vivo o

Greening of the tooth-amalgam interface during extended 10% carbamide peroxide bleaching of tetracycline-stained teeth: a case report.

UNLABELLED: At-home bleaching with 10% carbamide peroxide in a custom-fitted tray has been shown to have some minor effects on certain brands of amalgam, pertaining to mercury release, but generally, effects on amalgam are not considered clinically significant. However, in this case report, a greening of the tooth structure in certain areas immediately adjacent to amalgam restorations in the maxillary and mandibular first molars occurred during tooth whitening. Other amalgam restorations in mandibular and maxillary second molars in the same mouth did not demonstrate any green discoloration of

Invasive cervical resorption: report on two cases.

Invasive cervical resorption is a relatively uncommon form of external root resorption that may lead to tooth loss. This article presents the clinical and radiologic diagnoses and treatment modalities of invasive cervical resorption in 2 patients. In the first case, we did a slow orthodontic forced eruption to make the bone grow coronally. After 6 months, the tooth was extracted and an immediate implant was placed. Eight months later a zirconia crown was cemented. In the second case, we performed root canal treatment followed by a composite reconstruction of the defect. These 2 cases illustrat…

Open proximal contact associated with a cast restoration--progressive bone loss: a case report.

A patient presented with early to moderate periodontitis localized around tooth No. 30. This tooth had a cast restoration with an open proximal contact for the previous seven years. Radiographs spanning this time period showed progressive bone loss. Surgical periodontal therapy was performed after correction of the open proximal contact with a provisional restoration. A new porcelain-fused-to-metal crown was placed three months after surgical therapy. The patients has been on a regular six-month recall schedule for three years and has maintained a healthy periodontium. Prior food impaction has
