PURPOSE: To elevate the intraocular pressure (IOP) in eyes with persistent hypotony caused by excess filtration after trabeculectomy.
DESIGN: Interventional case series.
METHODS: The scleral flap was sutured directly through the conjunctival flap using a round, tapered needle with a 10-0 nylon suture. This procedure has been performed in 10 eyes.

Atropine sulfate, a mydriatic and cycloplegic agent, is frequently used in patients undergoing glaucoma surgery. The success of trabeculectomy surgery depends on the longevity of the filtering bleb. Bleb failure can be categorized into two types: the scarred bleb with high intraocular pressure or the over-filtering and leaking bleb with low intraocular pressure. METHODS: Description of an otherwise healthy woman who developed delayed-onset transient visual loss associated with a leaking filtering bleb.
RESULTS: A 62-year-old woman who underwent trabeculectomy with mitomycin C in her right eye 5 years earlier (Feb 23, 1999) presented with a 3-month history of transient decreased vision. The apparent cause was a periodic gush of aqueous flowing across the surface of the cornea.

The advantages of sodium hyaluronate (Healon) in filtering surgery have been mostly theoretical, with no study of sufficient numbers to justify its use. The authors used sodium hyaluronate in 119 consecutive trabeculectomies. These were compared to a control group of 122 cases. The long-term intraocular pressure and visual acuity results between the two groups were indistinguishable.
