A prospective study of 390 indwelling central venous catheters placed by way of the subclavian route identified bacteremia and tracheostomy as the only significant variables associated with the colonization of the catheters by pathogenic microorganisms. Catheter-related sepsis was likely in only 3 of 33 episodes of proved bacteremia. Detailed statistical analyses of other potential contaminating factors revealed no significant correlations.

Esophago-tracheal fistulae develop to an increasing extent due to the long-term intubation in intensive-care patients. Their treatment is usually problematic, since the general condition of the patient, weakened by his basic disease, very often does not permit any major intervention; in addition, the topographical location of the fistula makes the access to it even more difficult. In one patient two attempts failed to close the fistula from an extended tracheostomy. The narrow access passage between the anaesthetic tube and the tracheal wall did not permit an exact suture; the heavy formation

This report outlines principles of management for extensive corrosive esophageal burns in children complicated by formation of a tracheoesophageal fistula (TEF). Direct operative attack on the fistula usually is unsuccessful, since the tracheal tissues are so damaged that they will not hold sutures. On the basis of experiences with six children, we suggest the following plan of management: (1) early investigation of suspected TEF with thin barium or Dionosil; (2) early tracheostomy using a short, plastic tube; (3) end cervical esophagostomy with closure of the distal stump of the cervical esop

Laryngeal arthritis is a rare manifestation of JRA. The difficulty of indirect laryngoscopy in small children has precluded the type of study which has demonstrated this manifestation in up to one quarter of arthritic adults. Recognition of cricoarytenoid arthritis in the patient reported might have prevented the need for tracheostomy, and eventually led to satisfactory reduction of corticosteroid medication and healing of the tracheostomy stoma.

Coccidioidomycosis is a fungal disease endemic to the southwestern United States. The primary form of the disease is relatively benign and many patients, after exposure by inhalation of the sapyrophytic form of the organism, convert from negative to positive skin testing without significant clinical symptoms. The less common disseminated form represents a serious life-threatening disease and can present with granulomatous changes in the larynx. The authors' experience with disseminated coccidioidomycosis presenting in the larynx of adults and infants successfully treated with amphotericin B ar

In order to be able to carry out effective emergency medicine outside the hospital, knowledge of some of the important puncture techniques is essential. Sometimes injection and infusion is required under difficult conditions, and sometimes decompression punctures are the vitally decisive interventions. Among these are central venous access via the subclavian vein, intracardial injection, decompression puncture in cardiac tamponade, relief of tension pneumothorax, cannulation of the trachea and relief of mediastinal emphysema. These interventions are outlined according to indication, technique

In view of the severe damage caused by unyielding, low residual volume cuffs, various modifications to the cuff of an intratracheal tube have been introduced. The merits of two low-pressure cuffs were assessed in an experimental study in dogs; both cuffs produced little visible damage to the tracheal wall in dogs intubated continuously over a two-week period. A modified technique of producing tantalum tracheobronchograms without distrubing the mucous blanket or traumatizing the tracheal wall is described. These tantalum radiological studies demonstrated a progressive temporary increase in size

Simcock, A. D., and Prout, B. J. (1974).Thorax, 29, 145-146. A patient with respiratory obstruction in glandular fever. Glandular fever is a common disease, and one of the most consistent features is enlargement of tonsillar and nasopharyngeal lymph nodes. This is usually of little serious consequence and, if difficulty in swallowing or breathing occurs, responds rapidly to treatment. We report a case of glandular fever where lymph node enlargement caused respiratory obstruction which required urgent tracheostomy. Some other uncommon features are discussed.
