Artificial neural network-based prediction of prolonged length of stay and need for post-acute care in acute coronary syndrome patients undergoing percutaneous coronary intervention.

BACKGROUND: Prolonged length of stay (LOS) and post-acute care after percutaneous coronary intervention (PCI) is common and costly. Risk models for predicting prolonged LOS and post-acute care have limited accuracy. Our goal was to develop and validate models using artificial neural networks (ANN) to predict prolonged LOS > 7days and need for post-acute care after PCI. METHODS: We defined prolonged LOS as ≥7 days and post-acute care as patients discharged to: extended care, transitional care unit, rehabilitation, other acute care hospital, nursing home or hospice care. Data from 22 675 patien…

Interventions Evaluated to Improve Inhaler Adherence in Patients with Chronic Obstructive Pulmonary Disease: A Scoping Review.

BACKGROUND: Inhalation therapy is central for chronic obstructive pulmonary disease (COPD) management. However, poor inhaler adherence is as … tion-focused (n=8), behaviour-focused (n=4), inhaler technique-focused (n=4), multicomponent (n=3), transitional care-focused (n=4), and reminder-focused interventions (n=1). Five studies used digital delivery, including videoconferencing, telemedicine, SMS, a gamified website, and a WeChat mini-program. Most studies reported improved inhaler adherence, while one f …

Effect of Sex on the Oncological Outcomes in Response to Immunotherapy and Antibody-drug Conjugates in Patients with Urothelial and Kidney Cancer: A Systematic Review and a Network Meta-analysis.

BACKGROUND AND OBJECTIVE: Immune checkpoint inhibitors (ICIs) and antibody-drug conjugates (ADCs) herald a transformative era in metastatic renal cell carcinoma (RCC) and transitional cell carcinoma (TCC) treatment, amid acknowledged sex-based disparities in these cancers. We conducted a systematic review and network meta-analysis (NMA) to identify sex-specific differences in the efficacy of ICI/ADC monotherapy or combination therapies for RCC and TCC survival, in metastatic and adjuvant settings. METHODS: A systematic search was conducted up to October 2023 for English articles on ICIs and A

Do hospital-to-home transitions work for older adults with multiple long-term conditions including dementia? A realist review.

BACKGROUND: Hospital-to-home transitions involve multiple providers and are particularly complex for older adults with dementia, who often live with additional conditions. Frequent transitions increase the risk of errors, miscommunication, and treatment delays, compromising patient safety and leading to potentially increased mortality, morbidity, and preventable readmissions. Understanding what works and does not work in these processes is essential to improving outcomes. AIM: This realist review synthesised existing literature to explore how, for whom, and to what extent hospital-to-home tra

Prospective community-based cluster census and case-control study of stillbirths and neonatal deaths in the West Bank and Gaza Strip.

Obstetric complications and newborn illnesses amenable to basic medical interventions underlie most perinatal deaths. Yet, despite good access to maternal and newborn care in many transitional countries, perinatal mortality is often not monitored in these settings. The present study identified risk factors for perinatal death and the level and causes of stillbirths and neonatal deaths in the West Bank and Gaza Strip. Baseline and follow-up censuses with prospective monitoring of pregnant women and newborns from September 2001 to August 2002 were conducted in 83 randomly selected clusters of 30

Implications of Acute Kidney Injury Requiring Dialysis Versus End-Stage Kidney Disease Status.

More than 20% of patients receiving dialysis at in-center hemodialysis facilities initiate dialysis for acute kidney injury requiring dialysis (AKI-D) rather than end-stage kidney disease (ESKD). The transition from AKI-D to ESKD status is important to reorient clinical efforts from short-term monitoring for recovery to equally important long-term goals of establishing permanent dialysis access and planning for kidney transplant. The timing of this transition is highly variable and determined by the treating nephrologist. Because of numerous subtle policy differences, the implications of trans

Relapses of juvenile idiopathic arthritis in adulthood: A monocentric experience.

INTRODUCTION: Our aim was to describe a monocentric cohort of young adult patients with juvenile idiopathic arthritis (JIA), assessing the risk of relapse after transition to adult care. METHODS: We conducted a retrospective study and collected clinical, serological, and demographic data of young adult patients (18-30 years old) referred to the Transition Clinic of a single Italian centre between January 2020 and March 2023. Patients with systemic-onset JIA were excluded. Primary outcome was disease relapse, defined by Wallace criteria. Risk factors were analysed by Cox proportional hazards r

Physicians' perspectives on continuity of care for patients involved in the criminal justice system: A qualitative study.

BACKGROUND: In 2016, over 11 million individuals were admitted to prisons and jails in the United States. Because the majority of these individuals will return to the community, addressing their health needs requires coordination between community and correctional health care providers. However, few systems exist to facilitate this process and little is known about how physicians perceive and manage these transitions. OBJECTIVE: The goal of this study was to characterize physicians' views on transitions both into and out of incarceration and describe how knowledge of a patient's criminal just

Primary care follow-up of patients after attending a fracture liaison service: an integrative review.

UNLABELLED: Based on a small number of predominantly low-to-moderate quality studies with moderate-to-high risk of bias, the FLS-to-primary care transition is portrayed as a challenging time for patients, GPs, and FLS clinicians, who experience numerous barriers to care continuation and coordination at this care juncture. PURPOSE: Continuity and coordination of care between fracture liaison services (FLS) and primary care is required for optimal long-term osteoporosis care. This study aims to explore (1) how patients and healthcare providers (HCPs) experience the FLS to primary care transitio

The uneven impacts of avoiding public transit on riders' access to healthcare during COVID-19.

BACKGROUND: During the COVID-19 pandemic, many urban residents stopped riding public transit despite their reliance on it to reach essential services like healthcare. Few studies have examined the implications of public transit reliance on riders' ability to reach healthcare when transit is disrupted. To understand how shocks to transportation systems impact healthcare access, this study measures the impact of avoiding public transit on the ability of riders to access healthcare and pharmacy services during lockdowns. METHODS: We deployed a cross-sectional survey of residents of Toronto and V

Caregivers' experiences of service transitions in adult mental health: An integrative qualitative synthesis.

Approximately 5% of the UK population live with serious mental health problems. Data show that informal caregivers of people with mental illness provide care for the highest number of hours compared to other illness and the economic cost of this care is highest in the UK when compared internationally. People living with serious mental health problems make transitions between different intensities of service as their needs fluctuate, including referral, admission, transfer or discharge. Although caregiving is associated with both stress and positive reward, service transitions are particularly

Challenging Health Inequities in Incarceration: a Call for Equitable Care for Kidney Disease and Hypertension.

PURPOSE OF REVIEW: To review the current literature on care of hypertension and chronic kidney disease for people who are currently and formerly incarcerated, and to make recommendations for improving outcomes. RECENT FINDINGS: There is a growing body of literature describing care for kidney disease and hypertension for incarcerated and formerly incarcerated individuals that documents the provision of care itself, notably that many jails contract with private companies; the system is not designed to provide sustained, chronic disease care; and the transition from incarceration to community is

Navigating reentry: a qualitative analysis of health system engagement among adults with chronic health conditions after release from prison.

Transitioning from prison to the community is accompanied by multiple challenges which limit individuals' ability to access health care after release. To address this gap, we developed and pilot-tested a nurse-delivered, transitional case management intervention in two state prisons in the Midwestern U.S. The goal of this analysis was to understand how the intervention, named Call2Care, supported recently-released men and women to attend a primary care appointment after release from prison. We conducted semi-structured interviews with formerly incarcerated men and women after their participati

ANMCO Position Paper: hospital discharge planning: recommendations and standards.

The hospital discharge is often poorly standardized and affected by discontinuity and fragmentation of care, putting patients at high risk of both post-discharge adverse events and early readmission. The present ANMCO document reviews the modifiable components of the hospital discharge process related to adverse events or re-hospitalizations and suggests the optimal methods for redesigning the whole discharge process. The key principles for proper hospital discharge or transfer of care acknowledge that the hospital discharge: • is not an isolated event, but a process that has to be planned as

Transition Failure in Pediatric Inflammatory Bowel Disease: An Underrecognized Determinant of Long-Term Outcomes.

Background/Objectives: The transition from paediatric to adult care in inflammatory bowel disease (IBD) is a period of genuine clinical vulnerability. Despite international guidelines, real-world implementation of transition programmes remains inconsistent and the field lacks consensus on clinically meaningful outcome measures. We introduce the concept of "transition failure" as a provisional composite outcome framework to reorient research toward patient-centred endpoints. Methods: A narrative search of PubMed, MEDLINE, and Embase was conducted using terms including "inflammatory bowel diseas

Child-to-adult transition: a survey of current practices within the European Reference Network for Rare Neurological Diseases (ERN-RND).

BACKGROUND: Transition from child-centered to adult-centered healthcare is a gradual process that addresses the medical, psychological, and educational needs of young people in the management of their autonomy in making decisions about their health and their future clinical assistance. This transfer is challenging across all chronic diseases but can be particularly arduous in rare neurological conditions. AIM: To describe the current practice on the transition process for young patients in centers participating in the European Reference Network for Rare Neurological Diseases (ERN-RND). METHO
