Incidence of tuberculosis in Greek armed forces from 1965-1993.

Studies investigating the epidemiology of tuberculosis in Greece are lacking. Tuberculosis (TB) continues to be a public health problem and its dimensions are not clearly defined. The aim of this study was to estimate the incidence of pulmonary and pleural TB in each of the three branches of military personnel serving in the Greek armed forces, during the period 1965-1993. All military personnel with confirmed pulmonary or pleural TB during the period 1965-1993 were studied retrospectively. The age of the patients ranged from 18-45 years. A significant number of TB patients were detected by th

[Exudative pleurisy in children and adolescents].

Among 179 children and adolescents, tuberculous exudative pleurisy was identified in 157 (87.71%), parapneumonic pleurisy in 19 (10.61%), tumoral pleurisy in 2 (1.12%), posttraumatic pleurisy in 1 (0.56%). The histological study of parietal biopsy specimens (n = 103) and the biological test with the specimen (n = 24) provided evidence for pleural tuberculosis in 83.33% of the examined patients with tuberculous pleurisy.

[A complication of oleothorax--after 38 years].

We present a case-history of a 50 year-old woman who 38 years earlier had been operated for pulmonary tuberculosis by plugging of the pleural cavity with oil (oleothorax). She developed a tumour under the scapula, which it turned out was due to migration of the oil plug to the subscapular area, an unusual complication to oleothorax. Other complications are described.

Sister chromatid exchange rate from pleural fluid cells in patients with malignant mesothelioma.

This study assessed the impact of malignant mesothelioma on the frequencies of sister chromatid exchange (SCE) in the pleural effusion cells. Ten patients with mesothelioma and 20 control subjects were included in the study. The control subjects were the patients with tuberculosis pleurisy, and the remaining 10 subjects of control group were healthy volunteers and only heparinized blood samples were collected from these subjects. The pleural effusion cells were cultured with conventional culture methods. The samples were obtained from the patients after histopathologic confirmation of the mali

Pancreaticopleural fistula causing recurrent bilateral pleural effusions: A case report.

RATIONALE: Pancreaticopleural fistula (PPF) is an infrequent etiology of pleural effusion, characterized by nonspecific thoracic symptoms, which often leads to misdiagnosis and subsequent severe complications. Consequently, early diagnosis is crucial for effective management and the prevention of adverse outcomes. This report presents a rare case of PPF causing bilateral pleural effusions, aiming to enhance clinical recognition of this condition. PATIENT CONCERNS: A 51-year-old male with a history of hepatitis B, pulmonary tuberculosis, and chronic pancreatitis presented with recurrent chest

[Contribution of pleural fluid analysis to the diagnosis of pleural effusion].

Analysis of pleural fluid can have, on its own, a high diagnostic value. In addition to thoracocentesis, a diagnostic hypothesis based on medical history, physical examination, blood analysis and imaging tests, the diagnostic effectiveness will significantly increase in order to establish a definite or high probable diagnosis in a substantial number of patients. Differentiating transudates from exudates by the classical Light's criteria helps knowing the pathogenic mechanism resulting in pleural effusion, and it is also useful for differential diagnosis purposes. Soluble mesothelin-related protein in pleural effusion from patients with malignant pleural mesothelioma.

Malignant pleural mesothelioma (MPM) is a highly aggressive neoplasm primarily arising from surface serosal cells of the pleura and is strongly associated with asbestos exposure. Patients with MPM often develop pleural fluid as initial presentation. However, cytological diagnosis using pleural fluid is usually difficult and has limited utility. A useful molecular marker for differential diagnosis particularly with lung cancer (LC) is urgently needed. The aim of the present study was to investigate the diagnostic value of soluble mesothelin-related protein (SMRP) in pleural fluid. Pleural fluid

Myelomatous Pleural Effusion Revealing Underlying Plasma Cell Myeloma: An Uncommon Initial Presentation.

Myelomatous pleural effusion (MPE), an uncommon manifestation of plasma cell myeloma (PCM), is characterized by proliferation of neoplastic monoclonal plasma cells (PCs). The incidence of MPE in patients of PCM is scarce and often portends an aggressive disease course. We report the case of a 55-year-old male patient who presented with progressive dyspnea for three days along with complaints of nonspecific back pain for three months. The presence of bilateral pleural effusion on chest radiography prompted a clinical suspicion of tuberculosis, leading to pleural fluid analysis. Pleural fluid an

To correlate clinical and biochemical profile of pleural effusion: a retrospective study in tertiary care centre of central India.

BACKGROUND: Pleural effusion indicates an imbalance between pleural fluid formation and removal. Classified into exudative and transudative, with common symptoms of dry cough, dyspnea and pleuritic chest pain. Confirmed etiology has to be established for effective treatment. OBJECTIVE: Correlate clinical and biochemical profile of various etiologies of pleural effusion. MATERIALS & METHODS: Retrospective observational study of 2 years in the department of respiratory medicine, GMC Bhopal on 280 cases of pleural effusion. RESULTS: Most common etiology was tubercular 202 (72.4%) followed by m

A stepwise approach to the etiologic diagnosis of pleural effusion in respiratory intensive care unit and short-term evaluation of treatment.

BACKGROUND: Pleural effusions in respiratory intensive care unit (RICU) are associated with diseases of varied etiologies and often carry a grave prognosis. This prospective study was conducted to establish an etiologic diagnosis in a series of such patients before starting treatment. MATERIALS AND METHODS: Fifty consecutive patients, diagnosed with pleural effusion on admission or during their stay in RICU, were further investigated by a two-step approach. (1) Etiologic diagnosis was established by sequential clinical history and findings on physical examination, laboratory tests, chest radi

[Thoracic tuberculosis].

The diagnosis and treatment of thoracic tuberculosis follow common principles whatever the localisation. Tuberculosis is diagnosed on clinical signs: night sweats, fever, cough, weight loss, persisting after non-specific treatment, for more than 3 weeks. The demonstration of bacilli in sputum is the keystone of diagnosis, but in extrapulmonary localisations definitive proof is often drawn from a pleural, pericardial or even lymph node biopsy. After diagnosis, or when diagnosis is highly suspected, antituberculous treatment is started. Steroid could be useful in exsudative forms. The overall cu

Pregnancy with bilateral tubercular pleural effusion: challenges.

Pulmonary tuberculosis (TB) during pregnancy mimics some of the physiological changes that occur during pregnancy. Diagnosis is challenging, especially when the patient presents with acute respiratory distress. The incidence of pleural effusion in TB is 3-25% and in the majority of patients, is unilateral. We describe the intensive care management of a 27-year-old pregnant woman admitted to our hospital with life threatening respiratory distress and circulatory shock. She continued to have severe metabolic and respiratory acidosis with shock in spite of the resuscitative measures undertaken. A

Impact of pleural thickness on the sensitivity of computed tomography scan-guided cutting-needle pleural biopsy in diagnosing unexplained exudative pleural effusion.

BACKGROUND: In most cases, patients with pleural effusion require a pleural biopsy to confirm the diagnosis, due to the low diagnostic sensitivity of thoracentesis. Among the different biopsy modalities, real time computed tomography scan-guided cutting-needle pleural biopsy (CT-CNPB) ensures high sensitivity and accessibility. However, there is no study investigating the difference in the diagnostic sensitivity of CT-CNPB for lesions with variable pleural thickness in effusions of different types. METHODS: Of the 303 patients who underwent CT-CNPB, 218 met the eligibility criteria and were r

Evaluation of Clinical and Pathological Outcomes of Medical Thoracoscopy in Southern Iran: Six Years' Experience.

INTRODUCTION: Medical thoracoscopy (MT) is an important diagnostic and therapeutic procedure for pleural diseases, particularly unexplained exudative pleural effusions. Although widely used internationally, data describing MT outcomes from southern Iran remain limited. MATERIAL AND METHODS: This retrospective descriptive study evaluated 93 patients who underwent MT at a large tertiary referral hospital in southern Iran between 2017 and 2023. Data regarding patient characteristics, anesthesia methods, procedural details, complications, and pathological findings were collected and analyzed usin

Pleural and parenchymal radiological characteristics of tuberculous pleuritis and correlation with microbiological and molecular diagnostic yield.

INTRODUCTION: Tuberculous (TB) pleuritis is a common extrapulmonary manifestation of the disease, with significant healthcare burden and morbidity. While there is increasing use of computed tomography (CT) scans for evaluation of patients with suspected TB pleuritis, little is known regarding the correlation of CT findings and respiratory and pleural specimen microbiological yield. METHODS: We performed a retrospective analysis of radiological features and microbiological and molecular yield from different diagnostic modalities in patients with TB pleuritis from January 2021 to December 2024.

Validation of a diagnostic flowchart for tuberculous pleurisy in pleural fluid with high levels of adenosine deaminase.

INTRODUCTION: Adenosine deaminase (ADA) in pleural fluid is a useful marker for diagnosing tuberculous pleurisy. However, recent studies have reported a lower specificity of pleural fluid ADA levels. We previously developed a diagnostic flowchart for patients with pleural fluid ADA ≥40 U/L, incorporating variables such as pleural fluid lactate dehydrogenase <825 U/L, predominant pleural fluid neutrophils or cell degeneration, and a pleural fluid ADA/total protein ratio <14. This flowchart was effective in distinguishing between tuberculous pleurisy and other diseases. Here, we conducted a vali

The impact of ocular tuberculosis on vision after two months of intensive therapy.

Tuberculosis is an infectious disease of global importance with major economic and social burden accounting for 25% of all avoidable deaths in developing countries. Extrapulmonary involvement may occur either in association with clinically apparent pulmonary tuberculosis or in isolation. This cross-sectional descriptive study aimed to evaluate the impact of ocular tuberculosis in visual acuity at baseline and after two months of intensive anti-tuberculous therapy. A sample of 133 pulmonary tuberculosis patients, seven disseminated tuberculosis, and three pleural tuberculosis patients was evalu

Tuberculosis-associated chylothorax revealing an enlarged lymphatic duct due to tuberculosis lymphadenitis.

A 77-year-old woman presented to our hospital with complaints of persistent cough and low-grade fever for two months. On radiological analysis, she had moderate right-sided pleural effusion with right hilar and subcarinal lymphadenopathies. Thoracentesis showed chylothorax of unknown cause. Bronchoscopy revealed a non-specific inflammatory process. However, thoracoscopic surgery demonstrated a curiously enlarged lymphatic duct with its proximal portion compressed by subcarinal lymphadenopathies, pathologically diagnosed as granulomatous lymphadenitis. Hence, tuberculous lymphadenitis was prove
