Vesicocutaneous fistula: continent vesicostomy, an easier route for comfortable clean intermittent catheterization.

INTRODUCTION: Clean intermittent catheterization (CIC) through the urethra is the treatment of choice for patients with neurogenic bladder (NGB) or other etiologies that lead to incomplete bladder emptying. However, urethral catheterization can be problematic. Vesicocutaneous fistula (VCF) is a continent catheterizable channel with a low rate of complications. The aim of the study was to evaluate the safety and effectiveness of VCF as a route for CIC. MATERIAL AND METHODS: The authors retrospectively reviewed patients who underwent creation of the VCF for bladder drainage from November 2001 t

Appropriateness of use of indwelling urinary catheters in patients admitted to the medical service.

Of 836 medical admissions evaluated over a 1-month period, 89 (10.7%) had a urinary catheter placed within 24 hours; 34 placements (38%) had no justifiable indication. Risk for inappropriate catheterization was independent of age, gender, functional status, and mental status at admission. Preventive measures should focus on increasing awareness among healthcare providers.

Remodeled right colon: an alternative urinary reservoir.

A total of 11 patients underwent bladder substitution or continent supravesical diversion with a remodeled right colonic segment as a urinary reservoir. Of the patients 1 woman and 2 men had bladder substitution: in 2 the bladder neck and urethra were preserved, and in 1 man radical cystoprostatectomy with preservation of the distal sphincteric mechanism was performed. All 3 patients currently are continent. In the 8 diversion patients an anti-incontinence mechanism was reconstructed with a segment of plicated ileum sutured to the abdominal wall. Six patients are completely dry between cathete

Complicated catheter-associated urinary tract infections due to Escherichia coli and Proteus mirabilis.

Catheter-associated urinary tract infections (CAUTIs) represent the most common type of nosocomial infection and are a major health concern due to the complications and frequent recurrence. These infections are often caused by Escherichia coli and Proteus mirabilis. Gram-negative bacterial species that cause CAUTIs express a number of virulence factors associated with adhesion, motility, biofilm formation, immunoavoidance, and nutrient acquisition as well as factors that cause damage to the host. These infections can be reduced by limiting catheter usage and ensuring that health care professio

Unsuccessful foley catheterization had an unexpected explanation.

BACKGROUND: Foley catheterization is a simple procedure routinely performed during many obstetric and gynecologic procedures. Failure to adequately drain the bladder with catheter insertion should prompt further investigation to minimize morbidity to the patient. CASE: After repeated attempts to place a Foley catheter during a cesarean section, the urinary bladder did not drain. Postoperatively, it was found that the catheter was positioned inside the left ureter, and cystoscopy confirmed an ectopic ureter inserting into the proximal urethra. CONCLUSIONS: This case presents an unusual cause

Urinary catheters and nosocomial infection.

Latex catheters appear to be associated with a higher incidence of complications than silicone or Silastic tubing. Patients with indwelling catheters in whom asymptomatic bacteriuria develop usually do not require treatment until the time of catheter removal. In symptomatic patients, however, samples of blood and urine should be obtained for culture and sensitivity testing. Empiric therapy with parenteral antibiotics--such as an aminoglycoside plus ampicillin or piperacillin--should then be initiated. To reduce infectious complications associated with the use of urinary catheters, clinicians s

Double-lumen central venous catheter for percutaneous ureteral perfusion studies.

A commercially available double-lumen central venous catheter that can be inserted over a 0.018-inch guide wire was used for ureteral perfusion studies in three infants. Complications or difficulties were not encountered in any patients. This technique allows simultaneous monitoring of perfusion and intrarenal pressure and requires only a single puncture of the renal pelvis with a 21-gauge needle.

[Continent urinary reservoir using dilated renal pelvis of non-functioning pelvic kidney in a girl with cloacal exstrophy].

We performed construction of continent urinary reservoir in an 8-year-old girl with cloacal exstrophy who had double stoma of ileostomy and colon conduit. Preoperative evaluation revealed non-functioning right kidney with severely dilated renal pelvis and calyxes in her pelvis. Urinary reservoir was constructed using detubularized colon segment which had been used as the urinary conduit and dilated renal pelvis of non-functioning pelvic kidney. Using Mitrofanoff's principle, continent catheterizable channel was also made of the anterior wall of the renal pelvis. Postoperative course was uneven

Use of botulinum toxin for genitourinary conditions: What is the evidence?

Botulinum toxin A is licensed for the treatment of urinary incontinence due to neurogenic detrusor overactivity (NDO) and overactive bladder (OAB). Only onabotulinumtoxinA has at this moment such approval, in NDO at a dose of 200 U and in OAB at a dose of 100 U. Regulatory phase 3 trials have been carried out in both conditions. In NDO it was shown to decrease urinary incontinence, to improve urodynamic parameters and to increase quality of life in multiple sclerosis and spinal cord injured patients. Adverse events included urinary tract infections and necessity of de novo clean intermittent c

Non-invasive bladder volume measurement for the prevention of postoperative urinary retention: validation of two ultrasound devices in a clinical setting.

Ultrasound scanning of bladder volume is used for prevention of postoperative urinary retention (POUR). Accurate assessment of bladder volume is needed to allow clinical decision-making regarding the need for postoperative catheterization. Two commonly used ultrasound devices, the BladderScan® BVI 9400 and the newly released Prime® (Verathon Medical®, Bothell, WA, USA), with or without the 'pre-scan' option, have not been validated in clinical practice. The aim of this study was to assess the performance of these devices in daily clinical practice. Between June and September 2016 a prospective

Retrospective analysis of traumatic bladder injury: does suprapubic catheterization alter outcome of healing?

The role of suprapubic catheters in traumatic bladder injuries is not well defined. Current literature suggests that suprapubic catheters are only necessary with large intraperitoneal bladder ruptures. The purpose of this study is to show that all bladder injuries can be managed with transurethral catheterization alone with a similar leak rate, morbidity, and healing time. Retrospective analysis was done of all patients with traumatic bladder injuries at a level I urban trauma center from June 1992 through June 2003. Medical records were reviewed and data analyzed according to type of bladder

Nonsurgical management strategies for the functional complications of ileocolonic continent urinary reservoirs.

Urinary diversion with creation of a continent ileocolonic reservoir was performed in 25 patients with gynecological malignancies at our institution between September 1989 and September 1994. A retrospective review was conducted and cases were analyzed for functional complications associated with reservoir formation. Functional reservoir complications were defined as (1) difficulty with catheterization, (2) reservoir fistulae, (3) reservoir stones, and (4) ureteral stenosis. Management strategies and outcomes for these complications were determined. All patients had received prior pelvic radia

A Case of Female Acute Urinary Retention Presenting to the ED.

INTRODUCTION: Acute urinary retention is a rare occurrence in women necessitating further investigation. Potential underlying causes may be broadly classified into obstructive, neurological, pharmacological, and psychogenic categories. CASE: A 36-year-old nulliparous female presented to the Emergency Department with a two-day history of acute urinary retention. Point-of-care ultrasonography and CT scan imaging confirmed the presence of a large uterine mass causing compression of the bladder. The acute retention was relieved with urethral catheterization. A Uterine leiomyoma was confirmed on h

Bladder catheterization is a common procedure, which surgical patients are undergone in the perioperative period, and the occurrence of about 80% of all urinary tract infections associated with it. There is no postoperative standard tactics of management patients with the urinary catheter. However, implementation of recommendations for the prevention of the development of CAUTI led to a reduction in the i

A randomized clinical trial of how to best position retropubic slings for stress urinary incontinence: Development of a study protocol for the mid-urethral sling tensioning (MUST) trial.

The goal of this trial is to compare two techniques for tensioning retropubic midurethral slings: a Mayo scissor between the tape and urethra vs. a Babcock clamp creating a measured loop underneath the urethra. The primary outcome is a composite of abnormal bladder function at 12 months post surgery. Abnormal bladder function is defined as bothersome stress incontinence or worsening over active bladder symptoms, a positive cough stress test, re-treatment of stress urinary incontinence, post-operative urinary retention requiring either catheterization beyond 6 weeks or surgical intervention.
