Pharmacological treatment of Menière's disease. A review of common remedies with special emphasis on vasodilators, including hyperosmolar substances and hearing.

A review is given of common pharmacological therapies for Menière's disease. The use of vasodilators and hyperosmotic agents is discussed on the bases of clinical and experimental investigations. Osmotic reduction of intralabyrinthine water seems to play a major role in the induced hearing gain but positive effects on the inner ear blood flow cannot be excluded.

Does pulmonary vasoconstriction play an important role in patients with primary pulmonary hypertension? A skeptic's view of vasodilator therapy.

Despite isolated reports of dramatic hemodynamic and clinical improvement over the past 30 years, most patients with primary pulmonary hypertension fail to benefit from treatment with vasodilator drugs and many develop serious adverse reactions. The failure of this approach strongly suggests that pulmonary vasoconstriction does not play an important role in the pathogenesis of this disorder.

Efficacy of short-term, low-dose pulmonary vasodilators for chronic thromboembolic pulmonary hypertension: a retrospective pilot study.

BACKGROUND: Chronic thromboembolic pulmonary hypertension (CTEPH) is treated with pulmonary endarterectomy, pulmonary vasodilators (PVs), and balloon pulmonary angioplasty (BPA). PV therapy may be initiated before BPA; however, dose adjustment can delay BPA. Therefore, short-term, low-dose administration is sometimes used to enable earlier BPA; however, its efficacy and safety remain unclear. METHODS: Twenty patients with World Health Organization functional class III/IV CTEPH, who received low-dose PVs (riociguat or selexipag) and underwent right heart catheterization and first BPA within 60

Intracoronary and tailored vasodilator therapy for refractory ventricular fibrillation storm triggered by multivessel coronary spasm: a case report.

BACKGROUND: Multivessel vasospastic angina (VSA) is a recognized cause of malignant ventricular arrhythmia. Although guideline-directed vasodilator therapy and implantable cardioverter-defibrillators (ICDs) are recommended, some patients remain at risk for refractory ventricular fibrillation (VF). To date, no strategy has been established for these cases. CASE SUMMARY: A 67-year-old man experienced a VSA-related ST-elevation myocardial infarction complicated by VF, leading to subcutaneous ICD implantation. The patient remained stable for 1 year on medications without non-adherence or device t

Vasodilator activity of methanol extract obtained from Crataegus rosei leaves and the chemical characterization of its main constituents.

Persistent arterial hypertension (AHT) plays a very important role in the development of cardiovascular diseases (CVD). The present study assessed the vasodilator activities of the methanol extract (ME) and its fractions obtained from leaves of a Mexican Crataegus rosei plant. The fat- and tannin-free ME fraction (FTFME) produced the most potent vasodilator effect [EC50 8.253 (4.672 to 14.58 μg/mL)], that was 6.7-fold higher than that of the ME [EC50 = 55.49 (29.78 to 68.69) μg/mL], which was similar to that of ACh used as a positive control. This effect was dependent on the nitric oxide/cGMP

Mechanisms and Prognosis of Intolerance to Angiotensin Receptor-Neprilysin Inhibitors in Advanced Heart Failure: Insights From Vasodilator Challenge.

BACKGROUND: Angiotensin receptor-neprilysin inhibitor (ARNI) intolerance is common in patients with advanced heart failure (AdHF) and may be associated with worse prognosis. During right heart catheterization (RHC), afterload reduction induced by vasodilator challenge may reproduce the hemodynamic effects of ARNI. Through sodium nitroprusside (NTP) infusion, we characterized the hemodynamic mechanisms of ARNI intolerance and explored its prognostic relevance in AdHF. METHODS: We performed a retrospective, multicenter study evaluating AdHF patients undergoing RHC with NTP infusion. Hemodynamic

The Use of Pulmonary Vasodilators in Pediatric Patients with Single-Ventricle Palliation: A Ten-Year Experience in a Tertiary Care Center.

In single-ventricle circulation, pulmonary vascular resistances (PVR) play a crucial role at various stages of surgical palliation. Increased PVR detected at cardiac catheterization represents a contraindication to Fontan completion and may lead to an early circuit failure in the postoperative period. Pulmonary vasodilator therapy (PVT) may lower PVR and enhance pre- and post-surgical outcomes in Fontan patients. This study reports the experience with the use of PVT in a tertiary center, focusing on its role in lowering PVR before Fontan procedure and assessing its impact on postoperative outc

Impact of proning with and without inhaled pulmonary vasodilators and neuromuscular blocking agents in COVID acute respiratory distress syndrome.

BACKGROUND: A major cause of mortality in the coronavirus disease 2019 (COVID-19) pandemic was acute respiratory distress syndrome (ARDS). Currently, moderate to severe ARDS induced by COVID-19 (COVID ARDS) and other viral and non-viral etiologies are treated by traditional ARDS protocols that recommend 12-16 hours of prone position ventilation (PPV) with neuromuscular blocking agents (NMBA) and a trial of inhaled vasodilators (IVd) if oxygenation does not improve. However, debate on the efficacy of adjuncts to PPV and low tidal volume ventilation persists and evidence about the benefits of IV

Association of Cardio-Ankle Vascular Index With Tonometric Measures and Vasodilator Function Across a Spectrum of Cardiovascular Risk Burden.

BACKGROUND: Arterial stiffness and endothelial dysfunction are two important features of cardiovascular injury. Arterial stiffness can be measured by Pulse Wave Velocity (PWV) and endothelial dysfunction can be assessed with reactive hyperemia measured by flow-mediated dilation (FMD). Cardio-Ankle Vascular Index (CAVI) is a recently developed method for measuring arterial stiffness. Studies assessing CAVI's association with established tests of arterial stiffness and endothelial function are limited. METHODS: In a cross-sectional study of adults (ages 18-80) with a range of cardiovascular dis

Effects of vasodilators on beat-to-beat and every fifteen minutes blood pressure variability induced by noradrenaline infusion in rats.

Increased blood pressure variability (BPV) was shown to be associated with cardiovascular morbidities and/or mortalities. There are various types of BPV depending on time intervals of BP measurements, ranging from beat-to-beat to visit-to-visit or year-to-year. We previously found that continuous infusion of noradrenaline (NA) for 14 days increased short-term BPV every 15 min in rats. The aims of this study were to examine (1) whether NA infusion increases very short-term beat-to-beat BPV, (2) the effects of azelnidipine and hydralazine on NA-induced BPV, and (3) whether baroreceptor reflex se

Vasodilating Effects of Antispasmodic Agents and Their Cytotoxicity in Vascular Smooth Muscle Cells and Endothelial Cells-Potential Application in Microsurgery.

This study aimed to elucidate the vasodilatory effects and cytotoxicity of various vasodilators used as antispasmodic agents during microsurgical anastomosis. Rat smooth muscle cells (RSMCs) and human coronary artery endothelial cells (HCAECs) were used to investigate the physiological concentrations and cytotoxicity of various vasodilators (lidocaine, papaverine, nitroglycerin, phentolamine, and orciprenaline). Using a wire myograph system, we determined the vasodilatory effects of each drug in rat abdominal aortic sections at the concentration resulting in maximal vasodilation as well as at …

Augmentation of perfusion with simultaneous vasodilator and inotropic agents in experimental acute middle cerebral artery occlusion: a pilot study.

BACKGROUND: This study tests the hypothesis that simultaneous cerebral blood pressure elevation and potent vasodilation augments perfusion to ischemic tissue in acute ischemic stroke and it varies by degree of pial collateral recruitment. METHODS: Fifteen mongrel canines were included. Subjects underwent permanent middle cerebral artery occlusion; pial collateral recruitment was scored before treatment. Seven treatment subjects received a continuous infusion of norepinephrine (0.1-1.52 µg/kg/min; titrated 25-45 mmHg above baseline mean arterial pressure while keeping systolic blood pressure b…

Muscle vasodilator response via potential adrenaline secretion to L-cysteine microinjected in rostral ventrolateral medulla of rats.

We previously found that the thiol amino acid L-cysteine microinjected into rat medullary autonomic areas produces changes in arterial blood pressure (AP) via ionotropic excitatory amino acid receptors (iEAAr), but its effect on vascular beds is still unknown. Rostral ventrolateral medulla (RVLM) pressor area includes adrenal and lumbar presympathetic neurons which activation could cause opposite muscle vascular responses: vasodilation versus vasoconstriction. However, there are few data on the vasodilator response in rats. Ionotropic EAAr activation alone with L-cysteine may be effective to d…

Long-term efficacy of vasodilating β-blocker in patients with acute myocardial infarction: nationwide multicenter prospective registry.

BACKGROUND/AIMS: Long-term benefit of vasodilating β-blockers is unknown. This study aimed to investigate the long-term benefit of vasodilating β-blockers over conventional β-blockers in patients with acute myocardial infarction (AMI). METHODS: Using nationwide prospective multicenter Korean Acute Myocardial Infarction Registry data, we analyzed 3-year clinical outcomes of 7,269 patients with AMI who received percutaneous coronary intervention (PCI) and β-blocker therapy. Patients were classified according to treatment strategy (vasodilating β-blockers vs. conventional β-blockers). The primar

Inhaled nitric oxide has pulmonary vasodilator efficacy both in the immediate and prolonged phase of acute pulmonary embolism.

BACKGROUND: Inhaled nitric oxide (iNO) effectively reduces right ventricular afterload when administered in the immediate phase of acute pulmonary embolism (PE) in preclinical animal models. In a porcine model of intermediate-risk PE, we aimed to investigate whether iNO has pulmonary vasodilator efficacy both in the immediate and prolonged phase of acute PE. METHODS: Anesthetized pigs (n = 18) were randomized into three subgroups. An acute PE iNO-group (n = 6) received iNO at 40 ppm at one, three, six, nine and 12 hours after onset of PE. Vehicle animals (n = 6) received PE, but no active tre

Management of ischemic tissues and skin flaps in Re-Operative and complex hypospadias repair using vasodilators and hyperbaric oxygen.

INTRODUCTION: Repeated and multiple surgeries for hypospadias result in varying degrees of scarring and hypovascularity of penile tissues which may result in poor healing and increasing complication rates with each additional repair. Vasodilator agents such as Nitroglycerine (NTG) can be helpful in the immediate postoperative period to improve flap viability. Hyperbaric oxygen therapy (HBOT) has well-established benefits to promote angiogenesis and wound healing. We hypothesized that NTG and HBOT, methods to promote blood flow and wound healing, would provide better outcomes in redo hypospadia

Requisite roles of LOX-1, JNK, and arginase in diabetes-induced endothelial vasodilator dysfunction of porcine coronary arterioles.

Diabetes is associated with cardiac inflammation and impaired endothelium-dependent coronary vasodilation, but molecular mechanisms involved in this dysfunction remain unclear. We examined contributions of inflammatory molecules lectin-like oxidized low-density lipoprotein receptor-1 (LOX-1), stress-activated kinases (c-Jun N-terminal kinase [JNK] and p38), arginase, and reactive oxygen species to coronary arteriolar dysfunction in a porcine model of type 1 diabetes. Coronary arterioles were isolated from streptozocin-induced diabetic pigs and control pigs for vasoreactivity and molecular/bioc…
