BACKGROUND: The magnitude of the association between fibromuscular dysplasia (FMD) and spontaneous cervical artery dissection (sCeAD) remains uncertain, since data available derive from uncompleted vascular screening. This study aims to assess the frequency and types of cervical and extra-cervical arterial lesions, particularly of the FMD type, in patients with sCeAD.
METHODS: We recruited all patients with a diagnosis of sCeAD from January 2016 to December 2023. All patients underwent full-body vascular imaging to assess the presence of cervical and extra-cervical multifocal and focal stenos

We herein report a 5-day-old baby boy presented with a massive cerebellar hemorrhage due to suspected posterior inferior cerebellar artery (PICA) dissection. He was born by vacuum extraction at the gestational age of 41 weeks with 3370 g birth weight. On the fifth day of life, he developed dyspnea with worsening vital signs. CT of the head showed massive cerebellar hemorrhage and then transferred to our hospital. External ventricular drainages were emergently placed for his hydrocephalus on the first day of hospitalization, then cerebellar hematoma was evacuated with suboccipital craniotomy on

Although spontaneous cervical artery dissection (SCAD) is generally a rare contributor to a stroke, this condition triggers a considerable percentage of the strokes that are observed in young to middle-aged patients. We herein report the findings of a patient who presented with a stroke and a severe headache. A diagnosis of SCAD was made following a series of examinations. The patient had high-grade stenosis in the cervical artery and received carotid angioplasty along with stenting. A diagnosis of SCAD should be suspected if a patient who is less than 50 years of age presents with a stroke an

The Klippel-Feil syndrome is one of the spinal malformations characterized by fusion of the cervical vertebrae. It is well known that the malformation can cause some neurologic disorders. However, an aneurysm in the vertebral artery associated with the Klippel-Feil syndrome is extremely rare, with only one case having been reported in the literature. We present a 39-year-old female with a sudden onset of disturbed consciousness. Lateral cervical x-ray films showed the Klippel-Feil syndrome and hypermobility between C1 and C2. MRI showed multiple infarctions in the posterior circulation, includ

A patient with a fusiform aneurysm of the cervical vertebral artery at C-1 presenting with posterior circulation infarctions is reported. Exposure via the posterolateral approach, resection of the aneurysm, and end-to-end anastomosis are described. The authors emphasize the importance of the surgical approach and ability to expose and mobilize a generous portion of the cervical vertebral artery.

Intracranial artery dissection secondary to autosomal dominant polycystic kidney disease is far less common than cerebral aneurysm. A 55-year-old man presented a sudden onset of headache and disturbed consciousness caused by ischemic stroke in the bilateral frontal lobes with minor subarachnoid hemorrhage. The bilateral anterior cerebral arteries were firstly occluded and re-perfused with irregular narrowing and dilation in 3 days after stroke onset, indicating dissection. He was diagnosed with autosomal dominant polycystic kidney disease by abdominal CT findings and by his family history thou

Left subclavian artery revascularization during endovascular repair of aortic dissection is often accomplished by left carotid-subclavian artery bypass or transposition. In situ fenestration of thoracic stent grafts provides an alternative method of revascularization without manipulation of the left carotid artery. We describe a case whereby in situ laser fenestration, combined with catheter-directed thrombectomy, was utilized to revascularize a thrombosed left subclavian artery following a frozen elephant trunk repair of type A aortic dissection. A 75-year-old male presented with pericardial

We report the forensic and clinicopathological spectrum of 14 postmortem cases involving the vertebral artery. In all cases, there was either pontocerebellar infarction (n = 8) or subarachnoid hemorrhage (n = 6). The underlying pathology of the vertebral artery was segmental mediolytic arteriopathy (n = 5), traumatic rupture of the arterial wall (n = 3), arterial dissection (n = 2), or atherosclerosis (n = 4). Histopathologic changes were often present in both the intracranial and extracranial segments of the vertebral artery. In our case series, the most frequent disease in the vertebral arte

OBJECTIVE: The aim of this study was to evaluate the anatomic details and therapeutic implications of the double origin of the posteroinferior cerebellar artery (DOPICA).
METHODS: A total of 131 patients with intracranial posterior circulation aneurysms were consecutively treated using endovascular techniques during an 8-year period. We retrospectively searched the angiographies of these patients for evidence of DOPICA and evaluated its anatomic and treatment details and outcomes in cases of concurrent vertebral artery dissection.
RESULTS: DOPICA was found in eight patients. Five patients ha…

A 51-year-old man presented with an extremely rare case of intracranial subarachnoid hemorrhage caused by rupture of an anterior spinal artery aneurysm manifesting as disturbance of consciousness following sudden onset of neck pain and numbness of the extremities. Cranial computed tomography revealed subarachnoid hemorrhage, mainly in the posterior fossa. Cerebral angiography studies on admission and on the 4th day demonstrated no definite abnormality as a bleeding source. A ventricular catheter was inserted to treat the acute hydrocephalus, and conservative management was continued during the

The natural history of spontaneous cerebral artery dissection and thrombosis remains uncertain. Concurrent subarachnoid hemorrhage further complicates the therapeutic approach. Thus the best strategy for managing patients with acute vessel thrombosis in the setting of subarachnoid hemorrhage is unclear. Here we present a case of spontaneous posterior inferior cerebellar artery dissection presenting with subarachnoid hemorrhage and acute thrombosis. Although the patient was initially managed conservatively, angiographic follow-up demonstrated recanalization of the diseased vessel, necessitating

Carotid arterial dissections may result from spontaneous or traumatic causes. Postcoital arterial dissections have been reported in both the vertebral and coronary arteries. We report a rare case of spontaneous dissection from the common carotid artery to the internal and external carotid arteries after sexual intercourse, leading to fulminant middle cerebral artery (MCA) territory infarct. Although we restored the flow in the proximal carotid artery with stenting, brain swelling with hemorrhagic transformation was still noted in the region of the right MCA.
