Rotational vertebral artery occlusion syndrome with vertigo due to "labyrinthine excitation".

Leftward head rotations in a patient with a rotational vertebral artery occlusion syndrome elicited recurrent uniform attacks of severe rotatory vertigo and tinnitus in the right ear. These attacks were accompanied by a mixed clockwise torsional downbeat nystagmus with a horizontal component toward the right. A transient ischemia of the right labyrinth probably induced the attacks and led to a combined transient excitation of the right anterior and horizontal semicircular canals as well as the cochlea.

Vertigo as manifestation of vertebral artery dissection after chiropractic neck manipulations.

We recently observed a case of vertebral artery (VA) dissection following chiropractic neck manipulations. The first manifestation was unusual; in the form of vertigo. Therefore, the patient was referred to the otoneurologist. A VA dissection should be suspected in a case of vertigo following chiropractic neck manipulations, and vestibular tests should be done carefully, avoiding Rose's positions. In our case, the diagnosis was based on Doppler sonography and angiography. Chiropractic neck manipulations are the most frequent cause of traumatic VA dissection. Practitioners should be aware that,…

Hearing results and control of vertigo after retrolabyrinthine vestibular nerve section.

To determine the effect of retrolabyrinthine vestibular nerve section (RVNS) on hearing, vertigo, and associated symptoms, we reviewed our experience in 48 patients. Of the 48, 39 responded to a questionnaire. Although RVNS appeared to have little effect on hearing in Meniere's patients, 91% of non-Meniere's patients had significant and often delayed postoperative sensorineural hearing loss. Our results for vertigo control compared favorably to previous reports with 96% of Meniere's patients and 69% of non-Meniere's patients reporting improvement. Presently, we more frequently recommend RVNS a…

Integrating machine learning with QSPR for property prediction of anti-vertigo drugs.

Vertigo is a debilitating vestibular disorder affecting approximately 10-30% of the global population, yet no dedicated computational chemistry study has systematically characterised the properties of its pharmacological agents using machine learning. In this study, we present the first Quantitative Structure-Property Relationship (QSPR) analysis of 47 clinically established anti-vertigo drugs using nine Neighborhood degree-based topological indices, combined with three machine learning algorithms such as Ridge Regression, Random Forest (RF) and Gradient Boosting Regression (GBM). The topologi…

ChatGPT and patient education in benign paroxysmal positional vertigo, in French and in English.

OBJECTIVE: ChatGPT has gained popularity for its user-friendly applications in medicine, including otolaryngology. However, concerns have surfaced about accuracy of information, especially in languages other than English. This study assessed ChatGPT's proficiency in answering questions in English and French about benign paroxysmal positional vertigo (BPPV), a common ENT presentation. We aim to address concerns regarding its performance with medical queries in a language beyond its primary training set. METHODS: ChatGPT 3.5 was tested with questions taken from the guidelines of the American Ac…

Profile and postrehabilitation outcome of patients with vertigo in a clinic in Southern Santa Catarina.

For the purposes of the present study, we define vertigo as the sensation of rotation of the environment or of the person moving. The demand for care for this complaint has been increasing; therefore, it is important to know the profile of patients treated with this complaint, the causes, the comorbidities, the associated factors, and the response of these individuals to rehabilitation. To analyze the epidemiological profile of patients reporting vertigo and their outcome after physiotherapeutic rehabilitation. We conducted a descriptive cross-sectional study of secondary data analysis. The samp…

The postoperative relief of vertigo and migraine in Meniere's disease patients with and without migraine.

BACKGROUND: The correlation of Meniere's disease (MD) and migraine remains unclear. OBJECTIVES: To explore the relation of MD and migraine by analyzing the postoperative relief of vertigo and migraine in MD patients with migraine. MATERIAL AND METHODS: 104 MD patients were enrolled. The monthly vertigo episodes (MVE) and migraine headache index (MHI) were used for assessing effectiveness at pre- and post-endolymphatic sac decompression (ELSD). First, patients were divided into MD with migraine and MD two groups with vertigo relief rates compared. Then, the postoperative relief of vertigo and…

Assessing the impact of migraine on benign paroxysmal positional vertigo symptoms and recovery.

BACKGROUND: During episodes of benign paroxysmal positional vertigo (BPPV), individuals with migraine, compared with individuals without migraine, may experience more severe vestibular symptoms because of their hyperexcitable brain structures, more adverse effects on quality of life, and worse recovery processes from BPPV. METHODS: All patients with BPPV were assigned to the migraine group (MG, n = 64) and without migraine group (BPPV w/o MG, n = 64) and completed the Vertigo Symptom Scale (VSS), Vertigo Dizziness Imbalance Symptom Scale (VDI-SS), VDI Health-Related Quality of Life Scale (VDI…

Simultaneous, spontaneous and multivessel cervical artery dissection presenting with headache and vertigo.

Cervical artery dissection (CAD) is a relatively common cause of stroke, particularly in young adults. Typical presentation includes headache, neck pain and neurological signs of ischaemic stroke. More than half of these cases are spontaneous, without a preceding history of trauma, and most commonly affect a single vessel. We report a case of a man in his 20s presenting with spontaneous, non-traumatic multivessel CAD (triple vessel, quadruple dissection), bilateral cerebellar infarcts and concomitant basilar artery thrombus. Triple or quadruple vessel CAD is seldom reported in the absence of t

The importance of Epley maneuver in posterior canal benign paroxysmal positional vertigo.

OBJECTIVE: We investigated symptom scores and quality of life in unilateral posterior canal benign paroxysmal positional vertigo (BPPV) patients. PATIENTS AND METHODS: In this retrospective and multicentric study, 78 patients with unilateral posterior canal BPPV (47 right-sided and 31 left-sided) were included. All patients have performed the Standard Epley maneuver. Features of the nystagmus [nystagmus duration (second), latent period (second)] and features of the disease [side (right or left-sided), disease duration (years), and recurrence of disease (present or absent)] were noted. Before …

Association Between Multiple Sclerosis and the Symptoms of Vertigo and Facial Nerve Palsy.

Patients with vertigo and facial nerve palsy as initial symptoms are rarely diagnosed with multiple sclerosis. A 43-year-old woman presented to our department with symptoms of vertigo and right facial nerve palsy (Yanagihara 16-point system [total score, 40] or House- Brackmann grade IV [obvious facial weakness]). On the day of the visit, she presented with right eye abduction, left eye adduction, and complaints of diplopia. Based on magnetic resonance imaging findings, she was diagnosed with clinically isolated syndrome, which is an early manifestation of multiple sclerosis. She was treated w…

A Patient With (Initially) Non-Persistent Vertigo - A Posterior Circulation Stroke Case.

Posterior circulation strokes are responsible for about 20-25% of all ischemic strokes. Recognition and diagnosis of posterior circulation strokes or transient ischemic attacks is more difficult than that of other stroke types, being frequently misdiagnosed in acute setting/emergency evaluation - up to more than three times as often as anterior circulation strokes. In accordance with the guidelines in effect at the time of this case presentation, thrombolysis and medical therapy are the mainstay treatment. Mechanical thrombectomy is emerging as a treatment option in posterior vessel occlusion
