Vestibular evoked myogenic potentials in patients with acoustic neuromas.

BACKGROUND: To diagnose acoustic neuromas (ANs), the auditory brainstem response test and the caloric test have been used in addition to magnetic resonance imaging. The auditory brainstem response and the caloric tests mainly reflect functions of the auditory pathway, ie, the cochlear nerve and the superior vestibular nerve, respectively. Because the vestibular evoked myogenic potential (VEMP) has been thought to originate in the inferior vestibular nerve, we hypothesized that the VEMP could provide different information from the auditory brainstem response and the caloric test and that it cou

Sensitivity of auditory brainstem response in acoustic neuroma screening.

Auditory brainstem response (ABR) is the reference screening technique for acoustic neuromas, but because of a few false negatives and the increasing performance of magnetic resonance imaging (MRI), its role as the standard method has been questioned. We assessed sensitivity of screening tests in 89 patients with surgically proven acoustic neuromas. Sensitivity of ABR was 92%; 94% for extracanalicular neuromas and 77% for intracanalicular neuromas. For stapedius reflex (SR), sensitivity was 84% and for caloric vestibular response (CVR) 86%. The combined sensitivity of ABR + SR was 97% and of A

Regrowth of the residual tumour after acoustic neurinoma surgery.

Ideally, acoustic neurinomas should be completely removed without neurological deficit. In some cases, however, removal is subtotal in order to preserve anatomical continuity of the facial nerve. In this report we discuss the regrowth of residual tumour in these patients and the difference between regrowing and dormant tumours. Eight of 81 patients with acoustic neurinomas where the facial nerve was anatomically preserved leaving tumour overlying it were investigated and followed-up. Only one case with residual tumour along the 7th nerve from the intrameatal portion to its entry to the brainst

Use of the KTP-532 laser in acoustic neuroma surgery.

The potassium titanyl phosphate (KTP-532) laser has been applied to otologic surgery with a proven record of both safety and efficacy. The aim of this study was to demonstrate the use, safety, and advantages of laser dissection in the surgical treatment of acoustic neuromas. The authors' experience with 111 patients in whom laser surgery was used in acoustic neuroma is presented, with emphasis on surgical technique employed and facial nerve functional outcome. The method of laser dissection did not result in deleterious neurologic sequelae or laser-specific complications. In addition, laser di

Acoustic neuroma: potential benefits of fractionated stereotactic radiosurgery.

BACKGROUND: Single-fraction radiosurgery of acoustic neuromas less than 3 cm in diameter is remarkable for high control but not infrequent incidence of facial and trigeminal neuropathy. Larger tumors treated surgically often result in deafness and facial neuropathy. Fractionated stereotactic radiosurgery was used in an effort to maintain effective therapy while minimizing toxicity of treatment. METHODS: The authors described 38 patients with acoustic neuromas, with age range 35-89 years (mean, 60 years). 2,000 cGy in divided weekly doses of 400 or 500 cGy was most commonly prescribed. Tumors

Management of the contracted mastoid in the translabyrinthine removal of acoustic neuroma.

Several approaches to the internal auditory canal and cerebellopontine angle for acoustic neuroma removal have been described. We prefer the translabyrinthine approach in patients with tumors larger than 2 cm or poor preoperative hearing, since both factors predict poor hearing preservation. Many surgeons perceive this approach as confining and consider it contraindicated in large tumors or contracted mastoids. We have recently described the utility of the translabyrinthine approach for the removal of large (> 4 cm) acoustic neuromas. In more than 5000 tumor excisions performed by the senior a

Some uncharacteristic clinical signs and symptoms of acoustic neuroma.

OBJECTIVE: The most common initial symptoms of the acoustic neuroma are unilateral hearing loss that evolves gradually, tinnitus, and unsteadiness. However, atypical presentations may sometimes occur, more often with a small intracanalicular neuroma or with a large medial neuroma placed in the cerebellopontine angle. RESULTS: In our group of 51 patients suffering from acoustic neuroma, atypical presentations were observed in 9 cases (17.6%). Two patients had normal hearing function but reported tinnitus; two patients reported sudden hearing loss, with partial recovery; two patients had a hist

Monitoring the electrode position during acoustic neuroma surgery.

OBJECTIVE: To obtain information about the auditory brain stem responses during auditory brain stem implantation. SETTING: Operating room during acoustic neuroma surgery. METHODS: Electrical stimulation of the auditory system during acoustic neuroma surgery, by placement of a monopolar or bipolar electrode on the nerve or nerve entry zone of the brain stem, and monitoring of the evoked auditory brain stem responses (EABR) recorded from the scalp. In some patients, a multichannel silicon electrode array was placed at the foramen of Luschka. Biphasic rectangular current pulses were applied, an

The acoustic reflex in adults with histories of otitis media in childhood.

OBJECTIVE: To investigate the effects of a history of otitis media (OM) in childhood on the acoustic reflex threshold (ART) in young adults. DESIGN: Questionnaire responses on childhood ear and hearing problems were obtained from populations of university students. In Study 1, 31 students reporting histories of persistent childhood OM and 34 students with no known OM histories were identified. They received pure-tone audiometry, otomicroscopy, and measurement of ARTs for a 500 Hz pure tone. In Study 2, 20 students with OM histories and 20 students with no known OM histories received pure-tone

[Acoustic hallucinations as a symptom of a REM sleep-associated parasomnia].

This 52-year-old man suffered from auditory hallucinations that occurred during brief episodes of sleep paralysis at the end of REM sleep periods. During these episodes the patient experienced a dissociated state of consciousness with REM sleep intrusions into wakefulness. The occurrence of this mixed state, and of excessive sleep-onset REM periods during daytime polysomnography (MSLT = Multiple Sleep Latency Test), point to a disorder of REM sleep generation. The existence of narcolepsy could be ruled out. The observation of REM sleep-associated hallucinations has been reported earlier. In th

Mechanisms of auditory impairment during acoustic neuroma surgery.

Hearing loss during removal of acoustic neuroma (AN) may be due to labyrinthine and/or neural and/or vascular damage. Surgical maneuvers relating to perioperative and postoperative hearing may give rise to mechanisms of auditory impairment. Recording action potentials from the intracranial portion of the cochlear nerve (CN) has proven particularly useful for identifying the mechanisms of iatrogenic auditory injury. In this paper intraoperative and postoperative auditory impairments are investigated in relation to surgical steps in a group of 47 subjects with AN (size ranging from 5 to 25 mm) u

Detection of the acoustic reflex below 80 dB HL.

A new method for detecting the acoustic reflex that utilizes standard otoacoustic emissions recording techniques is introduced and discussed. Two successive identical tone bursts of 100 ms duration and 10 ms interstimulus interval are presented in the occluded ear canal at a repetition rate of one per second. If the acoustic reflex is elicited, the contraction of the stapedius muscle is delayed with respect to the onset of the first stimulus. Hence, the acoustic compliance in the ear canal decreases primarily during the second stimulus. The difference of the microphone signals produced by the

Hemorrhage and abnormal veins in acoustic neurinoma: MR findings.

We reviewed the MR imaging findings of 57 acoustic neurinomas which were verified at surgery or diagnosed on the basis of neuroradiological and neurootological data. Two uncommon MR findings of acoustic neurinoma were found. First, hypointense areas were observed on T2-weighted images in five of the 12 tumors larger than 25 mm in diameter. These hypointense areas represented hemosiderin deposition secondary to occult intratumoral hemorrhage. Second, curvilinear or round signal voids were noted at the periphery of 11 large or medium-sized tumors, and these corresponded to "abnormal veins" seen

Acoustic neurinoma with a large organized hematoma--case report.

A 66-year-old female presented with a large organized hematoma within an acoustic neurinoma. She had suffered from diminished hearing for 20 years and had headache 1 week before presentation. Computed tomography demonstrated an inhomogeneously high density cerebellopontine angle mass, and magnetic resonance imaging showed a mass with heterogeneous intensity and gadolinium-diethylenetriaminepenta-acetic acid enhancement of only the peripheral surface of the mass and the inner parts of the internal auditory meatus. At operation the majority of the mass was soft and feature-less with a firm capsu

MRI in the detection of acoustic neuromas--a suggested protocol for screening.

Magnetic resonance imaging (MRI) is the definitive investigation in the detection of an acoustic neuroma and its use is becoming increasingly widespread for this purpose. In the Oxford region, this facility has been available for 3 years, resulting in the scanning of the internal auditory meati and cerebellopontine angles of 892 patients and the detection of 38 acoustic neuromas. Based on our use of this investigation over this time period, we have attempted to rationalize our entry protocol for screening by limiting the availability of the test to those patients up to 70 years of age presenti

Acoustic neuroma: correlations between morphology and otoneurological manifestations.

Forty-two patients with acoustic neuroma (AN) were studied to determine whether different types of neuroma could be correlated with specific signs and symptoms of the disease. Based on gadolinium-enhanced TI-weighted MRI sequences, the 42 cases of AN could be divided into three groups, either by size (small: 11.9%, medium: 50%, and large: 38.1%) or by site of origin of the tumour (lateral: 16.7%, intermediate: 69%, and medial: 14.3%). Relations were found between the size and the site of origin of the neuromas and certain clinical, audiological and vestibular findings. The clinical presentatio
