Ocular topical aesculetin delivery via VA64/TPGS co-assembled micelles for redox-inflammatory modulation in selenite-induced cataract.

Cataract progression is closely associated with oxidative stress and inflammatory activation, while effective non-surgical interventions remain limited. Aesculetin (AES) possesses antioxidant and anti-inflammatory activities, but its topical ophthalmic application is restricted by poor aqueous solubility, insufficient dispersion stability, and limited local availability. Here, AES-loaded VA64/TPGS co-assembled micelles (AES@V-T) were developed as a topical ophthalmic delivery system. The optimized AES@V-T showed a mean particle size of 13.10 ± 0.16 nm, a narrow polydispersity index of 0.094 ±

Bleeding, Drains, and Reoperation in Transmasculine Chest Surgery: What 2,534 Patients Tell Us About Tranexamic Acid.

BACKGROUND: Hematoma, seroma, and unplanned reoperation are the most frequent early complications after gender-affirming mastectomy with hematoma being the most common complication and the most common reason for reoperation. Tranexamic acid (TXA) is increasingly incorporated into enhanced-recovery protocols, yet its route-specific benefits remain unclear. METHODS: In accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, databases were searched for comparative studies of TXA versus no TXA in transmasculine chest surgery. Review Manager 5.4 was used for

Effectiveness of topical sodium thiosulfate for ectopic calcifications and ossifications. Results of the CATSS-O study.

INTRODUCTION: Ectopic calcifications (ECs) and heterotopic ossifications (HOs) form in non-mineralized tissues, most often in subcutaneous and muscular areas. Local and systemic complications can cause severe disability. Systemic administration of sodium thiosulfate (STS) gives promising results but is difficult to use in clinical practice. OBJECTIVE: Evaluation of the efficacy and safety of topical STS in ECs and HOs. METHODS: Retrospective analysis of the CATSS-O registry that included patients receiving topical STS 25 % prepared by the pharmacy of Limoges hospital during 2014-2020. The ef

NSAID use among residents in 68 residential aged care facilities 2014 to 2017: An analysis of duration, concomitant medication use, and high-risk conditions.

PURPOSE: People in residential aged care are at increased risk of adverse events from nonsteroidal anti-inflammatory drugs (NSAIDs) due to their age and health status, but little is known about use of NSAIDs in this setting. We aimed to estimate the prevalence of NSAID use by route, differences by high-risk conditions, prevalence of concurrent proton pump inhibitor (PPI) use, and prevalence of the "triple whammy" combination (oral NSAID, diuretic, and angiotensin-converting-enzyme inhibitor or angiotensin receptor antagonist). METHODS: We conducted a dynamic cohort study using medication admi

The pharmacology of a novel topical retinoid, BMY 30123: comparison with tretinoin.

Preclinical studies pertaining to the pharmacology and toxicology of BMY 30123 (4-acetamidophenyl retinoate) are reported. BMY 30123 is a novel compound which has topical retinoid activity. This compound exhibits lower toxicity, both local and systemic, than other clinically used topical retinoids such as tretinoin (all-trans retinoic acid) in animal models. BMY 30123 is effective in a number of retinoid sensitive skin models including the rhino mouse utriculi reduction assay, the mouse epidermal hyperplasia model and in the suppression of DNA synthesis in mouse skin stimulated with phorbol es

Preoperative Testosterone Therapy Prior to Surgical Correction of Hypospadias: A Review of the Literature.

Hypospadias is a congenital anomaly of the male urethra that causes significant morbidity in the pediatric male population. The mainstay of treatment is hypospadias repair surgery. However, given the common co-occurrence of microphallus in these patients, surgery remains difficult without adequate tissue for proper reconstruction of the urethra. The use of preoperative testosterone therapy, parenterally or topically, has been a topic of study for several years in urologic literature. This literature review aims to summarize the different studies that have been conducted to address whether preo

Does Combined Intravenous and Topical Tranexamic Acid Offer Additional Benefit Compared With Intravenous or Topical Alone in Open Elbow Arthrolysis-A Pilot Randomized Controlled Trial.

PURPOSES: Open elbow arthrolysis (OEA) is a common method for treating post-traumatic elbow stiffness, which may lead to severe bleeding when resecting heterotopic ossification (HO) and fibrous scar tissue. The purpose of this study is to compare the efficacy and safety among three different ways of using tranexamic acid (TXA) in reducing total blood loss and drainage volume in OEA. PATIENTS AND METHODS: This was a pilot, double-blinded, randomized controlled trial involving a total of 60 patients of post-traumatic elbow stiffness. Patients in the IV group (n = 20) received TXA intravenously,

Topical sirolimus in dermatology: a systematic review.

Topical sirolimus is increasingly utilized off-licence to manage various dermatological conditions while avoiding typical adverse effects associated with systemic sirolimus. However, widespread use is limited by a highly heterogeneous evidence base of mixed quality. Our aim was to evaluate the current evidence base for the indications, efficacy and safety profile of topical sirolimus in dermatology. A literature search was conducted from 2005 to 4 July 2023, of English-language studies, with the following databases consulted: MEDLINE, PubMed, Embase, CENTRAL and EBSCO. Key words included 'topi

Recent advances in therapies for onychomycosis and its management.

Onychomycosis is the most common affliction of the nail. It may be caused by dermatophytes, yeasts, and non-dermatophyte molds. Traditionally, oral antifungal treatments have been used to treat the fungus, although they can be accompanied by side effects and drug interactions. Topical treatments provide an alternative modality, bypassing the systemic effects of oral drugs; recent research has centered on topical drug improvement and development. Physical and laser treatments are being used in conjunction with topicals, which may help penetrate the thick nail plate. In this review, techniques f

Antifungal therapy for onychomycosis in children.

Onychomycosis is a chronic infection of the nail unit, and its prevalence increases with age. Treatment options for children are similar to those for adults and include both oral and topical therapies. Oral agents, such as terbinafine, itraconazole, and fluconazole have been reported to have good efficacy and a low rate of side effects in children. Topical therapies, such as amorolfine and ciclopirox, can also be used as monotherapy or combined with oral agents to treat onychomycosis. Due to their thinner, faster-growing nails, children are more likely to respond to topical monotherapy than ad

[Place of immunosuppressors in atopic dermatitis].

Use of immunosuppressors in the treatment of atopic dermatitis is an important innovation that reinforces the therapeutic arsenal in this chronic disease. Two such drugs are used topically for the treatment of atopic dermatitis. Tacrolimus exists in pommade form at a concentration of 0.1% for adults and 0.03% for children. Pimecrolimus, another calcineurine inhibitor with similar efficacy and tolerability, is not marketed in France. These products inhibit cytokine production by antigen-stimulated T lymphocytes. Their clinical efficacy has been demonstrated in many studies in the United States

Advances in topical and systemic antifungals.

Topical antifungal agents are generally used for the treatment of superficial fungal infections unless the infection is widespread, involves an extensive area, or is resistant to initial therapy. Systemic antifungals are often reserved for the treatment of onychomycosis, tinea capitis, superficial and systemic candidiasis, and prophylaxis and treatment of invasive fungal infections. With the development of resistant fungi strains and the increased incidence of life-threatening invasive fungal infections in immunocompromised patients, some previously effective traditional antifungal agents are

Is tinea unguium still widely incurable? A review three decades after the introduction of griseofulvin.

BACKGROUND: Tinea unguium, especially of the feet, for ages has been looked on as an incurable disease. Today, therapeutic outcome is still controversial. To evaluate the present state of antifungal therapy of dermatophytoses of the nails, a survey of the pertinent literature has been performed. OBSERVATIONS: The introduction of griseofulvin three decades ago was first considered a major breakthrough. Today, however, conventional griseofulvin treatment regimens must be called disappointing in terms of clinical and microbiological cure to be achieved. Cure rates of 40% to 100% have been report

Treatment of cats with ocular disease attributable to herpesvirus infection: 17 cases (1983-1993).

Medical records of 17 cats with ocular disease attributable to herpesvirus injection were reviewed. Herpesvirus infection was confirmed by a positive result on an immunofluorescent antibody test or by detection of dendritic corneal ulcers. Cats were 3 months to 23 years old (mean, 4.8 years). Sex or breed predilections were not evident. Vaccination history was available for 13 cats, 9 of which had been adequately vaccinated against feline viral rhinotracheitis, calici, and panleukopenia viruses. Six cats had a history of respiratory tract disease. Twelve cats were tested for FeLV, and 3 had po

Cutaneous Metastasis of Triple Negative Breast Cancer: A Case Report and Review of Literature.

Cutaneous metastases (CMs) from breast cancer are rare but represent a significant manifestation of advanced disease, occurring in a small group of patients. They often present metachronously and may resemble dermatologic conditions, complicating diagnosis. We report a 95-year-old woman with a history of ductal triple negative breast cancer (TNBC), initially treated with mastectomy and radiotherapy. Eight years post-mastectomy, she developed an erythematous and indurated skin lesion in the left axilla and one in the rachis. Histopathology confirmed metastatic carcinoma of the breast and CT sca

Update on Treating Painful Diabetic Peripheral Neuropathy: A Review of Current US Guidelines with a Focus on the Most Recently Approved Management Options.

Painful diabetic peripheral neuropathy (DPN) is a highly prevalent and disabling complication of diabetes that is often misdiagnosed and undertreated. The management of painful DPN involves treating its underlying cause via lifestyle modifications and intensive glucose control, targeting its pathogenesis, and providing symptomatic pain relief, thereby improving patient function and health-related quality of life. Four pharmacologic options are currently approved by the US Food and Drug Administration (FDA) to treat painful DPN. These include three oral medications (duloxetine, pregabalin, and
