Using SNP microarray to distinguish mosaicism from chimerism in a phenotypically normal male newborn with prenatal 46,XX/46,XY karyotype.

BACKGROUND: The coexistence of 46,XX/46,XY cells in cultured amniotic fluid sample is not uncommon. The vast majority of these cases are the … etus of a 37-year-old nulliparous woman underwent amniocentesis at 19 weeks of gestation because of advanced maternal age. Chromosome analysis of the first and second samples revealed a mixture of 46,XX and 46,XY cell lines. Prenatal ultrasound examination showed normal male genitalia. After birth, peripheral blood samples were analyzed by karyotyping, fluorescence in …

Association Between Maternal Serum 25-Hydroxyvitamin D Concentrations and Gestational Diabetes Mellitus in Women Undergoing Diagnostic 100-g Oral Glucose Tolerance Testing.

Background: Gestational diabetes mellitus (GDM) is a common metabolic complication of pregnancy. Vitamin D has been implicated in glucose me … s (14.08 ± 9.88 vs. 19.06 ± 10.30 ng/mL, p = 0.003). In multivariable logistic regression analysis, advanced maternal age (OR 1.404, 95% CI 1.217-1.620; p < 0.001) and lower serum vitamin D concentrations (OR 0.933, 95% CI 0.880-0.989; p = 0.020) were independently associated with GDM. Conclusion: Lower maternal serum 25(OH)D concentrations were independently associate …

Acute appendicitis during pregnancy: a retrospective case series of seven cases.

Acute appendicitis is the most frequent non-obstetrical surgical emergency during pregnancy. Anatomical displacement of the appendix and atypical symptoms complicate diagnosis, often delaying treatment and increasing maternal-fetal risks. A retrospective study of seven pregnant women treated for acute appendicitis at National Maternal and Child Health Center between 01 January 2023 and 31 December 2024. Epidemiological, clinical, diagnostic, therapeutic, and outcome data were analyzed using descriptive and inferential statistics. Mean age was 31 years (SD 7.2; range 21-42). Most cases occurred

Association of risk factors, clinical presentation, and treatment with neonatal outcomes among pre-eclamptic and eclamptic women: a cross-sectional study.

BACKGROUND: Eclampsia and pre-eclampsia cause high feto-maternal mortality in Pakistan. This study aimed to identify potential risk factors … or those who hadn't received magnesium sulfate (20.6% survived vs. 6.5% non-survived). CONCLUSION: Advanced maternal age (≥35), overweight, elevated AST, ALT, and LDH levels, consanguinity, and grand multiparity were associated with higher perinatal mortality. Women with these predictive factors should be monitored for the development of pre-eclampsia or eclampsia.

Retinal Vascular Imaging Application in Women's Reproductive Health: Clinical Implications and Future Directions.

Over the past two decades, population-based studies employing semiautomatic computer-assisted programs have uncovered associations between retinal microvascular features and various systemic conditions. As the recognition of retinal imaging in cardiometabolic health grows, there is increasing evidence supporting its application in women's health, particularly during the reproductive age. This review aims to summarize the indications of retinal imaging in women's health and intergenerational health, where suboptimal retinal imaging has been found to mirror pathological systemic changes, such as

Maternal microchimerism in muscle biopsies from children with juvenile dermatomyositis.

OBJECTIVE: Recent advances in molecular techniques have revealed that there is bi-directional transfer of cells between mother and child during pregnancy, and the presence of a mother's cells in her child has been termed maternal microchimerism (MMc). There is the potential for maternal cells to provoke inappropriate immune responses in the child, which could be a factor in autoimmunity including JDM. The aim of this study was to determine whether maternal (female) cells could be detected in frozen muscle sections from seven males (age range 3-13 years) with JDM participating in the Juvenile D

Maternal deaths caused by eclampsia in Brazil: a descriptive study from 2000 to 2021.

OBJECTIVE: Eclampsia is a hypertensive disorder that occurs during pregnancy and can lead to death. The literature has gaps by not providing comprehensive data on the epidemiology of the disease, restricting analysis to limited temporal intervals and geographical locations. This study aims to characterize the epidemiological profile of women who died from eclampsia in Brazil from 2000 to 2021. METHODS: The maternal mortality data were obtained from the Sistema de Informações sobre Mortalidade, with the following variables of interest selected: "Federative Unit," "Year," "Age Range," "Race/Col

Can we explain why Brazilian babies are becoming lighter?

BACKGROUND: We tried to explain why a marked decrease in birthweight of 122 g occurred over a 15-year period in Ribeirão Preto, Brazil. METHODS: Factors reflecting biological, social, and health care characteristics (infant gender, parity, maternal age, marital status, type of hospital, maternal smoking, preterm birth, small for gestational age [SGA], and prenatal care) were assessed on 6711 newborns in 1978/1979 and 2838 in 1994 using multiple linear regressions. RESULTS: The birthweight distribution shifted to the left and the residual distribution of small preterm babies increased from 1.

Fetal macrosomia and its associated factors among pregnant women delivered at national referral hospital in Uganda, a case-control study.

BACKGROUND: The delivery of macrosomic newborns (newborns>4000gm) is increasing steadily worldwide and yet it is associated with many complications. The risk factors for fetal macrosomia include weight at first antenatal visit, previous delivery of a large newborn, newly diagnosed diabetes in pregnancy, increasing number of deliveries, a male fetus, and many others. There is paucity of data on fetal macrosomia in Uganda and the objective of this study therefore was to determine the risk factors for fetal macrosomia among women who delivered at a National Referral Hospital in Kampala, Uganda..

Favorable outcome in a neonate with methamphetamine, tobacco, and alcohol exposure with Hypoxic Ischemic Encephalopathy managed using therapeutic hypothermia.

This report details the management of a neonate with moderate Hypoxic Ischemic Encephalopathy (HIE) and prenatal polysubstance exposure. Born at 36 weeks gestation following maternal methamphetamine, tobacco, and alcohol use, the infant suffered a placental abruption. Therapeutic cooling was initiated at three hours of life for 72 h. While urine toxicology confirmed methamphetamine exposure, Neonatal Abstinence Syndrome (NAS) scores remained below the pharmacological threshold. Despite limitations in advanced neurophysiological monitoring common in resource-limited settings, the infant's recov

Gestational trophoblastic disease.

GTD occurs in fewer than 1 in 1200 pregnancies in the United States, but it is much more common in Asia and Latin America, where its incidence may be as high as 1 in 200 pregnancies. Risk factors for GTD include advanced or young maternal age, low socioeconomic status, and prior hydatidiform mole. Early diagnosis and prompt treatment are key to a favorable outcome, and thus recognition of the signs and symptoms of the disease is important for all physicians. Because these diseases have low incidences and occur after reproductive events, screening for them in the general population is not worth

Unusual segregation for 11q;22q parental translocation in a triplet pregnancy: prenatal diagnosis in chorionic villi and amniotic fluid.

The prenatal diagnosis of an 11q;22q translocation in a triplet pregnancy detected at the time of chorionic villus sampling (CVS) because of advanced maternal age is reported. Karyotypes obtained from two apparently different CV samples showed the balanced form of translocation, while the one obtained from a third empty sac showed the unbalanced form: 46,XX,-22,+der(22)t(11;22). Second-trimester amniocentesis confirmed the balanced translocation in one of the two viable fetuses and a normal karyotype in the other. The detected karyotypes derived from two different types of meiotic segregation,…

Emergent cerclage.

Emergent cerclage may be defined as that performed in the setting of advanced cervical dilation with bulging or "hourglass" membranes. During a five year period, 15 patients with bulging or hourglass membranes and marked cervical dilation in the second trimester were treated with emergent cerclage. Included were two triplet gestations. Complications were limited to intraoperative rupture of fetal membranes in two patients and chorioamnionitis either in the early postoperative period (two) or later in pregnancy (three). Pregnancy was prolonged for a sufficient time to deliver viable fetuses in

Tackling global inequalities in maternal hypertensive disorders: trends and the impact of public health emergencies, 1990-2021.

BACKGROUND AND AIMS: Despite progress made under the United Nations Millennium Development Goals (MDGs) and the Sustainable Development Goals (SDGs), inequalities in global health persist, particularly in the maternal health. Public health emergencies also affect health equity. This research examined long-term disease burden trends (1990-2021) of maternal hypertensive disorders (MHD), with a focus on the influence of age and socio-demographic index (SDI) differences, as well as short-term disruptions during the COVID-19 pandemic, to inform more equitable maternal health policies. METHODS: Usi

Placental insufficiency among high-risk pregnancies with a normal umbilical artery resistance index after 32weeks.

OBJECTIVE: To determine the incidence of abnormal multi-vessel Doppler values among advanced pregnancies at risk of suboptimal placentation but with a normal umbilical artery resistance index (RI), and to assess whether clinical and ultrasonography findings can identify them. METHODS: In a prospective cross-sectional study at Tygerberg Hospital, South Africa, women with high-risk pregnancies but normal umbilical artery RI after 32weeks underwent ultrasonography (fetal biometry, liquor, and placenta maturation) and Doppler assessment (uterine, umbilical, and middle cerebral arteries) between F

Distinct maternal and fetal pregnancy outcomes in women with sickle cell disease can be predicted using routine clinical and laboratory data.

We aimed to identify risk factors for adverse outcomes in pregnancies of women with sickle cell disease (SCD) and develop risk prediction models. Models were derived from a retrospective cohort of pregnant women with SCD and constructed using generalised estimating equation logistic regression, with clustering by woman. Maternal event(s) consisted of acute anaemia; cardiac, pulmonary, hepatobiliary, musculoskeletal, skin, splenic, neurological or renal complications, multi-organ failure, venous thromboembolism, admission-requiring vaso-occlusive events (VOE), red cell transfusion, mortality or
