Serum Albumin as a Signal for Near-end-of-life Chemotherapy and Care in Advanced or Recurrent Gastric Cancer.

BACKGROUND/AIM: Chemotherapy administered near death is a recognized quality indicator of end-of-life cancer care intensity. However, simple clinical signals to prompt treatment reassessment and coordinate planned end-of-life care remain unclear in advanced gastric cancer. This study evaluated the interval from last chemotherapy to death and explored factors associated with near-end-of-life chemotherapy and failure to transition to a planned end-of-life care setting. PATIENTS AND METHODS: This single-center retrospective study included 53 patients with advanced or recurrent gastric cancer who

[Medial venous catheter or midline (MVC)].

Current clinical practice is characterised for importance of the patient's quality of life and the need to reduce the costs of their treatment. We search intravenous therapy alternatives that meet the needs of the patient, reducing the complications associated with the use of venous catheters. Scientific evidence shows that there are midline venous catheters that offer patients and professionals the possibility of extending the duration of infusion therapy, using more venous compatibility materials, and with less risk of infection. The Midlines are becoming in a safe an efficient device for in

Quality Indicators of Primary Care Provider Engagement in Nursing Home Care.

The initiative described here aims to identify quality indicators (QIs) germane to the international practice of primary care providers (PCP) in post-acute and long-term care in order to demonstrate the added value of medical providers in nursing homes (NHs). A 7-member international team identified and adapted existing QIs to the AMDA competencies for medical providers. QI sources included the ACOVE 3 Quality Indicators (2007), NH Quality Indicators (2004), NH Residential Care Quality Indicators (2002), and AGS Choosing Wisely (2014). We recruited a technical expert panel (TEP) consisting of

A Meta-Analysis of the Efficacy of Ultrasound-Guided Nasojejunal Tube Placement.

BACKGROUND: Critically ill patients are prone to enteral nutrition interruptions during nutritional support, which are primarily due to gastroparesis and subsequent delayed gastric emptying and may hinder clinical recovery. Robust evidence supports the adoption of post-pyloric feeding in this population, with nasojejunal tube placement established as the gold-standard modality for achieving reliable transpyloric access while reducing aspiration risks. AIM: To evaluate the efficacy of ultrasound-guided nasojejunal tube placement in critically ill patients through a systematic review and meta-a

[Palliative care in the light of legal and regulatory requirements in Germany].

In Germany, palliative care has developed rapidly since the establishment of the first palliative care unit in 1983. More improvements in patient-centered care are only possible if legal requirements as well as education of physicians, nurses, and other professionals involved in palliative care are adapted to current needs. This paper provides an overview of legal and regulatory requirements including a critical appreciation of their influence on palliative care in Germany. Only recently has medical education in palliative medicine been implemented as an integral part of medical studies at Ger

Why families deny consent to organ donation.

Advances in medical practice and technology and the success of organ transplantation over the past 2 decades have resulted in an increased demand for organ donors. However, the health care community and organ procurement organisations (OPO) are faced with a worldwide shortage of donor organs. The non-consent of families is the most common reason that organs of medically suitable potential donors are not recovered. A review of published research post 1990 was conducted to primarily determine the major factors that influence a family's decision to deny consent to donation. Other objectives inclu
