Environmental factors in helicopter operations.

The environmental problems affecting aircrew are partly those which all soldiers face, such as noise, heat and cold, and partly peculiar to the medium and the vehicle in which aircrew train and fight, such as disorientation and decompression. The cockpit environment of the modern helicopter is luxurious in comparison with many of its predecessors, yet most of the adverse effects of flight on the man still pertain. The result can, predictably, be acute and disastrous, resulting in an accident produced by severe disorientation, or chronic, producing insidious fatigue and performance decrement, w

Thoracic and abdominal injuries in skiers: the role of air evacuation.

The increasing popularity of skiing as a recreational sport has resulted in a greater number of major thoracic and abdominal injuries. These injuries, unlike the more common orthopedic injuries, are often life threatening. This 8-year study reviews 44 thoracic and abdominal ski injuries managed at an urban trauma center since the inception of a helicopter air evacuation program with in-flight resuscitation capabilities. Twenty-five per cent of the injured had signs of hemorrhagic shock, and nearly 60% required a major operation. The injuries were caused by high-speed collisions with stationary

A wing-flapping robot with a bio-inspired folding mechanism derived from the beetle's hind wing.

When the beetle lands on the target, the hind wings fold regularly to form smaller wing packages and are hidden on the ventral side of the elytra due to the interaction between the elytra and abdomen. Its complex folding pattern is attributed to the flexibility of the hind wings, the super-elasticity of the folding joints, and the special geometric morphology of the veins. The corrugation and folding pattern of the hind wings can provide new insights for the design of folding anti-collision mechanisms and the improvement of aerodynamic performance of ornithopter. This paper first proposes a be

Idiopathic Syringomyelia in a Military Helicopter Pilot.

BACKGROUND: A syrinx is a fluid-filled cavity within the spinal cord. They can lead to a variety of symptoms, including limb weakness and back pain. Incidental finding of syringomyelia provides a challenge for clinicians due to the wide variety of possible symptoms. In military aviation, neurological findings in pilots can result in extensive investigation that can lead to potentially invasive management. Conversely, the potential for chronic progression of a spinal syrinx and subsequent neurological deterioration makes early identification critical. Ultimately, the discovery of a lesion may h

Use of the nine-step inflation/deflation test and resting middle-ear pressure range as predictors of middle-ear barotrauma in aircrew members.

OBJECTIVE: To explore the role of the nine-step inflation/deflation tympanometric test and resting middle-ear pressure range as predictors of barotrauma in aircrew members. METHODS: A prospective, non-randomised study was conducted on 100 aircrew members. Resting middle-ear pressure was measured and the nine-step inflation/deflation test performed on all subjects before flights. Subjects were allocated to two groups according to resting middle-ear pressure range (group A, within the range of +26 to +100 and -26 to -100 mmH2O; group B, -25 to +25 mmH2O). All aircrew members were assessed after

Virtual reality exposure and imaginal exposure in the treatment of fear of flying: a pilot study.

Fear of flying (FF) is an impairing psychological disorder that is extremely common in developed countries. The most effective treatment for this particular type of phobia is exposure therapy. However, there are few studies comparing imaginal exposure (IE) and virtual reality (VR) exposure for the treatment of FF. The present study compared the effectiveness of these two approaches using two manualized interventions based on the exposure technique. Patients with FF (N = 15) were randomly assigned to either VR (n = 7) or IE therapy (n = 8), consisting of a total of eight sessions: two assessmen

Activation of coagulation system during air travel: a crossover study.

BACKGROUND: There is an increased risk of venous thrombosis after air travel, but the underlying mechanism is unclear. Our aim was to ascertain whether flying leads to a hypercoagulable state. METHODS: We did a crossover study in 71 healthy volunteers (15 men, 56 women), in whom we measured markers of activation of coagulation and fibrinolysis before, during, and after an 8-h flight. The same individuals participated in two control exposure situations (8-h movie marathon and daily life) to separate the effect of air travel on the coagulation system from those of immobilisation and circadian r

Aircrew ejection experience: questionnaire responses from 20 survivors.

BACKGROUND: Published studies on ejection have focused predominantly on the injuries sustained by aircrew and discussed their preventive measures from an aeromedical perspective. However, studies have not discussed aircrew experiences related to ejection or how they would like to advise other aircrew to successfully handle ejection as an event. Such information can assist in designing realistic indoctrination and training programs. This study was conducted to fill gaps in our understanding of aircrew perspectives of successful ejections. METHODS: Aircrew reporting to the Institute of Aerospac

Noise induced nocturnal cortisol secretion and tolerable overhead flights.

Mainly dependent on level and dynamic increase sound produces over-shooting excitations which activate subcortical processing centers (e.g. the amygdala, functioning as fear conditioning center) besides cortical areas (e. g. arousing annoyance, awakenings) as well. In addition there exist very close central nervous connections between subcortical parts of the auditory system (e.g. amygdala) showing typical plasticity effects (sensitization) and the hypothalmic-pituitary-adrenal (HPA)-axis. Using that causal chain noise induce cortisol excretion even below the awakening threshold. Thus repeated

MR imaging appearance of frontal sinus barotrauma.

We present the case of a flight passenger who experienced acute and severe headache during landing. MR imaging was performed because the patient had a history of vascular malformation and revealed an incidental venous angioma. A mass lesion in the frontal sinus, consistent with submucosal hematoma secondary to barotrauma, was thought to be the cause of the headache. To our knowledge, this is the first case of sinus barotrauma described in the radiologic literature and the first to describe the associated MR imaging findings.

Virtual reality exposure therapy for the treatment of fear of flying: a controlled investigation.

Forty-five participants who refused to fly during a screening test and who also met Diagnostic and Statistical Manual of Mental Disorders criteria for specific phobia, agoraphobia, or panic disorder with agoraphobia were randomly assigned to 5 sessions of either virtual reality exposure (VRE) or attention-placebo group treatment (GT). At posttreatment, 65% of VRE participants and 57% of GT participants flew during a test flight. Both groups showed significant improvement following treatment on standardized self-report measures of flight anxiety, with a better outcome for the VRE group on 4 of

Risk factors for injury among construction workers at Denver International Airport.

BACKGROUND: The Denver International Airport construction project provided a rare opportunity to identify risk factors for injury on a large construction project for which 769 contractors were hired to complete 2,843 construction contracts. Workers' compensation claims and payroll data for individual contracts were recorded in an administrative database developed by the project's Owner-Controlled Insurance Program. METHODS: From claims andy payroll data linked with employee demographic information, we calculated injury rates per 200,000 person-hours by contract and over contract characteristi

The use of neuromuscular blocking agents by air medical services.

Neuromuscular blocking agents (NMBs) are frequently used to facilitate intubations in the hospital. The 1987 membership of the Association of Air Medical Services (AAMS) was surveyed to determine the frequency of NMB use by flight programs both before and after definitive airway control. Out of 141 programs, 101 returned completed survey forms. Of those responding, 39 of 101 used NMBs before intubation, and 67 programs used NMBs after intubation. The use of NMBs in the base hospital by emergency physicians was a significant predictor of the use by the air medical service. No other factors stud

High altitude haemofiltration.

Evacuating people in acute renal failure by air is difficult because the hazards of fluid overload and anaemia are potentiated by altitude. In two such patients continuous arteriovenous haemofiltration was used to control their fluid problems during aeromedical evacuation. In the first case, a patient with renal failure and blast lung, haemofiltration was performed at 500 ml/h over a four hour journey; in the second, a woman with severe pre-eclamptic toxaemia who developed acute renal failure after caesarean section, haemofiltration was performed at 200 ml/h over a 14 hour flight. Both patient
