Haemophilus influenzae type b disease and vaccination in Latin America and the Caribbean.

OBJECTIVE: Conjugate vaccines are highly effective in preventing Haemophilus influenzae type b (Hib) diseases. Data on the epidemiology of Hib disease in Latin America and the Caribbean are not easily accessible, are incomplete and are deserving of critical analysis. METHODS: Relevant information in Spanish, English and Portuguese from over 20 regions in 16 countries was reviewed, with special attention to all Hib diseases and to children < 5 years because of their proneness to disease. Total number of cases and deaths were estimated from the incidence rates obtained from 10 studies. RESULTS

André Barbeau and the oculopharyngeal muscular dystrophy in French Canada and North America.

André Barbeau (1931-1986) is best known world-wide in the neurologic community for his contributions to the study of Parkinson's disease, Huntington's chorea and Friedreich's ataxia. But in Québec, Canada, his name is associated with oculopharyngeal muscular dystrophy (OPMD), often called here 'maladie de Barbeau', on which he conducted a series of genealogic, genetic and clinical studies early in his career, most intensively from 1964 to 1966. He then demonstrated that most of the reported cases in North America could be traced back to French-Canadian ancestors. Furthermore, he identified thi

Risk of vaccine-associated paralytic poliomyelitis in Latin America, 1989-91.

A major factor influencing the success of poliomyelitis eradication in the Americas was the reliance on mass immunization campaigns with oral poliovirus vaccine (OPV). As global poliomyelitis eradication activities accelerate and campaign vaccine delivery strategies are applied elsewhere, it is critical to determine whether the risk of vaccine-associated paralytic poliomyelitis (VAPP) is altered when routine delivery strategies are supplemented with mass immunization campaigns. We analysed all 6043 cases of acute flaccid paralysis (AFP) reported in Latin America over the period 1989-91 in orde…

Primary infertility: characteristics of women in North America according to pathological findings.

STUDY OBJECTIVE: To determine, in women with primary infertility, whether specific characteristics or behavioural factors are associated with the various pathological conditions identified as contributing to the infertility. DESIGN: Case-control study. SETTING: Seven institutions in the USA or Canada. PARTICIPANTS: Study subjects were 1750 women who presented with primary infertility, among whom the main pathological cause of infertility was male factor (417), tubal obstruction (231), endometriosis (194), luteal phase defects (153), other ovulatory problems (193), cervical abnormalities (92

An education model for otitis media care field-tested in Latin America.

The World Health Organization has designated the teaching of otitis media management skills a "priority" status. Effective treatment of ear disease requires that the physician be both informationally educated as well as physically trained to use otoscopy. Little is known about how well this education can be provided in a short time and in a foreign country. To more objectively assess teaching effect, results of an education session for rural Mexican pediatric primary-care providers who were given an intensive otitis media lecture and otoscopy skills workshop in 1990 were evaluated. To test imm

Risk of health care professionals acquiring HIV infection in Latin America.

This study describes the professional risk of acquiring HIV infection while caring for AIDS patients at a teaching hospital in Brazil. Since 1985 we have tested health care professionals (HCP) for HIV-1 antibody after accidents with blood and body fluids from AIDS patients. The blood samples were tested twice using an ELISA FDA approved test and, if positive, we performed Western blot. Two hundred and forty seven health care professionals reported 338 accidents (50% were percutaneous and 22% were mucous membrane exposures to blood). A further 404 HCPs reported no occupational exposure but want

[Strategies to reduce morbidity and mortality caused by acute diarrhea in Latin America].

Following the World Health Organization guidelines, the Latin American Diarrheal Disease Control Programs have directed its efforts towards the promotion of Oral Hydration Therapy (OHT) and appropriate dietary management during the diarrheal episode and convalescent period, aimed at diminishing the mortality secondary to diarrhea. In developing countries, OHT is preventing, annually, one million of childhood deaths due to dehydration. Yet, only one fourth of the total population of children suffering diarrhea are being treated with this therapy. Among the strategies to decrease diarrhea morbid

Drug-taking among Nigerian students at universities in the United States of America.

A sample survey of Nigerian students conducted by means of a self-administered drug use questionnaire at three universities in the United States of America showed that at some time in their lives 91.2 per cent of the respondents had used alcoholic beverages, 62.3 per cent tobacco, 30.6 per cent cannabis, 6.3 per cent inhalants, 19.7 per cent amphetamines or amphetamine-type substances, 4.6 per cent cocaine, 2.1 per cent hallucinogens, 23.8 per cent tranquillizers, 4.2 per cent sedatives, 4.2 per cent opium, 1.3 per cent heroin and 20.5 per cent other opiates. Polyps in a low-risk colonic cancer population in Colombia, South America.

Colonic polyps were systematically searched for in 508 specimens collected from unselected autopsies in individuals more than 10 years of age from Medellín, Colombia, a low-incidence geographic region for colorectal cancer. Adenomatous polyps have a low prevalence rate in both sexes, comparable to that found in Cali, Colombia and other populations with low risk for cancer. They were found more frequently in the right side of the colon and are rare in the sigmoid colon. They begin to appear in the third decade of life in both sexes. Prevalence rates for single and multiple polyps increase with

[Chagas' disease and its significance for Central and South America (author's transl)].

After the successful struggle against malaria, smallpox and tuberculosis, Chagas' disease must be considered the commonest and most serious scourge of the American continent. Experts assess the number of persons suffering from Chagas' disease at about 7 millions with about 35 millions at risk. The acute infection with Trypanosoma cruzi in the blood stream is found chiefly in children in the first few years of life but rarely in adults. Mortality in the acute phase is about 10%. The disease tends to run a chronic course. After an asymptomatic phase (usually 10--12 years), chronic Chagas' myocar

Leptospirosis as an animal and public health problem in Latin America and the Caribbean area.

As a major livestock disease, leptospirosis cuts significantly into the world's supply of animal protein through its consequences of abortion and calf mortality, retarded growth, and reduced milk production. Evidently, such losses have severe economic implications as well. The disease's impact has been somewhat diminished in the developed countries thanks to mechanization of rural activities, improved personal and environmental hygiene, and broad application of preventive measures, including the administration of bacterins, in domestic animals. Still, even in these circumstances it remains a s

Epidemic Venezuelan equine encephalitis in North America in 1971: vector studies.

A major epidemic of Venezuelan equine encephalitis occurred in south Texas in the summer of 1971. More than 1500 equines died of VEE in Texas, and 110 human cases with no deaths were reported. Vector studies in south Texas and northern Tamaulipas revealed that the overall mosquito infection rates during the peak of the epidemic were about 1:100, one of the highest rates observed for a major epidemic. Mosquito infection rates of this magnitude could easily explain the intensity of VEE outbreaks in both equines and man. A total of 943 VEE virus isolations were made from mosquitoes. Eight of the
