Norepinephrine versus colloid for preventing spinal anesthesia-induced hypotension and its impact on intracranial pressure in preeclamptic parturients: a randomized double-blind controlled trial.

OBJECTIVE: This study aims to evaluate the effects of two strategies for preventing hypotension after spinal anesthesia (prophylactic fluid loading and norepinephrine infusion) on intracranial pressure (ICP) in preeclamptic women by measuring the optic nerve sheath diameter (ONSD) using ultrasound. METHODS: In this prospective, randomized, controlled trial, 60 healthy parturients and 60 preeclamptic parturients undergoing cesarean delivery under spinal anesthesia were enrolled. Normal parturients were randomly assigned to the infusion group (NA group, n = 30) and NE (norepinephrine) group (NB

Low-dose epidural anesthesia for autonomic dysreflexia prevention in individuals with high-risk cervical spinal cord injury undergoing transurethral procedures: A retrospective case series.

CONTEXT: Autonomic dysreflexia (AD) is a life-threatening condition frequently triggered during transurethral procedures in individuals with cervical spinal cord injury (SCI). General anesthesia (GA) does not consistently prevent AD in this population, and optimal anesthetic management remains uncertain. FINDINGS: We report five male individuals with C4-C6 SCI, American Spinal Injury Association Impairment Scale (AIS) grade A/B with documented AD during daily catheterization who underwent transurethral procedures under low-dose epidural anesthesia (EA). Lumbar EA was performed with ropivacain

Effect of Prophylactic Phenylephrine Infusion Versus Interventional Ephedrine Boluses on Umbilical Blood pH in Cesarean Deliveries Under Spinal Anesthesia: A Retrospective Case-Control Study.

Background/Objectives: Hypotension is a common complication of spinal anesthesia for cesarean section. Although phenylephrine has replaced ephedrine as the first-line vasopressor, comparative data on neonatal outcomes remain important in clinical decision-making. The objective of this study was to compare the effects of prophylactic phenylephrine infusion versus interventional ephedrine boluses on umbilical artery pH and maternal hemodynamic stability in women undergoing cesarean section under spinal anesthesia. Methods: In this retrospective case-control study we analyzed perioperative and ne

Effect of Music Therapy on Anxiety, Stress and Sedative Requirements in Patients Undergoing Lower Limb Orthopedic Surgery Under Spinal Anesthesia: A Randomized Controlled Study.

Background Music therapy is a safe, non-pharmacological way to help reduce anxiety, especially before surgery. It helps to calm the nervous system, promotes relaxation, and offers comfort by blocking outside noise and distracting from pain. This can be helpful in managing preoperative anxiety and symptoms like hypertension and tachycardia. While the benefits of music therapy for stress and sedation are well known, its specific effects on patients receiving spinal anesthesia haven't been studied yet. Understanding this could improve care for these patients. Overall, music therapy could be a val

[Effect of epidural pressure gradient on buprenorphine flux through the dural hole after combined spinal-epidural anesthesia--comparison between bolus injection and continuous infusion].

Combined spinal-epidural anesthesia is a useful technique. However, there has been no attempt to investigate the risk of epidural opioid, especially buprenorphine, flux through the dural hole. The purpose of this study was to compare the effect of epidural buprenorphine administered across the dura into subarachnoid space, between two different methods of administration; bolus injection (Group I) and continuous infusion (Group II). Sixty patients for transvaginal hysterectomy were divided into two groups. Group I received buprenorphine 0.1-0.2 mg with 0.25% bupivacaine, and Group II 0.4 mg wit

Empirical pharmacodynamic model of phenylephrine and intrathecal bupivacaine for mean arterial pressure prediction in obstetric patients presenting for elective cesarean delivery under spinal anesthesia.

Cesarean delivery under spinal anesthesia may be complicated by hypotension in up to 80% of the patients. The response to standard-of-care prophylactic phenylephrine infusion varies, and there is little guidance on achieving optimal blood pressure control. In this work, we developed a data-driven pharmacodynamic relationship between intravenous phenylephrine, intrathecal bupivacaine, and maternal mean arterial pressure (MAP) in patients presenting for cesarean delivery. In this single-center cohort study, secondary use data were available for normotensive patients presenting for cesarean deliv

Breaking myths and building awareness: spinal anesthesia for cesarean section in Palestine-what women know and what holds them backs- a cross- sectional study.

BACKGROUND: Cesarean section (CS) is one of the most frequently performed surgical procedures worldwide. Spinal anesthesia (SA) has become the preferred anesthesia technique for CS due to its advantages over general anesthesia, including better maternal hemodynamic stability and improved neonatal outcomes. However, maternal knowledge and attitudes toward SA significantly influence decision making and perioperative experiences. This study aimed to assess the knowledge, awareness, and attitudes of Palestinian women toward SA during CS. METHODS: A cross-sectional study was conducted among Palest

Comparison of low-dose morphine intrathecal analgesia and sufentanil PCIA in elderly patients with hip fracture undergoing single spinal anesthesia - a randomized clinical trial.

BACKGROUND: The complications of postoperative pain, such as hypertension, hypermetabolism, irritability, and postoperative cognitive dysfunction, significantly affect the postoperative rehabilitation of elderly patients. Intrathecal morphine prolongs analgesia after surgery, but has been implicated in nausea and vomiting, pruritus, postoperative respiratory depression, or apneic episodes. The present study explored the effect and safety of low-dose morphine used adjunctively with bupivacaine during single spinal anesthesia or sufentanil patient-controlled intravenous analgesia (PCIA) in elder
