Patients with locally advanced pancreatic cancer who undergo distal pancreatectomy with resection of the celiac axis (CA) are at risk for postoperative hepatic or gastric ischemia if collateral blood flow from the superior mesenteric artery (SMA) via the gastroduodenal artery is insufficient. This study presents a technique for preoperative angiographic evaluation of these collateral vessels by using an AMPLATZER Vascular Plug to temporarily occlude the CA or common hepatic artery while simultaneously performing digital subtraction angiography of the SMA. If collateral vessels are deemed suffi

We describe a case of type-2 bronchial artery endoleak after endovascular aortic repair of Kommerell diverticulum (KD) involving right-sided aortic arch and aberrant left subclavian artery (LSA). A 68-year-old woman underwent an endovascular repair of KD with an aberrant LSA in our hospital. Follow-up computed tomography (CT) at 6 months after the procedure showed an endoleak. Digital subtraction angiography revealed a type-2 endoleak from a bronchial artery, but no type-1 or type-2 endoleak from the aberrant left subclavian artery. We performed coil embolization of the KD and the left subclav

Spontaneous subarachnoid hemorrhage (SAH) is usually caused by a ruptured cerebral aneurysm. Despite the use of initial four-vessel cerebral digital subtraction angiography (DSA), 15 % of all cases remain idiopathic. According to the initial computed tomographic scan, the spontaneous SAH can be divided into a perimesencephalic group associated with a benign nature and a nonperimesencephalic group with a similar clinical course as aneurysmal SAH. We present a case of a 49-year-old man with a de novo aneurysm formation of the anterior communicating artery with SAH 7 years after initial cryptogen

Cerebral arteriovenous malformations (AVMs) are abnormal connections between arteries and veins leading to arteriovenous shunting with nidus formation. This study reviewed the clinical outcomes of surgical treatment for AVMs of Spetzler-Martin grades III to V in our institute. In addition, we summarized the technical aspects of surgical treatment for cerebral AVMs. Our development of the surgical modality for high-grade AVMs included intraoperative digital subtraction cerebral angiography, non-stick bipolar forceps, magnetic resonance tractography, and indocyanine green videoangiography. Tentorial dural arteriovenous fistulas (tDAVF) clinically present usually with subarachnoid and/or intraparenchymal hemorrhage. Reported rates range from 58% to 92% and neurological deficits occur in 79% to 92% of patients. This is due to venous congestion resulting from retrograde leptomeningeal venous drainage, which rarely, can be clinically silent. A 69-year-old woman presented with vertigo, double vision and gait instability. Cerebral digital subtraction angiography revealed a tDAVF with retrograde cerebellar venous drainage directed through the vein of Galen into the straight sinus. MRI

Aortocaval fistula (ACF) is a rare complication of abdominal aortic aneurysm (AAA), and its preoperative diagnosis is often difficult. A 71-year-old woman was admitted to our hospital due to unilateral leg edema. Abdominal computed tomography (CT) showed an abdominal aortic aneurysm (AAA), a common iliac aortic aneurysm (CIAA) and ACF was suspected. Digital subtraction angiography (DSA) was performed, enabling us to identify the region of ACF with AAA preoperatively. ACF is associated with high mortality because it is difficult to control venous bleeding from ACF. Detailed preoperative diagnos

Computed tomography (CT) is the initial radiological investigation of patients with an acute neurological event. A 64-year-old woman presenting with generalised weakness and headache for two days was diagnosed on CT to have subarachnoid haemorrhage. Digital subtraction angiography confirmed the cause to be a ruptured posterior communicating artery aneurysm. The patient was treated by neuroradiological intervention using occlusive coils. The CT features of subarachnoid haemorrhage are discussed. Accurate CT interpretation is essential to direct appropriate investigations and management in patie

We report a rare case of agenesis of the left internal carotid artery (ICA) and an aortic arch anomaly that presented with ipsilateral congenital Horner's syndrome. Digital subtraction angiography revealed left ICA agenesis shortly after its origin and anastomosis between the left maxillary artery and the supraclinoid segment of the left ICA. Aortography of the aortic arch revealed the right subclavian artery arising as the first branch of a left aortic arch, followed by a bicarotid trunk and a left subclavian artery. High resolution CT of the skull base revealed the absence of the left caroti

Three cases of middle cerebral artery occlusion in children are reported. The origin of M1 was the site of occlusion in two cases, while occlusion 1 cm after the origin of M1 was found in the third. The possible etiological diagnosis was infectious arteritis in the first two cases, cranial trauma in the third. In no case were serious contralateral motor deficits detected in follow-up examinations. Follow-up digital subtraction angiography showed complete regression of arterial lesions in the second case. The authors conclude that middle cerebral artery occlusion may have a benign clinical cour

Iatrogenic fistulae are sporadic but potentially serious complications of catheterization of central veins. The authors present the case-history of a 57-year-old patient with chronic renal failure where after 19 months following temporary insertion of a central venous catheter into the subclavian vein on the right from and infraclavicular approach a continual murmur was recorded in the right subclavicular area, along with increasing manifestations of left-sided cardiac insufficiency. Using digital subtraction angiography, the authors diagnosed an arteriovenous fistula between the internal thor

Sialadenitis ("iodide mumps") and allergic vasculitis are rare sequelae to administration of iodinated contrast media. The condition is characterized by rapid, painless, bilateral enlargement of salivary glands following administration of iodinated contrast media. An 81-year-old female patient with moderate renal failure is described in whom marked sialadenitis, allergic vasculitis with (in part) bloody blisters developed following excretory urography and digital subtraction angiography of the renal arteries. Additional but mild symptoms included fever and conjunctivitis. All lesions subsided

Newer imaging modalities, especially high resolution computed tomographic scanning, have radically altered and expanded the role of radiology in the evaluation and management of lesions affecting the skull base. The location, extent, and tissue characteristics of these lesions can be determined with greater precision than ever before. When necessary, diagnostic and therapeutic angiography can be performed more rapidly and more safely with digital subtraction techniques. In the near future magnetic resonance imaging may prove to be an even more precise imaging tool than computed tomography. Mag

Whereas digital subtraction angiography (DSA) has been extensively used in adults, few reports have dealt with its application in children. We utilized DSA in 52 children, studying various vascular territories. Technically satisfactory studies were obtained in 89%. Motion artifacts is the foremost technical problem. Extremity DSA with arterial injection was highly successful in a number of cases. Thoracic vascular studies were also successful, as were studies of the head and neck, whereas renal and abdominal vessel studies were somewhat less successful. DSA with arterial injection may be expec

Indirect noninvasive neurovascular tests provide information on hemodynamic changes cephalad to the carotid bifurcation, whereas direct tests measure anatomic or physiologic changes at the bifurcation itself. Batteries of tests are often done and should combine both indirect and direct methods. Results with two ultrasonic techniques and digital subtraction intravenous angiography suggest a larger role for these methods. Carotid arteriography is the definitive procedure for evaluating the carotid artery, although it should only be done when carotid endarterectomy is contemplated. The relative r

The successful diagnosis and surgical treatment of primary hyperparathyroidism due to parathyroid adenoma benefits significantly, in our experience, from a process of pre-operative imaging localization of the parathyroid adenoma. This prospective study evaluates a window of 25 consecutive patients who underwent pre-operative imaging localization prior to successful unilateral parathyroidectomy for parathyroid adenoma. All parathyroid adenomas were successfully localized by imaging, and subsequently documented photographically in surgical correlation, and pathologically confirmed. All patients

BACKGROUND/PURPOSES: There are various types of coronary artery fistula (CAF) with complex shapes; therefore, it is important to obtain a correct diagnosis and to understand its relations to the adjacent structures before surgery. This study evaluated echocardiography and 64-multislice computed tomography (64-MSCT) angiography in diagnosing CAF.
METHODS: Sixteen patients with CAF, confirmed by surgical operation or digital subtraction angiography, were examined by echocardiography. Five of them were further examined by 64-MSCT angiography for detailed anatomical information before surgery. Th

PURPOSE: To evaluate if computed tomographic (CT) angiography can replace digital subtraction angiography (DSA) for aneurysm detection and as preoperative work-up in patients with subarachnoid hemorrhage (SAH).
MATERIALS AND METHODS: Prospectively, 100 patients with SAH underwent CT angiography; 80 also underwent DSA. Two observers independently evaluated CT angiographic source images and maximum intensity projection slabs. Neurosurgeons compared CT angiograms and DSA images for presurgical evaluation.
RESULTS: On CT angiograms, the observers detected 73 and 70 of 75 symptomatic aneurysms; 9

Over 50 million people in the United States are hypertensive. Renovascular disease accounts for 3-5% of these hypertensive patients. Because renovascular hypertension is potentially curable, much effort has been devoted to detecting and treating renal artery stenosis. Conventional angiography has been traditionally used to diagnose renal artery stenosis. However, because of its invasiveness and cost, conventional angiography cannot be utilized as a screening test in all patients who may have renal artery stenosis. Several noninvasive studies have been advocated for screening in hypertensive pa

BACKGROUND: Carotid web (CaW)-related contrast stagnation on digital subtraction angiography (DSA) may be a marker supporting the pathophysiological theory of stasis and thrombosis/embolization. We aim to assess the correlation between DSA hemodynamic parameters with CT angiography (CTA) structural measurements and clinical characteristics.
METHODS: Cross-sectional analysis of consecutive patients with CaW who underwent CTA+DSA. DSA-derived hemodynamic parameters were calculated based on a region of interest at the carotid bulb (time-density curves; TDC). The correlation between duration of c
