An update on Escalating Drug Resistance among Uropathogenic Bacteria: Challenges in Treatment and Scope for Maneuver.

Uropathogens are among the most common infectious agents in humans worldwide. The problem is further exacerbated by the escalating emergence and dissemination of antibiotic resistance genes (ARGs) among bacteria causing urinary tract infections (UTIs). The World Health Organization (WHO) has identified antibiotic resistance as the most serious public health threat of this century, demanding global action. The most common bacteria contributing approximately 85% of UTIs include Escherichia coli, Klebsiella pneumoniae, and Enterococcus spp. Reports of intrinsic and/ or acquired resistance mechani

Emerging strategies for biofilm disruption in recurrent urinary tract infections.

Biofilm formation is a key microbial survival mechanism that enables bacterial persistence and recurrence of urinary tract infections (UTIs), especially in patients with catheters or structural abnormalities. These complex microbial communities hinder antibiotic efficacy and facilitate the development of antibiotic resistance. Recent advances in microbiology and materials science have led to the emergence of diverse therapeutic approaches targeting biofilms. This review highlights current and investigational strategies for biofilm disruption, including the inhibition of quorum sensing, enzymat

A versatile colorimetric array sensor based on FeCu-based prussian blue analogue heterostructures for efficient discrimination of tetracycline antibiotics and rapid identification of urinary tract infection pathogens.

Precise discrimination of tetracyclines (TCs) with highly similar chemical structures remains challenging for nanozyme-based sensing platforms. At the same time, rapid and reliable identification of pathogenic bacteria, including urinary tract infection (UTI) pathogens, remains limited, hindering targeted antibiotic use and contributing to antibiotic abuse. In this study, three FeCu-based Prussian blue analogues (PBAs) with adjustable laccase (LAC)-like activity and diverse recognition sites were synthesized and assembled into a three-channel, dual-application colorimetric sensor array for sen

The role of estrogen in prevention of recurrent urinary tract infections in postmenopausal women.

In the United States, postmenopausal women represent the largest group affected by recurrent urinary tract infections (rUTIs). Limited evidence supports long-term use of antibiotics for rUTI prophylaxis in this population. In addition to being expensive, repeated antibiotic use contributes to antimicrobial resistance, posing additional concerns for postmenopausal women who may be at higher risk for serious infection or complications from resistant infections. Topical estrogen, which directly addresses the pathophysiologic changes associated with rUTIs in this population, offers an effective an

Prevention and Management of Urinary Tract Infections Among Persons With a Spinal Cord Injury: Implications for Nurses.

Urinary tract infections (UTIs) are a frequent complication for persons with spinal cord injury (SCI) due to impaired bladder function and catheter use. This article presents a case study to illustrate current SCI UTI management strategies and describe how effective nursing care can impact patient outcomes. The method of bladder management affects UTI risk and bacterial resistance; clean intermittent catheterization lowers infection rates, while prolonged use of indwelling catheters increases multidrug-resistant infections. Antibiogram-guided therapy helps optimize antibiotic selection, improv

Culture-free detection of β-lactamase-Producing bacteria in urinary tract infections using a paper sensor.

Developing simple, inexpensive, fast, sensitive, and specific probes for antibiotic-resistant bacteria is crucial for the management of urinary tract infections (UTIs). We here propose a paper-based sensor for the rapid detection of β-lactamase-producing bacteria in the urine samples of UTI patients. By conjugating a strongly electronegative group -N+(CH3)3 with the core structures of cephalosporin and carbapenem antibiotics, two visual probes were achieved to respectively target the extended-spectrum/AmpC β-lactamases (ESBL/AmpC) and carbapenemase, the two most prevalent factors causing antib

Acute Cystitis in a Transfeminine Patient: Assessment and Treatment of Urinary Tract Symptoms.

Management of acute cystitis in a transfeminine patient is discussed as an example of treatment of urinary tract infections (UTIs). The case is an introduction for clinicians who typically care for cisgender women and wish to expand the populations they serve to include care of gender-diverse individuals. This is supportive of the 2021 American College of Nurse-Midwives Position Statement on Health Care for Transgender and Gender Non-Binary People. Possible differential diagnoses for urinary symptoms in transfeminine patients are discussed, as well as relevant history taking, examination skill

Direct single-cell antimicrobial susceptibility testing of Escherichia coli in urine using a ready-to-use 3D microwell array chip.

Empirical antibiotic therapies are prescribed for treating uncomplicated urinary tract infections (UTIs) due to the long turnaround time of conventional antimicrobial susceptibility testing (AST), leading to the prevalence of multi-drug resistant pathogens. We present a ready-to-use 3D microwell array chip to directly conduct comprehensive AST of pathogenic agents in urine at the single-cell level. The developed device features a highly integrated 3D microwell array, offering a dynamic range from 102 to 107 CFU mL-1, and a capillary valve-based flow distributor for flow equidistribution in dis

Pivmecillinam with Amoxicillin/Clavulanic acid as step down oral therapy in febrile Urinary Tract Infections caused by ESBL-producing Enterobacterales (PACUTI).

BACKGROUND: Oral treatment alternatives for febrile urinary tract infections are limited in the era of increasing antimicrobial resistance. We aim to evaluate if the combination of pivmecillinam and amoxicillin/clavulanic acid is non-inferior to current alternatives for step-down therapy in adult patients with febrile urinary tract infection. METHODS: We plan to perform an investigator-initiated non-inferiority trial. Adult hospitalised patients treated with 1-5 days of intravenous antibiotics for acute febrile urinary tract infection caused by extended spectrum beta-lactamase (ESBL) producin

Urinary Tract Infections in Pregnant Individuals.

Urinary tract infection (UTI) is one of the more common perinatal complications, affecting approximately 8% of pregnancies (1, 2). These infections represent a spectrum, from asymptomatic bacteriuria, to symptomatic acute cystitis, to the most serious, pyelonephritis. The presence of UTIs has been associated with adverse pregnancy outcomes, including increased rates of preterm delivery and low birth weight. Screening for and treating asymptomatic bacteriuria have been shown in multiple studies to reduce the incidence of pyelonephritis in pregnancy (3-5). Given the frequency at which UTIs are e

Particularities of Urinary Tract Infections in Diabetic Patients: A Concise Review.

Diabetes mellitus is a chronic disease that, untreated or poorly controlled, can lead to serious complications, reducing life expectancy and quality. Diabetic patients are more likely to develop infections, including many common infections, but also pathognomonic ones such as emphysematous pyelonephritis, malignant otitis externa, mucormycosis and Fournier's gangrene. Considering the fact that diabetic patients experience more frequently urinary tract infections (UTIs) with a worse prognosis than non-diabetic people, we conducted a review study based on data in the literature, following the pa

Predictors of ultrasound abnormalities among women treated for community-onset acute pyelonephritis: a prospective study.

OBJECTIVES: Acute pyelonephritis (AP) is a common but potentially severe infection. It may be complicated by pyelocaliceal dilatation which requires emergency urine drainage. In outpatient care, ultrasound (US) appears to be the easier way to diagnose pyelocaliceal dilatation, though most APs will not be complicated and do not require immediate ultrasound. We aimed to identify predictors of ultrasound abnormalities in an outpatient AP cohort. METHODS: A prospective study was conducted from April 2006 to September 2019 in an outpatient care network. Patients aged 15 and over treated for AP wer

Construction of a patient decision aid for the treatment of uncomplicated urinary tract infection in primary care.

BACKGROUND: Uncomplicated urinary tract infection (uUTI) is very common among women in primary care. The risk of developing pyelonephritis remains low after uUTI, nonetheless, empiric antibiotic therapy is frequently prescribed for symptomatic purposes. This may lead to adverse effects and antibiotic resistance. Furthermore, patients may express the will to limit the use of antibiotics. Some European countries recommend discussing a delayed prescription with the patient and developing a shared decision. The aim of this study is to create a patient decision aid (PtDA) used in primary care setti
