On being poor and feeling poor: low socioeconomic status and the moral self.

Persons of low socioeconomic status generally experience worse health and shorter lives than their better off counterparts. They also suffer a greater incidence of adverse psychosocial characteristics, such as low self-esteem, self-efficacy, and self-mastery and increased cynicism and hostility. These population data suggest another category of harm to persons: diminished moral agency. Chronic socioeconomic deprivation can create environments that undermine the development of self and capacities constitutive to moral agency--i.e., the capacity for self-determination and crafting a life of one'

The diving bell meets the butterfly: identity lost and re-membered.

Jean Dominique Bauby, former editor of Elle, suffered a stroke to his brain stem that left him with locked-in syndrome. Subsequently, through blinking his left eye, he writes his memoir of this experience, The Diving Bell and the Butterfly. This paper explores the meaning of embodiment, especially as one's body bears upon one's personal identity. It explores the various challenges and threats to selfhood that result from Bauby's experience and recounts how Bauby rises to the challenge through his memory and imagination.

The Nuremberg Code: its history and implications.

The Nuremberg Code is a foundational document in the ethics of medical research and human experimentation; the principle its authors espoused in 1946 have provided the framework for modern codes that address the same issues, and have received little challenge and only slight modification in decades since. By analyzing the Code's tragic genesis and its normative implications, it is possible to understand some of the essence of modern experimental ethics, as well as certain outstanding controversies that still plague medical science.

A pathological view of disease.

This paper is a response to Christopher Boorse's recent defense of his Biostatistical Theory (BST) of health and disease. Boorse maintains that his concept of theoretical health and disease reflects the "considered usage of pathologists." I argue that pathologists do not use "disease" in the purely theoretical way that is required by the BST. Pathology does not draw a sharp distinction between theoretical and practical aspects of medicine. Pathology does not even need a theoretical concept of disease. Its focus is not theoretical, but practical; pathology's goal is to contribute to the healing

Medical informatics and the concept of disease.

This paper attempts to address the general question whether information technologies, as applied in the area of medicine and health care, have or are likely to change fundamental concepts regarding disease and health. After a short excursion into the domain of medical informatics I provide a brief overview of some of the current theories of what a disease is from a more philosophical perspective, i.e. the "value free" and "value laden" view of disease. Next, I consider at some length, whether health care informatics is currently modifying fundamental concepts of disease. To this question I wil

Animal farm--the ethics and public policy of xenotransplantation.

In 1997, over four thousand people on organ waiting lists lost their lives. The current organ shortage has led to the need to find alternative sources of organs for transplantation. Advances in immunosuppressive therapies and genetics have made xenotransplantation, the transplantation of animal organs into humans, an increasingly viable technology. However, the promise of xenotransplantation is tempered by the unspecified amount of risk that zoonotic pathogens could mutate and infect humans with potentially deadly diseases.

Clinical specificity and the non-generalities of science. On innovation strategies for neurological physical therapy.

How to improve clinical practice and, in particular, that of physical therapy? Currently, several strategies are used which all fit the label 'scientification'. These scientific strategies have to make physical therapy's clinical practice more homogeneous. Sometimes this homogenization is thought to be necessary for other strategies of innovation including effectiveness research. But it has also been suggested that more homogeneity in the clinic is already itself an improvement. In this article we comment on these strategies. More specifically, we direct our attention at attempts in physical t

Ecological Adverse Selection Theory: A Structural Framework for Understanding Health System Inequity.

The inverse care law-the persistent pattern whereby those with greatest health need receive the least care-has remained stubbornly intact for over fifty years. Despite universal health coverage commitments, health systems continue to concentrate disadvantage in hospitals and exclude the most vulnerable from effective care. This paper argues that we lack a mechanism-level explanation for why the inverse care law persists. It introduces Ecological Adverse Selection Theory (EAST) to fill this gap. Drawing on Akerlof's adverse selection framework, EAST argues that health systems are designed aroun

What Does It Mean to Be Cured?: Perspectives from Individuals Living with Sickle Cell Disease.

BACKGROUND: Hematopoietic stem cell transplantation (HSCT) and gene therapies for sickle cell disease (SCD) are frequently described as cures in scientific literature, clinical spaces, and media reports. Yet, little empirical research has examined how individuals living with SCD perceive the meaning of "cure". METHODS: Over a 24-month period, ethnographic observations were conducted at a single large clinical research site alongside semi-structured interviews with 25 participants (23 individuals living with SCD and 2 caregivers of children with SCD). Participants represented diverse treatment

"They Really Save Us From Ourselves" : Autism Researchers' Insights on Community Engagement and Ethics.

Autism research depends on the participation of autistic children and adults as well as their families, but researchers have not routinely involved autistic people in the planning or execution of research. Community-engaged research practices, standard in other fields such as areas of public health, are gaining acceptance in autism research, although some researchers and other stakeholders remain concerned about their appropriateness, feasibility, or utility. This study, part of a larger project on stakeholder views on the ethics of community engagement practices in autism research, examines r

The ethics of separation: conjoined twins, bodily autonomy, and abortion.

This essay examines a prominent claim in abortion ethics: that bodily autonomy alone suffices to justify abortion even if the fetus is granted the moral status of a person. Instead of revisiting Thomson's violinist, it introduces a comparator offered less as a direct analogy to pregnancy than as a stress test of-and possible counterexample to-that claim. It imagines conjoined twin sisters, one temporarily dependent on the other's body, though a safe separation allowing both to survive is expected within months. The supporting twin's condition imposes serious but non-catastrophic demands; immed

Walking the Middle Path : Problem, Policy, and Adaptation to Generative-AI in Academic Publishing.

The arrival of publicly-accessible generative-AI tools, such as OpenAI's ChatGPT, has significantly disrupted standard practice in education, research, and publishing. Academic publishers and journals, in particular, have responded by adopting policies which reflect both how they perceive the problems that generative-AI tools pose, and what kinds of solutions address these issues while remaining consistent with key scholarly values: integrity, transparency, and the primacy of human authorship. This article surveys the current state of generative-AI in academic publishing-including relevant pol

The Infected Homo Sacer: The (Auto)Immunity and Biopolitics of Haitian Zombies in Junot Díaz's "Monstro".

This article reconsiders the figure of the zombie in Junot Díaz's "Monstro" by shifting attention to its role as a biopolitical mechanism that determines who is protected, abandoned, and left to die. While existing scholarship has tended to read the zombie as an allegory of colonial trauma and racial violence, this study argues that zombification operates as a material technology of governance through which populations are classified, regulated, and exposed to varying degrees of precarity within neoliberal regimes. Drawing on Agamben's concept of homo sacer and Mbembe's necropolitics, the arti

Emotional AI in Chinese Healthcare: Benevolent Deception Machines and Their Medical Ethical Dilemmas.

This paper examines the ethical challenges posed by the application of emotional AI in Chinese healthcare, characterizing it as a "benevolent deception machine." The article argues that emotional AI's "optimism bias" and "simulated empathy" render it an efficient tool for executing "benevolent deception," giving rise to two primary ethical concerns: erosion of patient autonomy and the potential to trigger deep-seated crises of trust. Nevertheless, the paper contends that emotional AI can be justified within the Chinese sociocultural context-Confucian familism grants the family a central role i
