An audit tool for assessing the appropriateness of carotid endarterectomy.

BACKGROUND: To update appropriateness ratings for carotid endarterectomy using the best clinical evidence and to develop a tool to audit the procedure's use. METHODS: A nine-member expert panel drawn from all the Canadian Specialist societies that are involved in the care of patients with carotid artery disease, used the RAND Appropriateness Methodology to rate scenarios where carotid endarterectomy may be performed. A 9-point rating scale was used that permits the categorization of the use of carotid endarterectomy as appropriate, uncertain, or inappropriate. A descriptive analysis was under

ECMO in evolution: the impact of changing patient demographics and alternative therapies on ECMO.

The incidence of neonatal extracorporeal membrane oxygenation (ECMO) is decreasing nationally. This decrease is presumed to be a result of the emergence of alternative technologies such as high-frequency oscillatory ventilation (HFOV), nitric oxide (NO), and surfactant therapy as well as others. The purposes of the present report were to determine just how rapidly the demographics of ECMO are changing and to determine the impact of competing technologies on ECMO use. The authors reviewed their entire ECMO experience of 455 cases (370 neonatal, 38 pediatric, and 47 cardiac). The neonatal cases

Uses of the 1980 National Natality and Fetal Mortality Surveys for the assessment of obstetric technologies.

Because of the multipurpose utility of followback surveys, six Public Health Service (PHS) agencies including the Bureau of Health Care and Delivery Assistance (BHCDA) financially assisted the National Center for Health Statistics (NCHS), which conducted the 1980 National Natality Survey (NNS) and 1980 National Fetal Mortality Survey (NFMS). These are called "followback" surveys because NCHS uses mail questionnaires and telephone to follow back informants (mothers, hospitals, and attendants at delivery) and medical sources of radiation named by informants, identified on Certificates of Live Bi

Alginate-based biomaterials as versatile carriers for biomedical applications.

Alginate-based materials have attracted considerable attention in the medical industry due to their biocompatibility, biodegradability, and great potential in good health and well-being. Brown algae's alginate is a natural polysaccharide that forms hydrogels in the presence of divalent cations; hence, it finds employment as a carrier in several different biomedical applications. As an ideal vehicle for drug delivery, tissue engineering, wound repair, and cell encapsulation, alginate's chemical structure, gelling process, and physical characteristics are thoroughly examined. Functionalization a

Beyond the 'End of AIDS': What comes after 2030?

'Ending AIDS as a public health threat by 2030' has been one of the most powerful organising promises in global health, reshaping research, funding and policy. As the 2030 milestone approaches, however, this promise risks becoming a political end rather than an epidemiological or social success: a moment in which the problem is declared solved while the conditions that sustain the epidemic persist. This commentary critically examines how the 'End of AIDS' agenda has been produced and governed, and what is at stake in imagining what comes after 2030. Drawing on interdisciplinary scholarship and

Electrochemical sensing of cell viability: A scalable platform from 2D monolayers to 3D tissue models.

The trend toward animal-free testing in toxicology and drug development has accelerated the adoption of complex 3D in vitro models. However, standard colorimetric viability assays suffer from severe limitations due to chemical interferences. To address these challenges, we designed a versatile sensor platform based on screen-printed carbon electrodes to overcome optical bottlenecks by quantifying cell viability through the electrochemical reduction of MTS-formazan. The electrochemical sensor was validated in 2D human dermal fibroblasts and keratinocytes at different cell confluences. Important

Decoding the "Fingerprint" of Implant Materials: Insights into the Foreign Body Reaction.

Foreign body reaction (FBR) is a prevalent yet often overlooked pathological phenomenon, particularly within the field of biomedical implantation. The presence of FBR poses a heavy burden on both the medical and socioeconomic systems. This review seeks to elucidate the protein "fingerprint" of implant materials, which is generated by the physiochemical properties of the implant materials themselves. In this review, the activity of macrophages, the formation of foreign body giant cells (FBGCs), and the development of fibrosis capsules in the context of FBR are introduced. Additionally, the rela

Spatial transcriptomics using multiplexed deterministic barcoding in tissue.

Spatially resolved transcriptomics of tissue sections enables advances in fundamental and applied biomedical research. Here, we present Multiplexed Deterministic Barcoding in Tissue (xDBiT) to acquire spatially resolved transcriptomes of nine tissue sections in parallel. New microfluidic chips were developed to spatially encode mRNAs over a total tissue area of 1.17 cm2 with a 50 µm resolution. Optimization of the biochemical protocol increased read and gene counts per spot by one order of magnitude compared to previous reports. Furthermore, the introduction of alignment markers allowed seamle

Chronic interfacing with the autonomic nervous system using carbon nanotube (CNT) yarn electrodes.

The ability to reliably and safely communicate chronically with small diameter (100-300 µm) autonomic nerves could have a significant impact in fundamental biomedical research and clinical applications. However, this ability has remained elusive with existing neural interface technologies. Here we show a new chronic nerve interface using highly flexible materials with axon-like dimensions. The interface was implemented with carbon nanotube (CNT) yarn electrodes to chronically record neural activity from two separate autonomic nerves: the glossopharyngeal and vagus nerves. The recorded neural s

Addressing neurological disorders with neuromodulation.

Neurological disorders are becoming increasingly common in developed countries as a result of the aging population. In spite of medications, these disorders can result in progressive loss of function as well as chronic physical, cognitive, and emotional disability that ultimately places enormous emotional and economic on the patient, caretakers, and the society in general. Neuromodulation is emerging as a therapeutic option in these patients. Neuromodulation is a field, which involves implantable devices that allow for the reversible adjustable application of electrical, chemical, or biologica

Behavioral medicine in the prevention and treatment of cardiovascular disease.

Cardiac behavioral medicine is the application of behavioral and psychosocial principles to the prevention and treatment of heart disease. Most biomedical cardiovascular risk factors (e.g., high blood lipids, high blood pressure, diabetes) require behavioral and medical interventions. Other risks, including obesity, high-fat eating pattern, smoking, and inactivity, clearly require lifestyle change. Behavioral medicine screening and intervention have been applied to psychosocial risk factors such as depression, hostility, and social isolation. Appropriate assessment of risk factors is essential

Does family-centered maternity care empower women? The development of the woman-centered childbirth model.

Family-centered maternity care (FCMC) developed as a consumer reaction to the depersonalization of birth that had been the management standard for childbirth during the first half of the century. The philosophy and focus shifted from technologization to personalization, from birth as a biomedical event to birth as a normal developmental task. Proponents of FCMC focused on the emotional and psychosocial needs of the childbearing family but often defined the family as the traditional nuclear family. Because of this restrictive view of the family, care within FCMC defies simple definition and see
