Prevalence and correlates of DSM-5 eating disorders in patients with bipolar disorder.

OBJECTIVE: To determine prevalence rates and clinical correlates of current DSM-5 eating disorders in patients with bipolar disorder (BP). METHODS: Prevalence rates of current DSM-5- and DSM-IV-defined binge eating disorder (BED), bulimia nervosa (BN), and anorexia nervosa (AN) were assessed with the Eating Disorder Diagnostic Scale (EDDS) in 1092 patients with BP. Psychiatric illness burden was evaluated with five proxy measures of BP illness severity. Medical illness burden was evaluated with the Cumulative Index Rating Scale (CIRS). RESULTS: Twenty-seven percent of patients had a current …

Are anxiety disorders associated with a more severe form of bipolar disorder in adolescents?

BACKGROUND: Anxiety disorders are common among children and adolescents with bipolar disorder. Among adults, anxiety disorder comorbidity is associated with a more severe form of bipolar disorder and a poorer outcome. There is limited data on the effect of comorbid anxiety disorder on bipolar disorder among children and adolescents. AIM: To study the prevalence of anxiety disorders among adolescents with remitted bipolar disorder and examine their association with the course and severity of illness, global functioning, and quality of life. MATERIALS AND METHODS: We evaluated 46 adolescents w…

Comorbidity of bipolar and eating disorders: distinct or related disorders with shared dysregulations?

BACKGROUND: The co-occurrence of bipolar and eating disorders, though of major clinical and public health importance, remains relatively unexamined. METHODS: In reviewing the literature on this comorbidity, we compared bulimia, anorexia nervosa, bulimia nervosa, binge eating disorders and bipolar disorders on phenomenology, course, family history, biology, and treatment response. RESULTS: Epidemiological studies show an association between subthreshold bipolar disorder and eating disorders in adolescents, and between hypomania and eating disorders, especially binge eating behavior, in adults…

Bipolar I and bipolar II subtypes in older age: Results from the Global Aging and Geriatric Experiments in Bipolar Disorder (GAGE-BD) project.

OBJECTIVES: The distinction between bipolar I disorder (BD-I) and bipolar II disorder (BD-II) has been a topic of long-lasting debate. This study examined differences between BD-I and BD-II in a large, global sample of OABD, focusing on general functioning, cognition and somatic burden as these domains are often affected in OABD. METHODS: Cross-sectional analyses were conducted with data from the Global Aging and Geriatric Experiments in Bipolar Disorder (GAGE-BD) database. The sample included 963 participants aged ≥50 years (714 BD-I, 249 BD-II). Sociodemographic and clinical factors were co…

Several regions - 1p, 1q, 6q, 8p, 13q and 16p - have been previously reported to be linked to bipolar disorder. Our results suggest that dissection of the disease phenotype can enrich the harvest of linkage signals and expedite the search for susceptibility genes. This is the first large-scale linkage scan of bipolar disorder to analyze simultaneously bipola …

Toward the Definition of a Bipolar Prodrome: Dimensional Predictors of Bipolar Spectrum Disorders in At-Risk Youths.

OBJECTIVE: The authors sought to assess dimensional symptomatic predictors of new-onset bipolar spectrum disorders in youths at familial risk of bipolar disorder ("at-risk" youths). METHOD: Offspring 6-18 years old of parents with bipolar I or II disorder (N=359) and community comparison offspring (N=220) were recruited. At baseline, 8.4% of the offspring of bipolar parents had a bipolar spectrum disorder. Over 8 years, 14.7% of offspring for whom follow-up data were available (44/299) developed a new-onset bipolar spectrum disorder (15 with bipolar I or II disorder). Measures collected at ba…

Women With Bipolar Disorder are at Increased Risk of Sarcopenia.

INTRODUCTION: While bipolar disorder is associated with significant comorbidity, little is known about the musculoskeletal health of individuals with bipolar disorder. Thus, we aimed to investigate the association between bipolar disorder and muscle strength, mass, quality and sarcopenia. METHODS: Women with bipolar disorder (n = 106) were recruited from healthcare settings and age-matched to controls (n = 300), without bipolar disorder, from the Geelong Osteoporosis Study. Both groups, aged ≥ 20 years. were drawn from South-Eastern Australia. Bipolar disorder was identified using a clinical …

Contraceptive use in women of childbearing age with bipolar disorder.

INTRODUCTION: Unplanned pregnancies are frequent in women with bipolar disorder (BD) while perinatal period is associated with a higher risk of relapse. Effective contraception during childbearing age in BD could reduce the risk of unplanned pregnancy and its consequences. The present study studied the prevalence of contraceptive use and which sociodemographic and clinical variables affect its use in women with BD. METHODS: 796 women (18-50 years old) with BD were recruited and evaluated by the MONTRA survey in France among hospital and consultant psychiatrists. Sociodemographic, clinical and…

Circadian-Related Hypothalamic Structure Differs by Chronotype in Bipolar Disorder.

BACKGROUND: Circadian rhythm disturbances represent a core feature of bipolar disorder (BD), with evening chronotype as a marker for poorer outcomes. We hypothesized that BD psychopathology combined with evening chronotype would be associated with structural alterations in circadian-related hypothalamic regions, particularly the suprachiasmatic nucleus (SCN), specific to BD relative to other psychiatric diagnoses. METHODS: We investigated structural neuroimaging data from the UK Biobank (113 BD, 205 major depressive disorder, 91 psychotic disorders, 199 healthy control). The SCN-containing an…

Lifestyle and psychotherapy interventions for older adults with bipolar disorder.

PURPOSE OF REVIEW: Bipolar disorder (BD) in older adults presents unique clinical challenges, and nonpharmacological interventions are an important component of comprehensive care, with more favorable side-effect profiles. This review synthesizes recent evidence on lifestyle and psychotherapy interventions for older adults with BD. RECENT FINDINGS: Evidence derives predominantly from observational studies, with few interventional trials. Lifestyle analysis - largely from large-scale biobank cohorts - identifies smoking, sleep dysregulation, sedentary behavior, suboptimal diet, and weight extr…

Lived experiences of bipolar disorder and family caregiving in Pakistan.

Bipolar disorder (BD) is associated with substantial disability and caregiver burden. In Pakistan, prevalence is unusually high, and limited mental health services place families at the center of care. Cultural and religious beliefs strongly shape how the illness is recognized, understood and managed. This study explored how individuals living with BD and their caregivers understand and navigate the illness within this context. Semi-structured interviews were conducted with 12 adults diagnosed with BD (type I or II) and 12 caregivers recruited through a national registry. Open-ended questions …

Medicine Use Preceding Fatal Poisonings Among Individuals With Bipolar Disorder.

Pharmacotherapy may reduce overall suicide risk in bipolar disorder, but prescribed medicines can also be causative agents in suicidal and accidental poisonings. This nationwide, nested case-control study utilized data from several national registers in Finland in 1996-2018. In a cohort of 60 045 individuals aged 15-65 with bipolar disorder, cases with fatal poisoning due to medicines or illicit substances were matched by age and sex to four controls each. The association between fatal poisoning and medicine use in the prior 2 weeks was examined using conditional logistic regression. The 777 (…

Rest-Activity Rhythms and Suicidal Ideation in Pediatric Bipolar Disorder.

INTRODUCTION: Risk for suicide is disproportionately elevated among youth with bipolar spectrum disorder (BD), which is characterized by recurrent mood episodes and significant shifts in sleep and circadian rhythm. Disruptions in circadian rhythm, such as 24-h rest-activity patterns, are a relevant risk factor for SI, yet understudied in BD youth. The present cross-sectional study examined the associations between actigraphy-derived rest-activity rhythms (RARs) and self-reported suicidal ideation (SI) severity in a sample of 54 BD youth (aged 12-18 years). METHOD: Participants completed daily

Functional brain networks involved in fluid intelligence in bipolar disorder.

fMRI data were collected during the Raven's Standard Progressive Matrices (RSPM) to investigate the networks involved in fluid intelligence (Gf) in people with bipolar disorder (BD), compared to community controls. Brain networks were extracted using a multidimensional analysis method that provides network anatomical and temporal information about task-induced BOLD signal changes. Four networks were elicited. Two networks matched the multiple demand network (MDN), one of which consisted of the MDN desynchronized with all other networks and peaked earliest in the trial (∼ 6 s). The other networ…

Novel treatment strategies for the management of refractory bipolar disorder.

INTRODUCTION: Bipolar disorder is often treatable with conventional psychopharmacology. Despite the availability of numerous established treatments for bipolar disorder, cases of treatment resistance persist. The identification of optimal therapies proves challenging, as the term treatment resistance lacks a clear, standardized definition. Ambiguity in defining frequency, duration, and number of medication failures across the different subtypes of refractory bipolar disorder contributes to an inconsistent framework which negatively impacts both clinical decision making and clinical outcomes. …

Speech and Language Markers of Bipolar Disorder: Challenges and Opportunities.

BACKGROUND: Clinicians aspire to predict the emergence of Bipolar Disorder (BD) in a timely manner. To accomplish this, markers reflecting mental states that can be gathered non-invasively and at large scale are needed. Here, we systematically evaluate evidence relating speech-based markers to mood states in BD. METHODS: We searched Medline and Google Scholar for all published studies in English up to February 2026 on the use of speech markers in BD. We undertook thematic analysis on abstracts using topic modeling and a qualitative gap analysis to identify potential opportunities for future r…

Cognitive reserve and functional outcome in euthymic Bipolar I Disorder.

Functional impairment often persists in bipolar disorder despite symptomatic remission. Cognitive reserve (CR) has been proposed as a protective factor that may influence recovery. This cross-sectional study examined the association between CR and functional outcome among euthymic individuals with Bipolar I Disorder (BD-I). Eighty-four outpatients with DSM-5 BD-I attending a tertiary-care teaching hospital in Puducherry, India, were assessed using the Lifetime of Experiences Questionnaire (LEQ) and the Functioning Assessment Short Test (FAST). Higher CR was significantly associated with better…

Genomic relationship between polyendocrine metabolic ovarian syndrome and bipolar disorder.

Women with bipolar disorder (BIP) have a higher risk of developing polyendocrine metabolic ovarian syndrome (PMOS). Shared genetic architecture may underlie this comorbidity. Valproate, a mood-stabilizer commonly used to treat BIP, increases the risk of PMOS. Still, the mechanism underlying PMOS in BIP remains unknown. Here, we aimed to identify genetic variants shared between BIP and PMOS, as well as their interaction with valproate. We used the results of large-scale genome-wide association studies of BIP (41,510 cases and 354,340 controls), and PMOS (3609 cases and 229,788 controls). Using …

DNA methylation in bipolar disorder: mechanistic insights and translational challenges.

Bipolar disorder (BD) is a severe and recurrent psychiatric disorder characterized by alternating manic, hypomanic, and depressive episodes, frequent comorbidity, high relapse rates, and an increased risk of suicide. Although the pathophysiology of BD remains incompletely understood, increasing evidence suggests that DNA methylation may represent an important epigenetic regulatory layer involved in BD-related biological heterogeneity. DNA methylation alterations have been reported in genes related to dopamine, serotonin, glutamate, and gamma-aminobutyric acid (GABA) systems, suggesting a poten…
