Factors affecting the results of distal femoral fractures treated by retrograde intramedullary nailing.

OBJECTIVE: To assess the factors that affect the outcome of distal femoral fractures treated by retrograde intramedullary nailing. METHODS: Between November 1999 and June 2006, 30 patients with 31 distal femoral fractures (15 males and 15 females, 1 bilateral) were treated by retrograde nailing. Their ages ranged from 44 to 82 years old (average, 67.2 years old). The causes of injury were slipping downs in 17 patients, traffic accidents in 11, and falling from height in 2. The average interval from injury to operation was 1.8 days. There were 14 cases of A1 fracture, 7 cases of A2, 8 cases of

A retrospective case series of the Park-Heo technique for surgical fixation of distal phalanx base fractures.

Distal phalanx base fractures are difficult to manage because of strong tendon traction forces and the fragility of small bone fragments. Conventional fixation methods, such as Kirschner wires or pull-out sutures, may risk complications including skin necrosis and nail deformities due to dorsal incisions. To address these challenges, we applied a minimally invasive dorsal 2-point subcutaneous suture penetration technique, with the option of using a custom-made "M" endo-button to provide additional stability in complex cases. We conducted a retrospective review of patients with distal phalanx b

Successful Correction of Lower-Limb Malunions Using Intramedullary Nailing and Chipping Osteotomy: A Case Series.

BACKGROUND Lower-limb fracture malunion can result in angular deformity that requires surgical correction. Chipping corrective osteotomy (CCO) is a novel method that involves a chipping technique at the center of rotation of angulation, followed by single-stage internal fixation. This case series describes 3 cases of correction of fracture malunion of the lower limb managed using combined CCO and intramedullary nail stabilization. CASE REPORT Case 1: A 47-year-old man with a 17° varus deformity of the left tibia from a past accident had left knee pain. CCO was performed, achieving bone union i

Treatment of tibial shaft nonunion with a retained nail in morbidly obese patients.

PURPOSE: Morbid obesity is a rising worldwide health problem. Tibial shaft nonunion with retained intramedullary nail (IMN) in the morbidly obese is a challenging orthopaedic problem. Prolonged immobilization carries a significant risk of complications. This study aimed to investigate whether decortication and cancellous bone grafting with the application of a circular external fixator (CEF) over the preexisting IMN can achieve fracture union and avoid serious complications. METHODS: We retrospectively reviewed 27 consecutive morbidly obese patients treated for tibial diaphysis aseptic nonuni

Is the involvement of the distal interphalangeal joint in psoriatic patients related to nail psoriasis?

The aim of this study was to investigate the relationship between onychopathy and distal interphalangeal (DIP) joint involvement in psoriatic patients. Twenty-five consecutive unselected, unrelated patients with psoriatic onychopathy and 25 consecutive unselected, unrelated patients with psoriatic arthritis without onychopathy, were enrolled in the study. X-ray films of the hands were taken to identify DIP arthritic involvement and/or bone changes of the distal phalanx, which were categorized into five classes (0: no lesions; 1: tuftal minimal erosions; 2: tuftal bone resorption; 3: tuftal per

Triple Osteosynthesis Technique for Implant-Associated Infection with Nonunion in a Pediatric Open Ulna Fracture: A Case Report.

CASE: A 14-year-old boy with a closed radius and grade 2 open ulna fracture, plus ulnar nerve deficit, developed secondary osteomyelitis after initial plating. Our staged management involved segmental resection with external fixation, followed by fibular strut graft stabilized with flexible nailing and neutralization plate (triple osteosynthesis). CONCLUSION: In literature, seldom has a triple construct been used for such nonunions arising from osteomyelitis. The few existing instances mention using bone graft with either only intramedullary nail or only plate and screws, while we used all 3.

Presentation of a Humeral Shaft Fracture Treated by Locked Intramedullary Nailing With Unlocked Technique.

INTRODUCTION: Although intramedullary nailing (IMN) is used in a reamed or unreamed fashion for treatment of long bone fractures, the locked nails may also be used in the unlocked form if so decided by the orthopedic surgeon. CASE PRESENTATION: We describe a 50-year-old man who had a shaft fracture of his right humerus. The fracture was treated with a reamed, locked IMN using unlocked technique. CONCLUSIONS: The functional outcome 22 months post injury showed that although primary treatment method uses locked IMN in humeral shaft fractures, unlocked IMN can be used in appropriate cases. Onycholemmal carcinoma.

We report on a slowly growing malignant tumor of the nail bed epithelium in a 69-year-old male. On light microscopic examination, the tumor was composed of: (1) some small cysts filled with eosinophilic, amorphous keratin and lined by an atypical squamous epithelium devoid of a granular layer and (2) solid nests and strands of atypical keratinocytes filling the dermis and penetrating the phalangeal bone. Because the nail bed epithelium is comparable to the outer root sheath, or trichilemma of the hair follicle, and since the reported tumor showed some analogies with trichilemmal carcinoma, we

An autosomal recessive disorder with glomerular basement membrane abnormalities similar to those seen in the nail patella syndrome: report of a kindred.

We present a 9-year-old boy, the product of a consanguineous marriage, with proteinuria, edema, and microscopic hematuria with mild renal impairment since age 2 yr. Renal biopsy showed the histopathologic electron microscopic changes seen in hereditary osteo-onchyodysplasia (HOOD) [Hoyer et al, 1972; Bennet et al, 1973; DelPozo and Lapp 1970; Vernier et al, 1974; Morita et al, 1973] or "nail-patella syndrome." Neither the patient nor his relatives had bone or nail abnormalities; however, 2 other sibs had died of complications of end-stage renal disease at age 6 and 9, leading us to suspect aut

Intercalary allograft reconstructions using a compressible intramedullary nail: a preliminary report.

BACKGROUND: Although intercalary allograft reconstructions are commonly performed using intramedullary devices, they cannot generate compression across host-allograft junctions. Therefore, they sometimes are associated with gap formation and suboptimal healing conditions. QUESTIONS/PURPOSES: We describe a new technique and present preliminary results for intercalary allograft reconstructions for tumors using a compressible intramedullary nail. PATIENTS AND METHODS: We retrospectively reviewed 10 patients (19 host-allograft junctions) who underwent intercalary allograft reconstruction using t

Cement-a-TAN Temporary Proximal Femoral Replacement for Staged Joint Replacement - A Case Report.

INTRODUCTION: Infected non-unions of proximal femoral fractures are difficult to treat. If debridement and revision fixation is unsuccessful, staged revision arthroplasty may be required. Non-viable tissue must be resected; coupled with the introduction of an antibiotic-eluting temporary spacer before definitive reconstruction. Tissue microbiological diagnosis and targeted antibiotic therapy are required. In cases of significant proximal femoral bone loss, spacing options are limited. CASE REPORT: We present a case of a bisphosphonate-induced subtrochanteric fracture that progressed to infect

Limb salvage using distraction osteogenesis. A classification of the technique.

We report the results of distraction osteogenesis (callotasis) for the reconstruction of extensive defects after the excision of skeletal tumours in the limbs. Bone transport was performed in ten patients (five osteosarcomas and five giant-cell tumours), shortening-distraction in three (two osteosarcomas and one Ewing's sarcoma), and distraction osteogenesis combined with an intramedullary nail to reduce the time of external fixation in six (three osteosarcomas, two chondrosarcomas, and one malignant fibrous histiocytoma). The mean length of the defects after excision of the lesion was 8.4 cm.

Finite Element Analysis of Conventional Fixation and 3D-Printed Scaffold Integration for Treating Large Osseous Femoral Defects.

This study investigated the biomechanical efficacy of conventional and 3D-printed scaffold-augmented fixation methods for a large 6 cm osseous femoral defect. Finite element analyses were conducted to compare four conventional techniques: single plate, intramedullary nail, combined plate and nail, and double plate. These were then evaluated with the addition of three porous Ti-6Al-4V scaffold designs (Weaire-Phelan, Diamond, and Voronoi) with 70% porosity. Models were subjected to peak physiological loading from gait, simulating a 106 kg patient. Performance was assessed based on implant stres
