Pontine tubercular abscess: A rare presentation of CNS tuberculosis masquerading as glioma in a child.

Tuberculous brain abscess (TBA) in a child was initially misdiagnosed as glioma. Two craniotomies, abscess drainage, and anti-tubercular therapy led to recovery. Pontine TBA, though rare and atypical, can have better outcome with timely intervention.

Diffusion magnetic resonance imaging for enhanced visualization of malignant cerebral tumors and abscesses.

BACKGROUND: Several malignant tumors and brain abscesses have similar magnetic resonance imaging (MRI) findings. Because the prognosis and treatment strategies differ among these disorders, accurate preoperative diagnosis is very significant. Recently, diffusion-weighted imaging (DWI) and calculation of apparent diffusion coefficients (ADCs) have been reported to be useful to differentiate between the two disorders. MATERIALS AND METHODS: MRI and DWI were performed in 37 patients: 13 with glioblastomas, 14 with metastatic tumors, 7 with malignant lymphomas, and 3 with brain abscesses. The min…

Secondary encephalocele in an adult leading to subdural empyema.

Brain abscesses are an uncommon but potentially fatal infection. They can spread directly from an adjacent source or hematogenously from a distant source. Encephaloceles represent a rare form of neural tube defects that can potentially be complicated by the development of meningitis or brain abscess. We report a case of a 63-year-old female who presented with bilateral lower extremity weakness and was ultimately found to have a Streptococcus pneumoniae subdural empyema and an associated frontal lobe encephalocele extending through the left frontal sinus. She was treated with surgical drainage,…

Cladophialophora bantiana infection mimicking neuromyelitis optica.

Cladophialophora bantiana (C. bantiana) is a life-threatening melanized mycelial fungus causing brain abscess. C. bantiana is usually observed in tropical countries, including India. We report a Japanese case of C. bantiana presenting with myelitis mimicking neuromyelitis optica (NMO) and brain abscess. A 73-year-old man was administered prednisolone (30 mg/day) for antineutrophil cytoplasmic antibody (ANCA)-related vasculitis 100 days before admission. He had right side-dominant paraplegia and sensory loss in the right leg. T2-weighted spinal cord MRI revealed longitudinal high-intensity sign…

[A case report of epidural abscess caused by acute frontal sinusitis].

Epidural epidural abscess is usually secondary to acute frontal sinusitis and osteomyelitis of the frontal bone. Nose-derived intracranial connplications are less common than otogenic ones, and infections caused by nasal and sinus trauma, foreign bodies and surgery are more common than those caused by inflammation alone. The frontal lobe abscess caused by frontal sinusitis is more common in brain abscess with nasal intracranial complications.

Nocardial cerebral abscess associated with mycetoma, pneumonia, and membranoproliferative glomerulonephritis.

Nocardial brain abscesses remain a clinical challenge. We successfully treated a patient with nocardial brain abscess, mycetoma, pneumonia, and glomerulonephritis. Nocardial soft tissue involvement, mycetoma, is well known. However, the fact that actinomycetoma can metastasize may not be as well appreciated. The association between nocardiosis and glomerulonephritis should be better clarified.

Magnetic resonance spectroscopy findings of pyogenic, tuberculous, and Cryptococcus intracranial abscesses.

Proton magnetic resonance spectroscopy (MRS) complements conventional methods used to differentiate intracranial cystic lesions. We report MRS findings of three cases that were diagnosed as pyogenic, tuberculous, and Cryptococcus abscesses before instituting any medical or surgical therapy. The pyogenic brain abscess had typical specific spectral findings (i.e., the demonstration of amino acids). Lactate and lipid peaks were visible in the tuberculous abscess. Cryptococcus neoformans can appear differently in different brain regions, which may lead to different spectral findings.

[Otogenic cerebellar abscess - two case histories].

OBJECTIVE: The otogenic brain abscess remains a serious intracranial complication of otitis media despite advantages in diagnostics and antibiotic treatment during the last decades. We discuss the clinical picture and the possible ways of treatment by the example of two case histories. CASE HISTORIES: Case 1: We report about a 38-year-old man, who was suffering in cholsteatoma since 30 years. The surgical revision was restricted by the patient since this time. The acute symptoms of meningitis were caused by intracranial involvement and cerebellar abscess due to cholesteatoma. After surgical r…

Magnetization transfer imaging diagnosis of intracranial tuberculomas.

Tuberculomas (TB) can mimic brain tumors and abscesses. We performed prospective magnetization transfer ratio (MTR) analysis on 60 patients with 52 intracranial TB, 13 pyogenic abscesses and 65 tumors to determine the efficacy of magnetization transfer (MT) imaging in the differential diagnosis of brain tumors and infection. The nonenhancing cores of TB had significantly higher (P=.026) MTR (0.14+/-0.29) than necrotic components of high-grade gliomas (-0.19+/-0.22). The MTR of cores of TB were also higher than those of the cystic areas of low-grade gliomas (-0.53+/-0.32), benign (-0.09+/-0.21)

Intracranial complications of sinusitis in children. A sequela of periapical abscess.

The cases of two children with periapical abscess in the upper incisors, sinusitis, and intracranial abscess are described. The ethmoid and maxillary sinuses were involved in both patients. Subdural empyema occurred in both, and one of the children had also cerebritis and brain abscess. Anaerobic bacteria were isolated from the infected subdural empyemas. Peptostreptococcus intermedius and microaerophilic streptococci were recovered in one patient and Fusobacterium sp in the other. Surgical drainage and appropriate antimicrobial therapy resulted in complete eradication of the infection in both…

[A cerebral abscess due to Listeria monocytogenes in a 15-month-old infant].

We report a case of brain abscess due to Listeria monocytogenes in an infant. We recall that listeriosis is infrequent in pediatric patients beyond the neonatal period, that most cases occur in immunocompromised hosts, and that clinical features are non-specific in neuromeningeal forms. Management of brain abscesses is discussed. The role of the patient's general health condition seems to have a determinant influence on prognosis.

Spreading of multiple Listeria monocytogenes abscesses via central nervous system fiber tracts: case report.

Animal studies have shown that Listeria monocytogenes can probably access the brain through a peripheral intraneural route, and it has been suggested that a similar process may occur in humans. However, thus far, its spreading through the central nervous system (CNS) has not been completely elucidated. The authors present a case of multiple L. monocytogenes cerebral abscesses characterized by a pattern of distribution that suggested spread along white matter fiber tracts and reviewed the literature to identify other cases for analysis. They elected to include only those cases with 3 or more ce

Contrast-Enhanced Chest Computed Tomography Reveals Treatable Causes of Cerebral Abscesses in Patients without Antecedent Surgery or Trauma.

OBJECTIVE: Brain abscesses cause substantial morbidity and mortality even after appropriate therapy, and no underlying cause is found in 25% of cases. We investigated the added utility of contrast-enhanced chest computed tomography (CT) in the diagnostic work-up of patients presenting with cerebral abscesses and no history or prior trauma or cranial surgery. METHODS: All patients presenting to a single institution with a surgically proven brain abscess were reviewed. Concurrent contrast-enhanced chest CT imaging results were reviewed when available to identify treatable predisposing causes of…

[Cerebral aspergillosis].

BACKGROUND: The diagnosis of intracerebral fungal abscesses may be difficult due to the paucity of laboratory tests and similar radiological … abscesses, with almost complete resolution within 12 weeks. INTERPRETATION: Adequate management of brain abscesses requires correct identification of the causative agent, so that proper treatment can be initiated as soon as possible. MRI plays an important role in distinguishing between pyogenic and fungal brain abscesses. Headaches or focal neurological defic …

Oropharyngeal Necrobacillosis with Septic Thrombophlebitis and Pulmonary Embolic Abscesses: Lemierre's Syndrome in a New Zealand White Rabbit.

Lemierre's syndrome, oropharyngeal infections induced by anaerobic bacteria, leading to fatal septic thrombophlebitis of the internal jugular vein and pulmonary embolic abscesses in humans, was diagnosed in a 6-month-old, male, New Zealand White rabbit. After acute onset of anorexia, lethargy, and depression, the rabbit died suddenly despite emergency clinical care. Necropsy revealed swelling, necrosis, and abscess in the soft tissues around the left caudal mandibular ramus, oral mucosa, and molar teeth, with systemic embolic abscesses and necrosis, especially in the jugular vein, lungs, and b
