Exiting nerve root injury during full-endoscopic trans-Kambin's triangle lumbar interbody fusion (FE-KLIF) and how to avoid it:a multicenter study.

Full-endoscopic trans-Kambin's lumbar interbody fusion (FE-KLIF) is a type of minimally invasive fusion surgery that requires only a 14-mm skin incision for cage insertion. The only surgery-related complication of concern with FE-KLIF is exiting nerve root injury (ENRI). We investigated the incidence of ENRI in 131 cases that underwent FE-KLIF at 10 hospitals. After encountering 5 cases of ENRI, all of which occurred during insertion of an open cannula, we devised a novel type of open cannula, which we call a rescue cannula, to avoid ENRI. There were 5 cases of ENRI (5.7%) in the 88 patients w…

Progression of Respiratory Support Following Pediatric Extubation.

OBJECTIVES: High-flow nasal cannula and noninvasive positive pressure ventilation have become ubiquitous in contemporary PICUs. Practice patterns associated with the use of these modalities have not been well described. In this study, we aimed to describe the use of high-flow nasal cannula and noninvasive positive pressure ventilation in children after extubation and analyze the progression of usage in association with patient factors. Our secondary aim was to describe interventions used for postextubation stridor. DESIGN: Single-center retrospective cohort study. SETTING: A 36-bed quaternar…

Guiding surgical cannulation of the inferior vena cava with transesophageal echocardiography.

UNLABELLED: We studied 150 adult cardiac surgery patients to assess visualization of the venous cannula and the venous system by intraoperative transesophageal echocardiography and to register the incidence of cannulation of hepatic veins. The quality of images, the dimensions of the venous system, the position of the venous cannula, and the adequacy of venous return were registered. Acceptable image quality of the inferior vena cava and the right hepatic vein (RHV) was obtained in 95% and 87% of cases, respectively. Considerable individual variations were found in the dimensions of the venous…

Novel use of a flowable collagen-glycosaminoglycan matrix (Integra™ Flowable Wound Matrix) combined with percutaneous cannula scar tissue release in treatment of post-burn malfunction of the hand--A preliminary 6 month follow-up.

INTRODUCTION: Long-term function following severe burns to the hand may be poor secondary to scar adhesions to the underlying tendons, webspaces, and joints. In this pilot study, we report the feasibility of applying a pasty dermal matrix combined with percutaneous cannula teno- and adhesiolysis. PATIENTS AND METHODS: In this 6 month follow-up pilot study, we included eight hands in five patients with hand burns undergoing minimal-invasive, percutaneous cannula adhesiolysis and injection of INTEGRA™ Flowable Wound Matrix for a pilot study of this new concept. The flowable collagen-glycosamino…

[Acute respiratory distress due to tracheostomy tube migration].

Tracheostomy cannula care is of paramount importance in the daily management of tracheotomized patients. While some complications are commonplace, specific events can occur, often according to type of cannula. We herein report the case of a 71-year-old patient; following a lengthy stay in the intensive care unit, she had received a Safe T-Tube cannula designed to provide support in a stenotic trachea. At home, while suctioning her tracheal secretions, she suddenly experienced respiratory distress requiring a rapid intervention. On arrival, no seeable cannula was found, either in the tracheosto…

Complications of ileal cannulation in cats.

BACKGROUND AND PURPOSE: The anatomic position of the ileum is such that use of simple, noninvasive sampling techniques is difficult. Sampling of ileal contents by use of a cannula provides information about those contents-digestibility and fermentation patterns; however, complications with use of cannulas in horses and dogs have been reported. Therefore, cats were chosen as a laboratory model in which to study ileal cannulation. METHODS: Simple T-piece cannulas were surgically placed in the ileum of six adult cats. RESULTS: Postsurgical complications included cannula displacement and leakage…

The use of stab incisions for instrument access in laparoscopic operations.

BACKGROUND/PURPOSE: Traditional laparoscopic approaches require cannulas for instrument access to the abdominal cavity. This study reports the authors' experience using minimal access (MA) stab incisions, rather than cannulas, for insertion of laparoscopic instruments into the peritoneal cavity. METHODS: All patients undergoing MA procedures by the authors from November 1999 through March 2003 were included. Procedures included foregut, biliary, adrenal, splenic, colonic, and genitourinary operations. A single cannula was used for insertion of the telescope. In select cases, a second cannula …

Minitracheotomy: complications and follow-up with fibreoptic tracheoscopy.

Complications and changes in tracheal mucosa after minitracheotomy were evaluated in 28 patients. Tracheal mucosa was inspected fibreoptically after the insertion of a minitracheotomy cannula, and then at 3-day intervals until the cannula was removed. Thereafter, assessments were made every third day until the mucosa was considered normal. Three significant complications occurred: mediastinal puncture, paratracheal entrance of the cannula and subcutaneous emphysema. Difficulties at insertion of the minitracheotomy cannula were encountered in 15 of 28 patients (54%). Air flow detected through t…

Port site electrosurgical (diathermy) burns during surgical laparoscopy.

BACKGROUND: Direct and capacitive coupling of diathermy current have been reported as causes of occult injury during surgical laparoscopy. METHODS: In order to determine the incidence of electrosurgical injury adjacent to metal and plastic cannulas, skin biopsies at 19 port sites used for monopolar electrosurgery were analyzed for coagulative necrosis. Prior to surgery the cannulas were randomized to either metal or plastic. RESULTS: Coagulative necrosis was observed at nine electrosurgery port sites compared to only one control (chi2 = 4.872; df = 1; 0.05 > p > 0.02). Plastic cannulas affor…

Infection control during parenteral nutrition therapy.

Parenteral nutrition therapy can achieve an anabolic state in patients who are unable to maintain normal nitrogen balance; however, it may be associated with infectious complications. Infections may be related to contamination of the cannula and the cannula wound, of the infusate, or of other parts of the parenteral nutrition system. A variety of microorganisms has been associated with these infections. The exact mechanisms that initiate cannula-related infection are poorly understood. Susceptibility of the host, the method and site of cannula insertion, colonization of parenteral nutrition ca…

Examining Hearts Containing Left Ventricular Assist Devices at Necropsy.

There are no publications describing hearts at necropsy containing left ventricular assist devices (LVADs). The purpose was to study the relation of the LVAD cannula to the left ventricular (LV) cavity and wall. We studied the hearts at necropsy of 15 adults who had an LVAD inserted from 4 to 1,423 days (median 60) earlier. In 13 patients, the cannula had been inserted at an angle to the major longitudinal axis of the LV chamber, and in 11 patients, the orifice margin of the cannulas contacted the LV mural endocardium. In 3 patients, the LVAD cannula was inserted into the posterior wall, and, …

Decreasing the Incidence of Hypoxia and Airway Maneuvers During GI Procedures.

Many outpatient gastrointestinal procedures are completed with propofol anesthesia. A side effect of propofol is airway obstruction and subsequent hypoxia. This study was designed to determine whether the use of a high-flow nasal cannula is associated with a decreased incidence of hypoxia or airway obstruction in patients undergoing propofol sedation in the gastrointestinal laboratory with a STOP-BANG score ≥5. High-flow nasal cannula was administered at 70 L/min on 27 patients with a STOP-BANG score ≥5 receiving monitored anesthesia care sedation for an esophagogastroduodenoscopy, endoscopic …

Administering a glyceryl trinitrate infusion: big is not always best.

With the use of some simple calculations it can be demonstrated that the choice of cannula and initial glyceryl trinitrate (GTN) infusion rate for patients with acute left ventricular failure (LVF) requires some careful thought. If a GTN infusion is commenced at a rate of 1 ml/h, a critically unwell patient with a large cannula--for example, a grey cannula--will have to wait over 6 min for the drug to enter the body. This compares with 1.5 min for a pink cannula at the same infusion rate. If a large-diameter cannula is chosen for these patients, then a fast initial infusion rate should also be…

Comparison of hypoxemia, intubation procedure, and complications for non-invasive ventilation against high-flow nasal cannula oxygen therapy for patients with acute hypoxemic respiratory failure: a non-randomized retrospective analysis for effectiveness and safety (NIVaHIC-aHRF).

BACKGROUND: Optimization of preoxygenation procedure can help to secure the method of intubation by reducing the risks of severe hypoxemia and other problems. There is confusion for efficacy of non-invasive ventilation compared to high-flow oxygen therapy regarding occurrence of severe hypoxemia during the intubation procedure. The purpose of the study was to compare the difference between noninvasive ventilation and high flow oxygen therapy to prevent desaturation during laryngoscopy. METHODS: Patients underwent high-flow nasal cannula oxygen therapy (HCO cohort, n = 161) or non-invasive ven…

The impact of aortic/subclavian outflow cannulation for cardiopulmonary bypass and cardiac support: a computational fluid dynamics study.

Approximately 100 000 cases of oxygen deficiency in the brain occur during cardiopulmonary bypass (CPB) procedures each year. In particular, perfusion of the carotid and vertebral arteries is affected. The position of the outflow cannula influences the blood flow to the cardiovascular system and thus end organ perfusion. Traditionally, the cannula returns blood into the ascending aorta. But some surgeons prefer cannulation to the right subclavian artery. A computational fluid dynamics study was initially undertaken for both approaches. The vessel model was created from real computed tomography…
