The teenager with palpitations.

Palpitations can result from cardiac awareness (increased conscious perception of the heart beating) or from a fast or irregular cardiac rhythm. Most causes for palpitations in the teenager can be diagnosed with minimal testing. Patients with an abnormal ECG, non-sinus tachycardia, abnormal cardiac examination, concerning family history, or palpitations associated with activity or syncope should be referred to a pediatric cardiologist. This article discusses the evaluation, testing, and management of teenagers with palpitations. It also provides a general guideline for referral for subspecialt

Headache and cardiovascular disease: old symptoms, new proposals.

Evidence of a link between headache symptoms and cardiovascular disease has rapidly grown in recent years and it is of utmost importance for the cardiologist and neurologist to be aware of this intimate connection. A brief overview of different cardiovascular diseases (namely hypertension, stroke, coronary heart disease, patent foramen ovale, atrial septal defects, atrial septal aneurisms, mitral valve prolapse, and aortic and carotid disease) that may be related to headache is presented in this article. Proposed pathophysiological mechanisms for this association and landmark studies are revie

Congenital coronary artery abnormalities.

Congenital coronary artery abnormalities are rare and account for approximately 0.1 to 2% of congenital heart defects. They may pose significant risk of mortality or morbidity to the patient. The pediatrician and the pediatric cardiologist should be aware of their subtle but very serious presentations and diagnostic steps to be undertaken to pinpoint the diagnosis. Prevention of serious complications from these abnormalities can be achieved by making the appropriate diagnosis and performing timely surgical intervention. This review will discuss the most common congenital coronary artery abnorm

[Correlation between myocardial perfusion and coronary angiography].

Determining the severity of coronary heart disease is of great importance to the cardiologist. There is a very good correlation between the severity of coronary heart disease and perfusion abnormalities to the myocardium as determined by radioisotope studies. We present our experience with forty (40) cases of ischemic heart disease diagnosed with sestamibi and its correlation with the obstruction of the coronary arteries by angiography. There is good correlation between the sestamibi findings and the coronary artery angiography obstructions and our findings concur with those published in the l

[Ambulatory care of children with heart diseases].

The out-patient unit for pediatric cardiology is important in establishing diagnoses and caring for children with congenital heart disease. In addition it provides postoperative care and helps the local physician to decide whether a heart-catheter or a heart operation is necessary. Innocent murmurs in children can be recognized, cardiac arrhythmias analyzed and treated. In this sense the pediatric cardiology out-patient unit is expected to maintain the work of a locally practising pediatric cardiologist, as well as providing student and postgraduate training facilities in pediatric cardiology.

Premature Ventricular Contractions (PVCs): A Narrative Review.

Premature ventricular complexes/contractions (PVCs) are relatively common, occurring in 3%-20% of the general population, and are often found during work-up of palpitations or incidentally on routine electrocardiographic testing. They are usually considered benign but can be a sign of underlying cardiac disease and cause significantly impairing symptoms. While often thought of as a problem for the cardiologist, it is not uncommon for PVCs to be identified initially in a primary care or inpatient general medicine setting. This paper will review etiologies, epidemiology, evaluation, and manageme

Subglottic Mass as Isolated Manifestation of Relapsing Polychondritis at Presentation.

Relapsing polychondritis (RP) is a rare systemic disease that is characterized by recurrent episodic inflammation of the cartilaginous structures of the body, resulting in their progressive destruction and subsequent replacement with fibrotic scar. We present a case of RP that initially manifested with subglottic involvement and we propose an innovative strategy for the treatment for laryngeal RP in phase of active inflammation. A multidisciplinary approach (rheumatologist, otolaryngologist, immunologist, internist, cardiologist, etc) and adequate follow-up are essential. The timeliness of the

Prognosis in fetal tetralogy of Fallot.

The outcome in a series of 23 cases of tetralogy of Fallot diagnosed prenatally was compared to published figures for this condition when (a) identified postnatally or (b) treated surgically. There was a marked difference in the survival between these groups, with the highest mortality (75%) occurring when the diagnosis had been made prenatally. The high incidence of chromosomal and extracardiac anomalies in this group (60%) largely accounted for the discrepancy. This, and the potential for progressive changes in the anatomy of the defect during pregnancy, must be taken into account by the ped

Retinal emboli in patients with mitral valve prolapse.

Small fibrin-platelet emboli caused by prolapsed mitral valves may cause retinal occlusive disease with a wide range of ophthalmic manifestations, including amaurosis fugax in young people, retinal or choroidal arteriolar occlusion, and retinal neovascularization (atypical Eales' disease). Six patients with retinal occusive disease underwent extensive noninvasive cardiac and systemic tests and were found to have prolapsed mitral valves. Patients with unexplained ocular emboli should be examined by a cardiologist for possible mitral valve prolapse.

Awareness and practice of physical activity in Omani children with CHDs: a cross-sectional study.

BACKGROUND: CHDs, affecting 1.1% of newborns, are the most prevalent congenital anomalies. Improved survival rates expose children with CHD to long-term risks such as metabolic and acquired cardiovascular disorders. Despite physical activity's benefits, participation is often limited by real and perceived safety concerns. This study evaluates awareness and practice of physical activity among Omani children with CHD. AIM: To quantify physical activity levels and identify influencing factors in Omani children with CHD. METHODS: This cross-sectional study, conducted at major Omani paediatric ca

A Patient and Physician Perspective of Chronic Coronary Syndrome and Persistent Stable Angina in Brazil.

Obstructions in the coronary arteries, arterial vasospasm, and microcirculation dysfunction may cause chronic coronary syndrome (CCS). Angina is a cardinal symptom of CCS and the journey of patients with angina is often filled with challenges and uncertainties. Diagnosis and treatment differences seem to vary between men and women with angina. This article is co-authored by a male and a female patient diagnosed with persistent stable angina and their treating cardiologist at the Heart Institute in São Paulo, Brazil. The patients share their personal experiences of more than 20 years of coping

Nonauscultatory clinical criteria are sensitive for cardiac pathology in low-risk paediatric heart murmurs.

BACKGROUND: Healthy children with likely innocent heart murmurs are frequently referred to cardiologists for reassurance. Existing guidelines that advise against these referrals are not consistently followed partly because they involve subjective auscultatory judgements with which many care providers are uncomfortable. Here, we investigate whether clinical criteria with no subjective auscultatory component are sensitive for cardiac pathology. METHODS: A retrospective chart review was performed of all new patients seen in our paediatric cardiology clinic for assessment of a murmur from January…

Realistic Vascular Replicator for TAVR Procedures.

Transcatheter aortic valve replacement (TAVR) is an over-the-wire procedure for treatment of severe aortic stenosis (AS). TAVR valves are conventionally tested using simplified left heart simulators (LHS). While those provide baseline performance reliably, their aortic root geometries are far from the anatomical in situ configuration, often overestimating the valves' performance. We report on a novel benchtop patient-specific arterial replicator designed for testing TAVR and training interventional cardiologists in the procedure. The Replicator is an accurate model of the human upper body vasc…

Estimating pressure gradients by auscultation: How technology (echocardiography) can help improve clinical skills.

AIM: To extend our previously-published experience in estimating pressure gradients (PG) via physical examination in a large patient cohort. METHODS: From January 1, 1997 through December 31, 2009, an attending pediatric cardiologist compared clinical examination (EXAM) with Doppler-echo (ECHO), in 1193 patients with pulmonic stenosis (PS, including tetralogy of Fallot), aortic stenosis (AS), and ventricular septal defect (VSD). EXAM PG estimates were based primarily on a murmur's pitch, grade, and length. ECHO peak instantaneous PG was derived from the modified Bernoulli equation. Patients w
