Endoscopic endonasal transmaxillary-pterygoid approach for skull-base non-vestibular schwannomas in 10 consecutive patients.

PURPOSE: Non-vestibular schwannomas (NVSs) of the skull base occur in several sites, and few previous studies have evaluated the usefulness of the endoscopic endonasal transmaxillary-pterygoid approach (EETMPA) to resect these lesions. We aimed to evaluate the characteristics and clinical outcomes of patients who underwent EETMPA for skull-base NVSs and to investigate the efficacy, safety, and indications for the procedure. METHODS: We retrospectively reviewed the clinical data of 10 consecutive patients (mean age, 45 ± 17) who underwent EETMPA for skull-base NVSs at the University of Tsukuba

Tuberculous perihepatic abscess and neurosarcoidosis: report of 2 uncommon manifestations of 2 common granulomatous diseases in 1 patient.

Infections caused by pathogens of the Mycobacterium tuberculosis complex, i. e., tuberculosis (TB), and the non-infectious, autoimmune disease sarcoidosis are among the most common granulomatous diseases worldwide. Typically, the lung is the primary site of infection and manifestation, respectively which makes the two diseases important differential diagnoses. Both diseases can affect virtually all organ systems, albeit with significantly lower incidence. CASE PRESENTATION:  We report the case of a 50-year-old Indian man presenting with a tuberculous perihepatic abscess and a systemic inflamma

Application of a Novel and Sustainable Silicate Solution as an Alternative to Sodium Silicate for Clay Swelling Inhibition.

Shale swelling during drilling operations causes many problems mainly related to wellbore instability. The oil-based muds (OBMs) are very effective in controlling the swelling potential of clay-rich shale formation, but their environmental concerns and the economic aspects curtail their usage. In the application of water-based mud (WBM), it is mixed with various swelling inhibitors such as inorganic salts (KCl and NaCl), sodium silicate, polymers, and amines of various types. The above-mentioned materials are however afflicted by some limitations in terms of their toxicity, their effect on dri

The distribution of important sero-complexes of flaviviruses in Malaysia.

Flaviviruses (FVs) are arthropod-borne viruses of medical and veterinary importance. Numerous species of FVs have been isolated from various host; mainly humans, animals, ticks, and mosquitoes. Certain FVs are extremely host-specific; at the same time, some FVs can infect an extensive range of species. Based on published literatures, 11 species of FVs have been detected from diverse host species in Malaysia. In humans, dengue virus and Japanese encephalitis virus have been reported since 1901 and 1942. In animals, the Batu Cave virus, Sitiawan virus, Carey Island, Tembusu virus, Duck Tembusu v

First comparative transcriptomic analysis of wild adult male and female Lutzomyia longipalpis, vector of visceral leishmaniasis.

Leishmaniasis is a vector-borne disease with a complex epidemiology and ecology. Visceral leishmaniasis (VL) is its most severe clinical form as it results in death if not treated. In Latin America VL is caused by the protist parasite Leishmania infantum (syn. chagasi) and transmitted by Lutzomyia longipalpis. This phlebotomine sand fly is only found in the New World, from Mexico to Argentina. However, due to deforestation, migration and urbanisation, among others, VL in Latin America is undergoing an evident geographic expansion as well as dramatic changes in its transmission patterns. In thi

Overview, prevention, and treatment of rabies.

Each year, approximately 55,000 individuals worldwide die from an infection due to the rabies virus. Rabies is a life-threatening disease caused by an RNA virus that is usually transmitted to humans through bites from rabid animals. More recently, reports of transmission by means of organ transplantation have been reported. Since human rabies is nearly 100% fatal if prophylactic measures are not followed, an increased awareness of who should receive prophylaxis and when prophylaxis should be administered is necessary. Preexposure prophylaxis entails the administration of the rabies vaccine to

[Management of bone-invasive en-plaque petrosal meningiomas. Removal using tailored petrosectomy].

BACKGROUND AND PURPOSE: The invasion of bone responsible for hyperostosis is a typical phenomenon associated with en-plaque meningiomas. Although the sphenoid wing and spheno-orbital region are most frequently affected, petrosal involvement is rare and its surgical treatment difficult. Hyperostosis is caused by bone invasion, is responsible for the clinical signs, and prompts the surgeon to use an à la carte drilling that has to be evaluated preoperatively and carried out depending on tumor extension and the treatment goals. METHODS: We report two cases of invasive and evolving en-plaque petr

Small bowel transplantation in mice.

Since 1990, the development of tacrolimus-based immunosuppression and improved surgical techniques, the increased array of potent immunosuppressive medications, infection prophylaxis, and suitable patient selection helped improve actuarial graft and patient survival rates for all types of intestine transplantation. Patients with irreversible intestinal failure and complications of parenteral nutrition should now be routinely considered for small intestine transplantation. However, Survival rates for small intestinal transplantation have been slow to improve compares increasingly favorably with

Prolonged administration of oral etoposide in patients with relapsed or refractory small-cell lung cancer: a phase II trial.

Twenty-two patients with recurrent small-cell lung cancer (SCLC) were treated with single-agent etoposide 50 mg/m2/d by mouth for 21 consecutive days. Eleven patients had received previous chemotherapy with cyclophosphamide, doxorubicin, and vincristine (CAV) or etoposide (CAE) or both (CAVE). Four of the latter patients also received salvage treatment with cisplatin and etoposide (EP). Nine patients had been treated with EP as induction therapy, while two patients had received high-dose cyclophosphamide, etoposide and cisplatin (HDCEP). Altogether, 18 patients had received previous intravenou

The fine structure of the gall bladder epithelium of the mouse.

Sections of mouse gall bladder epithelium fixed by perfusion with buffered osmium tetroxide have been studied in the electron microscope as an example of simple columnar epithelium. The free surface presents many microvilli, each presenting a dense tip, the capitulum, and displaying a radiating corona of delicate filaments, the antennulae microvillares. Very small pit-like depressions, representing caveolae intracellulares, are encountered along the cell membrane of the microvilli. The free cell surface between microvilli shows larger cave-like depressions, likewise representing caveolae intra

A Systematic Intracranial Exposure-Response Evaluation of Brain-Penetrant Inhibitors: Informing Dose Selection for Brain Metastases.

Brain metastases (BMs) represent a major clinical challenge due to their high incidence in cancers such as non-small cell lung cancer (NSCLC) and breast cancer, and the limited efficacy of systemic therapies resulting from inadequate drug penetration across the blood-brain barrier. Although several brain-penetrant tyrosine kinase inhibitors (TKIs) have been developed, the quantitative relationship between intracranial drug exposure and intracranial clinical response has not been well established, leaving an important gap in dose selection strategies. In this study, we systematically evaluated

Discovery of natural products that modulate signaling in patient-derived cells.

Natural products derived from biosynthetically apt microorganisms represent an important source for chemotherapeutic agents founded first via non-specific, cytotoxicity-guided isolation, then investigated for translational potential. Analyzing biosynthetic gene clusters within microbes predicts significant untapped potential of undiscovered therapeutically relevant natural products; however, stimulating biosynthesis of such compounds in sufficient quantities for isolation, structure determination, and biological assessment remains a gap. We address this with a bioactivity discovery pipeline th
