A novel route of revascularization in basilar artery occlusion and review of the literature.

Ischemia of the basilar artery is one of the most devastating types of arterial occlusive disease. Despite treatment of basilar artery occlusions (BAO) with intravenous tissue plasminogen activator, antiplatelet agents, intra-arterial therapy or a combination, fatality rates remain high. Aggressive recanalization with mechanical thrombectomy is therefore often necessary to preserve life. When direct access to the basilar trunk is not possible, exploration of chronically occluded vessels through collaterals with angioplasty and stenting creates access for manual aspiration. We describe the firs

Mechanical thrombectomy with the solitaire stent at Lyon, France.

BACKGROUND/AIMS: The Solitaire stent has been suggested as a promising device to perform intracranial thrombectomy in large vessels. We report our experience. METHODS: Consecutive patients in whom a thrombectomy with Solitaire stent had been performed for acute ischemic stroke in the Lyon Stroke Unit, France, from November 2009 to November 2010 were enrolled. RESULTS: There were 12 patients with a mean age of 66 years and a mean baseline National Institutes of Health Stroke Scale score of 17.5. There were 10 cases of anterior cerebral artery and 2 cases of basilar artery occlusion. The mean

Entrapment of a micro-guidewire during stenting of basilar stenosis.

Entrapment of a microwire related to intracranial endovascular therapy is an uncommon complication. A 64-year-old man with symptomatic basilar artery stenosis was treated with stenting. A 300 cm Synchro microwire became trapped in the P1 segment of the left posterior cerebral artery during the procedure. The decision was made to leave the microwire in place, considering that aggressive retrieval procedures could cause injury to the cerebral vasculature. The entrapped microwire was later inadvertently dislocated and removed while pulling the microwire back into the femoral artery with a looped

Hemorrhagic moyamoya disease in children: clinical features and surgical outcome.

OBJECTIVE: The clinical presentation of moyamoya disease (MMD) typically includes cerebral ischemia in children and intracranial hemorrhage in adults. Because of its rarity, the benefit of surgery in the hemorrhagic type of pediatric MMD has not been clearly established. The purpose of this study was to delineate the clinical features and surgical outcome of hemorrhagic MMD in children. METHODS: Thirteen patients (six boys and seven girls with a combined mean age of 11.1 years and a range of 4-18 years) were retrospectively confirmed as having MMD with intracranial hemorrhage between 1988 and

Arterial bypass surgery using a spontaneously formed "bonnet" superficial temporal artery in a patient with symptomatic common carotid artery occlusion: case report.

BACKGROUND: Common carotid artery (CCA) occlusive disease may cause hemodynamic cerebral ischemia resulting in the development of ischemic symptoms. The blood flow in the superficial temporal artery (STA) ipsilateral to the occluded CCA is usually poor, which limits its use as a donor artery for extracranial-intracranial arterial bypass surgery. CLINICAL PRESENTATION: Despite antiplatelet therapy, recurrent transient ischemic attacks manifesting as motor aphasia developed in a 72-year-old man. Neuroradiological imaging revealed misery perfusion in the bilateral cerebral hemispheres caused by

Minimally invasive superficial temporal artery to middle cerebral artery bypass through an enlarged bur hole: the use of computed tomography angiography neuronavigation in surgical planning.

The authors describe their minimally invasive technique for performing a superficial temporal artery (STA) to middle cerebral artery (MCA) bypass, which relies on an enlarged bur hole (2-2.5 cm) rather than the standard craniotomy. They perform this procedure in a minimally invasive fashion, using CT angiography for intraoperative neuronavigation as well as for preoperative identification of the donor and recipient vessels and planning of bur hole location. They present 2 cases in which this procedure was used, including one involving a patient with multivessel occlusive disease and significan

Early revascularization in a patient with perfusion computed tomography/diffusion-weighted magnetic resonance imaging mismatch secondary to acute vertebral artery occlusion. Case report.

A 67-year-old woman with a recent history of recurrent ischemic stroke secondary to right vertebral artery stenosis suffered acute onset of left homonymous hemianopsia and the medial longitudinal fasciculus syndrome, which resolved with hyperdynamic therapy. However, consciousness deteriorated 6 hours later. Perfusion computed tomography (CT) revealed regions of prolonged mean transit time in the bilateral cerebellar hemispheres, vermis, brainstem, and right occipital lobe, which were more extensive than the ischemic lesions demonstrated by diffusion-weighted magnetic resonance (MR) imaging. [A case of dural AVM detected after STA-MCA anastomosis].

A case of dural arteriovenous malformation (AVM) in the posterior cranial fossa detected after STA-MCA anastomosis surgery. A 52-year-old male consulted a neighbourhood hospital for sudden headache and vomiting. He was diagnosed as having intraventricular hemorrhage on CT scan. Though the obstruction of the right internal carotid artery was revealed angiographically, his symptoms improved after conservative therapy. Two weeks after onset, his consciousness deteriorated and he developed left hemiparesis. Thereafter, he was transferred to our hospital. After thorough examination, right STA-MCA a

Reoperation for moyamoya disease refractory to encephalo-duro-arterio-synangiosis.

Three cases of Moyamoya disease successfully treated by reoperation are reported with special reference to the operative techniques in the second operation. These children first underwent encephalo-duro-arterio-synangiosis (EDAS). Because two of the three postoperative collateral formations through EDAS were either poor or not present at all and their symptoms remained. Encephalo-myo-synangiosis (EMS) was later added in the posterior frontal and/or parietal regions of the same side as the former EDAS. In the remaining one, the collaterals through EDAS were well formed but transient ischaemic a

[Cerebral bypass surgery].

When an obstruction of a cerebral vessel is hemodynamically relevant and insufficiently collateralized--and when the mechanisms of compensation are exhausted--it can lead to ischemia. The second and more common way a vascular obstructive lesion can become symptomatic is the shedding of emboli to the periphery. The extra-/intracranial arterial bypass (EIAB), most often constructed as an anastomosis between the superficial temporal artery (STA) and a cortical branch of the middle cerebral artery (MCA), increases cerebral blood flow when all the mechanisms of compensation are exhausted. When not,

Amaurosis fugax: some aspects of management.

Fifty-one patients with amaurosis fugax were followed for a mean of 4.9 years from their first attack. Females predominated under the age of 50 years in contrast to men who presented in the older age group. Minor permanent visual sequelae occurred in only three patients, but cerebral vascular symptoms were present at some time in just under half the group. A range of abnormalities was identified on cerebral angiography and some of these correlated with certain clinical findings. Twenty patients were treated by surgery and permanent relief of significant improvement in symptoms occurred in 14.

[Detection and Management of Asymptomatic Cerebrovascular Disease].

Asymptomatic carotid or intracranial arterial stenosis, silent brain infarction, cerebral white matter lesions, and cerebral microbleeds are well-established predictors of stroke. When these findings are detected through brain screening programs ("brain dock"), aggressive management of vascular risk factors is warranted. In cases of severe carotid stenosis, revascularization procedures such as carotid endarterectomy or carotid artery stenting may be considered. In East Asia, including Japan, genetic predisposition appears to play a significant role. The moyamoya disease-related RNF213 p.R4810K

Stenosis of the proximal external carotid artery in an adult with moyamoya disease: moyamoya or atherosclerotic change?

A 55-year-old woman presented with moyamoya disease manifesting as recurrent transient ischemic attacks despite taking aspirin and antihypertensive agent. Angiography showed the characteristic angiographic appearance with bilateral internal carotid artery occlusion and abnormal collateral vessels. Left external carotid angiography demonstrated moderate stenosis of the proximal external carotid artery (ECA). A self-expandable stent was successfully placed in the left ECA to improve ipsilateral cerebral perfusion. The patient had an uneventful outcome after a 1-year follow up. Involvement of the

Cerebral autoregulation studies in clinical practice.

During the past 15 years several paradigms to study dynamic cerebral autoregulation (CA) were developed by measuring cerebral blood flow (CBF) velocity with transcranial Doppler (TCD) in response to blood pressure changes. As a more indirect approach to measure autoregulation, vasomotor reactivity (VMR) can be determined by the use of vasodilatory stimuli. CA or VMR are often severely disturbed in occlusive carotid artery disease. Several prospective studies have shown that reduced VMR is an important risk factor for stroke or TIA in patients with symptomatic and asymptomatic carotid artery st

Letter to the Editor. Effect of encephalomyosynangiosis procedure on post stroke recovery in a permanent model of ischemic stroke.

This correspondence comments on the methodological issues of Anthony et al. on encephalomyosynangiosis (EMS) for acute ischemic stroke in mice. The central issue is the clinically unrealistic intervention time of 4 h post-occlusion, which limits the translational relevance of the reported benefits in infarct reduction and recovery. Furthermore, the surgical design-specifically, the differential handling of the temporalis muscle between groups-introduces a potential confounder, as the excision in controls may alter baseline intracranial pressure compared to the preserved muscle in the EMS group

Artificial dural regeneration matrix as a substitute for autologous tissue in indirect bypass in Moyamoya disease: Investigation of a rat model of chronic cerebral hypoperfusion.

Indirect bypass using autologous tissue is effective in Moyamoya disease, especially among pediatric patients. This study aimed to evaluate the effectiveness of indirect bypass using DuraGen (absorbable artificial dura mater composed of collagen matrix), as a substitute for autologous tissue in a rat model of chronic cerebral hypoperfusion. Male Wistar rats were subjected to bilateral internal carotid artery occlusion and divided into three groups: a control group without bypass surgery, a group wherein indirect bypass was performed using the temporalis muscle (encephalo-myo-synangiosis [EMS]

Flow-Augmentation Bypass Surgery: Indications and Decision-Making.

Flow-augmentation bypass aims to enhance blood flow to hypoperfused brain regions in patients with cerebrovascular steno-occlusive diseases. This surgical procedure is indicated for ischemic and hemorrhagic moyamoya vasculopathy and for selected patients with chronic steno-occlusive disease and acute ischemic stroke. Flow-augmentation bypass has been rigorously evaluated in randomized clinical trials, including the EC-IC Bypass Trial, the Carotid Occlusion Surgery Study (COSS), the Japanese Adult Moyamoya (JAM) trial, and the recent Carotid and Middle Cerebral Artery Occlusion Surgery Study (C
