We report a case of glaucoma following scleral buckling that required filtering trabeculectomy. Despite anterior entry to the anterior chamber, swollen ciliary processes were incarcerated in the sclerostomy site, blocking aqueous flow. Resection of the occluding ciliary processes resulted in restoration of aqueous flow through the sclerostomy. This case demonstrates that anterior rotation of the ciliary body and processes plays an important role in the pathogenesis of glaucoma following scleral buckling operation.

The eyes of an 89-year-old male were obtained for postmortem examination five months after implantation of a foldable silicone elastic lens implant. Two months after surgery, the implant had been repositioned from capsular bag to ciliary sulcus fixation to correct postoperative decentration and tilt. Pathological examination showed both haptics located in the ciliary sulcus. There was no significant inflammatory reaction in the adjacent uveal tissue and no erosion of the haptics into the iris or ciliary body.

The space between the posterior surface of the base of the iris and the anterior surface of the ciliary body - the ciliary sulcus - and the capsular bag are the sites for a posterior chamber lens implant after extracapsular cataract extraction. In human cadaver eyes and in eyes after cataract extraction, the exact topographical relationship of the ciliary sulcus and the surrounding tissue was analyzed. The anatomical findings and important variations are documented by light microscopy and macrophotography and the relationship to lens implantation is discussed.

After local application of gentamycine, dexamethasone and tropicamide to rat eyes, no effect was observed on the permeability of the vascular wall of the iris and ciliary body compared with fluorescein-Na. Nor were there any changes in vessel caliber. The constriction of the dye column observed in corneal neovascularizations was not seen in the (normal) iris vessels and the vessels of the ciliary body. With regard to the iris region, however, this might be due to mechanical causes and/or the possibility that vessels of different origins react differently.
