Computer-assisted Femur Fracture Reduction.

INTRODUCTION: When using intramedullary nailing of femur fractures false axis alignment or malrotation is the greatest deterrent to successful reduction often causing extended x-ray exposure and the need to revert to an open procedure. The aim of this study was to demonstrate the advantages of using a trauma navigation system during intramedullary nailing of femur fractures. MATERIALS AND METHODS: A fluoroscopic navigation unit was used and successful closed fracture reduction, x-ray exposure, and operation time were measured. Between 2004 and 2007 19 patients receiving intramedullary nailing…

Mandibular fractures: a review of 156 consecutive cases.

This report deals with a consecutive series of 156 mandibular fractures that were managed during a three-year period. The age and sex incidence are presented and the causative factors of the trauma are discussed. The fractures are classified according to anatomic site and laterality. The majority of the patients were treated with closed reduction techniques and the reasons for open reduction are tabulated. The results and complications of the treatment are enumerated. A conservative technique is described for open reduction of fracture-dislocations of the condylar head.

The use of external fixation for the management of acute and chronic Monteggia fractures in children.

PURPOSE: The main objective of this study was to retrospectively evaluate the clinical and radiological outcomes of acute (AMF) and chronic Monteggia fractures (CMF) in children treated by closed or open reduction and external fixation (EF). METHODS: This is a retrospective review of 26 patients with Monteggia fracture. Patients with AMF (time between trauma and surgery less than two weeks) were treated by closed reduction and EF of the ulna (Group A; 15 patients) while those with CMF (time between trauma and surgery more than three weeks) were managed by closed or open reduction and EF of th

Iatrogenic open humeral shaft fractures following functional bracing.

INTRODUCTION: The treatment of closed humeral shaft fractures tends to be successful with functional bracing. Treatment failure due to iatrogenic conversion to an open fracture has not been described in the literature. We present a case series of patients that experienced open humeral shaft fractures after initially being treated with functional bracing for closed humeral shaft fractures and describe what factors are associated with this complication. MATERIALS AND METHODS: This was a retrospective case series performed at three level 1 trauma centers across North America. All nonoperatively

Surveillance ultrasonography for conservative treatment of femoral shaft fractures in young children.

BACKGROUND: The treatment for femoral shaft fracture (FSF) depends on the age of the patient. While the Pavlik harness is the first choice for patients under 6 months of age, spica casting is preferred for patients over 6 months and under preschool age. Minimally-invasive surgery using elastic stable intramedullary nails is also used in some cases. Skin traction is another treatment choice for some patients who are not candidates for the above methods. This study aimed to evaluate the feasibility of surveillance ultrasonography (US) for the conservative treatment of FSFs in young children. MA

Optimal postoperative immobilisation for supracondylar humeral fractures.

BACKGROUND: Supracondylar humeral fractures (SCHFs) are very common in paediatric patients. In France, percutaneous fixation with two lateral-entry pins is widely used after successful closed reduction. Postoperative immobilisation is typically with a long arm cast combined with a tubular-bandage sling that immobilises the shoulder and holds the arm in adduction and internal rotation to prevent external rotation of the shoulder, which might cause secondary displacement. The objective of this study was to compare this standard immobilisation technique to a posterior plaster splint with a simple

Prevention of re-fractures of both bones of the forearm in children.

PURPOSE: The aim of this study was to observe if the re-fracture rate after forearm both bones was decreased by protecting the forearm with a preventive brace for 6 months following the fracture and by ceasing all physical activities. METHODS: We performed a retrospective study in 75 consecutive cases of diaphyseal fracture of both bone of forearms, in 52 boys and 23 girls aged from 6 months to 11 years. It concerned a first episode of fracture in 84% of cases and a re-fracture in 17%. RESULTS: Re-fracture rate was 0% in the group where a protective brace was worn while it was 20% in the gro

Blood Flow Restriction Therapy after Closed Treatment of Distal Radius Fractures.

Background  Blood flow restriction (BFR) therapy is an innovative rehabilitative program that enables patients to increase strength at a fraction of the weight typically necessary in endurance exercises. Therefore, we conducted a pilot study evaluating patient outcomes with a BFR therapy program for closed management after a distal radius fracture compared to a traditional rehabilitation protocol. Literature review  A randomized-controlled study was conducted comparing a standardized hand therapy rehabilitation protocol alone to a combined protocol with the use of BFR therapy in patients who w

Femoral neck fractures in children and the role of early hip decompression in final outcome.

BACKGROUND: Femoral neck fractures in children are very rare and account for about 1% of all paediatric fractures. The aim of this retrospective study was to analyse the clinical and radiographic outcome in paediatric femoral neck fracture and to review the role of early decompression of the hip in the final outcome. PATIENTS AND METHODS: The study was performed at the Department of Paediatric Orthopaedics and Traumatology, University Children's Hospital in Belgrade, Serbia from January 1996 to January 2010. The study included 28 patients, 12 female and 16 male, aged 4-14 years. Patients who

Incarcerated patellar tendon in Hoffa fracture: an unusual cause of irreducible knee dislocation.

Irreducible knee dislocation due to incarcerated soft tissue structures in associated fractures is extremely uncommon. We report a case of irreducible, closed dislocation of the knee secondary to incarcerated patellar tendon in a Hoffa fracture (unicondylar fracture of femur in the coronal plane) with associated communited fracture of patella and disruption of both cruciates. We treated this case initially with open reduction and internal fixation of fracture and repair of the patellar tendon. During the total followup period of eighteen months, the patient had restricted joint range of motion

Pediatric Coronoid-Deficient Elbow Reconstruction with Iliac Crest Allograft: A Case Report.

CASE: A 5-year-old boy sustained a right elbow fracture dislocation with an associated displaced comminuted coronoid fracture. Acute closed reduction as well as an initial attempt at open repair and fixation failed. This case was salvaged 8 weeks after injury with coronoid reconstruction using an iliac crest allograft. CONCLUSION: This is the first reported case of coronoid reconstruction using iliac crest allograft in a pediatric patient. 2.5 years after reconstruction, the elbow joint is stable, pain free, and demonstrates elbow motion well within the range for full function.

Facial Nerve Paresis After Combined Intraoral and Preauricular Approach to High Condylar Fractures: A Case Report.

Condylar fractures are the most common mandibular fractures, and a variety of open and closed approaches are used for repair, each with its own inherent risks and benefits. A 27-year-old man had a severely displaced and angulated high subcondylar fracture with anterior open bite deformity. The patient underwent open reduction and internal fixation of the condyle via a combined preauricular and intraoral approach as well as maxillomandibular fixation to repair his fracture. This combined approach allows the surgeon to maximize exposure while limiting risks of permanent injury from surgery.

Novel Method for Reduction of Distal Radius Fractures.

Distal radius fractures are one of the most common fractures seen in orthopaedics. These fractures may be treated surgically or conservatively depending on patient-related and radiographic factors. Displaced fractures should be reduced in the acute setting to better align the fracture fragments and a splint applied to hold the fracture in this position. Fractures that are acceptably reduced may be treated conservatively with casting and close radiographic follow-up to ensure maintained alignment. In this video, we describe our technique for closed reduction of distal radius fractures and revie

MRI examination of the TMJ after surgical treatment of condylar fractures.

The position of discs in 20 adult patients whose unilateral condylar fractures were treated by open reduction was investigated by means of magnetic resonance imaging. In four (20%) of the 20 cases, the disc was anteriorly displaced in both the closed mouth and open mouth positions. Three of the four cases had a high condylar neck fracture with dislocation and one had a high condylar neck fracture with displacement. The results of this study showed that repositioning the dislocated condyle did not always lead to anatomical restoration of the joint structures.

Primary fusion of fracture dislocations of central carpometacarpal joints.

Six patients with dislocations or fracture dislocations of at least the second and third carpometacarpal joints are presented. Closed reduction was attempted in all cases. Two dislocations with minimal periarticular fractures were stable and were treated with cast immobilization. Four fracture dislocations were unstable and were treated with primary arthrodesis of the injured joints. One patient was lost to followup and the other 5 were observed for an average of 24 months. Establishment of a stable or rigid central carpometacarpal joint produced an excellent result in 4 patients and a satisfa

A comparison of fracture reductions performed by physician extenders and orthopaedic residents in the acute pediatric orthopaedic practice.

BACKGROUND: The Accreditation Council on Graduate Medical Education-mandated 80-hour work week has led many teaching hospitals to find adequate replacements for residents. In addition, the subspecialty of pediatric orthopaedics is currently experiencing a workforce shortage. At the authors' institution, Level I emergency department and hospital on-call duties alternate between orthopaedic residents and orthopaedic-specific nurse practitioners. There are no studies looking at the quality of care rendered in the pediatric orthopaedic setting by physician extenders. METHODS: A consecutive series

Open reduction and internal fixation of type II intra-articular calcaneus fractures.

Open reduction and internal fixation of 23 type II calcaneus fractures in 21 patients was performed using a standard extended lateral approach. Average follow-up was 26 months (range, 12-47 months). The Creighton-Nebraska Health Foundation Assessment Score for Fractures of the Calcaneus was used to evaluate fractures. The average score was 91.4 (range, 80-100), which represented an excellent result. A series of 10 type II fractures treated with closed methods was also evaluated using the same evaluation methods. The average score was 70 (range, 60-84), which represented a fair result. There wa

Bosworth ankle fracture-dislocation: current concept review.

Bosworth fracture (BF) is a special type of locked ankle fracture-dislocation, characterized by displacement of a fragment of the fractured fibula from the fibular notch behind the posterior surface of the distal tibia. BF is a complex injury affecting multiple structures of the ankle joint, which is still frequently misjudged even today, potentially leading to severe complications. CT examination, including 3D reconstructions, should be the diagnostic standard in BF, as it provides a complete picture of the fracture pathoanatomy, most prominently the morphology of the frequently associated po

Achilles tendon rupture. Occurrence with a closed ankle fracture.

A 61-year-old man, involved in an automobile accident, sustained a complete Achilles tendon rupture with an ipsilateral, closed slightly displaced medial malleolus ankle fracture. The tendon rupture was not diagnosed before operation but was recognized at the time of open reduction of the ankle. This rare combination of injuries was apparently secondary to hyperdorsiflexion of the foot. The tendon rupture would have been missed had surgical treatment not been required. Unrecognized tendon ruptures associated with closed ankle fractures may be a cause of residual ankle-foot weakness, pain, loss
