Skeletal coccidioidomycosis is extremely rare and in the non-endemic areas, diagnosis is often delayed or missed resulting in extensive and … ver staining of a bone biopsy sample revealed multiple fungal spherules, which were confirmed to be Coccidioides immitis by culture and PCR. On questioning, the patient admitted to have spent 2 weeks in Arizona (an endemic area) few months ago. He was discharged home on long-term fluconazole. At 1 month clinical follow-up, a significant improvement in his lesi …

Coccidioidomycosis is a common fungal infection acquired in the southwestern United States. This is the first study in over 2 decades to determine the seroincidence of Coccidioides immitis infections among U.S. military members performing training exercises in an area of endemicity. Only 8% of participants were aware of coccidioidomycosis, despite the majority having visited or lived previously in an area of endemicity. One (0.6%) of the 178 participants developed "definite" serologic evidence of infection over a 5-week training period; four (2.3%) additional patients developed "possible" cocc…

The value of cultures of tissue obtained by image-directed core needle biopsy of lung nodules has not been determined. Of the 250 biopsies performed during a 5-year period in an area endemic for coccidioidomycosis, 225 (90%) were diagnostic. Granulomas were identified in 75 specimens, whereas 3 specimens revealed abscess. Ziehl-Neelsen stain was positive for acid-fast bacilli (AFB) in 2 cases. Spherules of Coccidioides immitis were seen in 54 of the biopsy specimens with granulomas. Microbiologic cultures were positive for C immitis in 5 (9.6%) of 52 biopsy specimens. Both of the AFB-positive …

The clinical use of fluconazole in dosages > or = 800 mg/day has been reported in about 900 patients against candidemia, oropharyngeal candidiasis and cryptococcal meningitis in HIV-infected patients as well as for initial therapy of endemic mycoses. Especially in patients with life-threatening infections caused by Candida spp., Cryptococcus neoformans and Coccidioides immitis, the results of a limited number of dose-finding trials with non-neutropenic and HIV-infected patients show dose-dependent response rates. These findings strongly advocate the application of high dose-fluconazole; their …

This study was designed to define the clinical utility of fungal blood cultures for human immunodeficiency virus type 1-infected individuals … lates in all 15 cases: Candida species and Candida glabrata (6 cases), Cryptococcus neoformans (7), Coccidioides immitis (1), and Histoplasma capsulatum (1). All invasive fungal infections were diagnosed by other means before fungal blood cultures were reported as positive. The results of this study suggest that the routine performance of such cultures in clin …

Mycotic immunodiagnosis was performed in 186 hospitalized patients with different respiratory diseases, mostly considered as tuberculosis and others with a doubtful diagnosis. Crude histoplasmin, coccidioidin, paracoccidioidin, blastomycin, candidin, aspergillin, and sporotrichin, as well as purified polysaccharide-protein complexes (PPC) of Histoplasma capsulatum, Coccidioides immitis, and Paracoccidioides brasiliensis were used as antigens. Immune tests used included skin test (ST), gel immunodiffusion (ID), counterimmunoelectrophoresis (CIE), complement fixation (CF), and ELISA. A possible …

One hundred and twelve patients with progressive pulmonary, skeletal, or soft tissue infections caused by Coccidioides immitis were randomly assigned to treatment with 400 or 800 mg per day dosages of ketoconazole. During therapy, if response was unsatisfactory, the protocol provided for treatment with higher doses. With 400 mg, ketoconazole resulted in 23.2 percent successes, which was similar to 32.1 percent successes with 800-mg treatments (p = 0.29). An additional six of 23 patients in whom initial therapy failed and who later received 1,200 or 1,600 mg per day of ketoconazole also showed …

A case of unsuccessfully treated coccidioidomycosis with intraocular manifestations is presented. Fever, skin lesions and a variety of constitutional symptoms dominated the clinical course. At autopsy there were extensive systemic dissemination and conspicuous intraocular lesions involving the uveal tract. The ocular and systemic features and the immunologic aspects of Coccidioidomycosis are discussed. Previously reported intraocular cases are reviewed and compared to our case.

Pericarditis with effusion can occur as a complication of disseminated coccidioidomycosis. Information on management of this condition is very scanty in the medical literature. One case is described in detail. Early diagnosis and appropriate therapy with amphotericin B have been emphasized as keys to success in treating this condition.

Fourteen patients with chronic coccidioidomycosis, many of whom had complicating concurrent diseases and/or had failed to respond to amphotericin therapy, were treated with intravenous miconazole, a synthetic imidazole drug previously shown to be effective in experimental murine coccidioidomycosis. Up to 3.6 g/day was given for up to three months. 7inimal inhibitory concentrations of mycelial and endospore phases of all clinical isolates of C. immitis were less than 2.0 mug/ml. Peak concentrations in the blood of up to 7.5 mug/ml (by assay against C. immitis in vitro) were achieved. Doses abov

Guinea pigs sensitized by repeated skin testing with potent, merthiolated coccidioidin, equal to Smith standard, exhibited erythema, edema, and an induration-like reaction to intradermal injection of the preparation. No statistically significant migration inhibition of peritoneal exudate macrophages from these animals was observed.

Refractory disseminated coccidioidomycosis (DCM) is a severe disease with limited treatment options. This report reviews our own and prior published experiences using adjunctive interferon-γ therapy in refractory DCM, showing overall favorable responses in patients without signal transducer and activator of transcription 1 gain-of-function mutations.

Coccidioidomycosis is a fungal infection endemic to arid regions. Although it usually presents as a pulmonary infection, extrapulmonary diss … aging and excisional biopsy with histopathology and culture confirmed cervical lymphadenitis due to Coccidioides. Treatment with oral fluconazole (Diflucan) was started, achieving complete clinical resolution after 12 months. This case highlights the importance of considering different fungal infections in immunocompromised patients presenting with lymphadenop …

Haemophagocytic lymphohistiocytosis (HLH) is a rapidly fatal disease caused by dysregulated histiocytes leading to an excessive inflammatory reaction. While genetic forms of HLH exist, the most common form is acquired, frequently associated with infection. Here we report the first case of HLH associated with a coccidiomycosis infection. This patient is a 13-year-old previously healthy boy who presented with a flu-like illness, which rapidly progressed to refractory shock, severe ARDS, multiorgan failure and death despite maximal medical therapy, including broad-spectrum antibiotics to treat we

Coccidioidomycosis is a systemic fungal infection endemic to the southwestern United States. Although cell-mediated immunity is considered critical in control of the infection, little is known of the cellular population in naturally occurring lesions. To characterize the lymphocytic infiltration, archived formalin-fixed, paraffin-embedded tissues (subcutis, pericardium/heart, lung, bone, and synovium) from 18 dogs with coccidioidomycosis were studied with immunohistochemistry for CD3 and CD79a. In nearly all lesions, T lymphocytes were more numerous than B lymphocytes and were distributed thro
