Mimics and chameleons in stroke.

Ischaemic stroke is a treatable medical emergency. In an era of time-dependent reperfusion techniques, it is crucial that an accurate and prompt diagnosis is made. Approximately 30% of patients admitted to hyperacute stroke units are subsequently found not to have a final diagnosis of acute stroke although some of these patients do have incidental or previously symptomatic cerebrovascular disease. These patients do not benefit from thrombolysis and may require the input of other specialists or treatments. Meanwhile, a proportion of patients with acute stroke have unusual presentations and are

What does a comparison of the alcoholic Korsakoff syndrome and thalamic infarction tell us about thalamic amnesia?

In this review, the clinical, neuropsychological, and neuroimaging findings in the alcoholic Korsakoff syndrome and in thalamic amnesia, resulting from focal infarction, are compared. In both disorders, there is controversy over what is the critical site for anterograde amnesia to occur-damage to the anterior thalamus/mammillo-thalamic tract has most commonly been cited, but damage to the medio-dorsal nuclei has also been advocated. Both syndromes show 'core' features of an anterograde amnesic syndrome; but retrograde amnesia is generally much more extensive (going back many years or decades)

Impact of vertigo and spatial disorientation on concurrent cognitive tasks.

Disorientation may draw attention from other activities and its impact on cognition may cause pilot error and problems of concentration in dizzy patients. Tasks used to study the impact of disorientation include number processing, intelligence scales and Brookes matrices (reproduction of 2xD patterns of numbers vs. nonsensical sentences), and Stroop tasks to probe selective effects on spatial versus verbally-loaded tasks. This variety of tasks has been used to evaluate cognitive performance in numerous scenarios, including visual-vestibular mismatch, vection, spinning, coriolis, balancing, and

[A case of agnosia for streets without visual memory disturbance].

A 70-year-old, right-handed man was admitted to our hospital for his sudden-onset topographical disorientation. He failed to find his way to familiar places, but he knew distance and direction to the places. Neurological examination revealed homonymous left-upper quadrantanopsia on Goldmann perimeter and hypoesthesia over the left side of his body. Magnetic resonance imaging showed an abnormal intensity area at the right medial temporo-occipital region, due to the infarct of the right posterior cerebral arterial territory. The neuropsychological examination revealed agnosia for streets, and pr

Scopolamine intoxication as a model of transient global amnesia.

In Colombia (South America) during recent decades the administration of scopolamine, extracted from plants belonging to the Datura or Brugmansia genus, has become an important neurologic and toxicologic phenomenon. These extracts have been popularly known as "Burundanga." Chemical characteristics and clinical features of scopolamine intoxication are described. Anterograde amnesia and submissive behavior found in patients intoxicated with scopolamine are analyzed. Burundanga intoxication is related to other toxic phenomena found in different countries and similitudes with transient global amnes

Depression in the elderly.

Depressions in the elderly have many similarities to depressions in other age groups, particularly in their episodic nature, their tendency to remit, and their potential for favorable immediate outcome. They may, however, be far more easily overlooked. They often are considered to be consistent with senescence because of their frequent association with metabolic disturbance, with a variety of physical illnesses, with impaired cerebral function, and with hypochondriacal manifestations that may be viewed as characteristic of old age. It is important, therefore, that the clinician who treats thes

Avoiding mental changes and falls in older Parkinson's patients.

Falling occurs more often in patients with Parkinson's disease when there is inadequate--not excessive--effect of the antiparkinsonian agent. Thus, falling may be evidence of a need for increased dosage. Of PD patients participating in a study, 60% reported experiencing hallucinations, confusion, vivid dreams, or nightmares. Confusion (34%) was the most commonly reported mental complaint; 27% reported vivid dreams; 26%, hallucinations; and 20%, nightmares.

Affective Reactions When Learning That Our Answer Is Biased: The Role of Negative Feedback in the Arousal of Epistemic Emotions.

This study investigated how different types of feedback influence emotional reactions in decision-making tasks involving high-confidence errors. The sample consisted of 596 undergraduate and postgraduate university students. Participants completed tasks and received either low informative feedback (indicating correctness) or high informative feedback (offering detailed explanations of correct answers). They reported their confidence levels and epistemic emotions of surprise, confusion, and curiosity. Participants reported epistemic emotions after each type of feedback. The results showed that …

Parallel processing blocked by letter similarity in letter by letter dyslexia: A replication.

An investigation of the joint effects of orthographic neighbourhood size (N size) and of letter confusability in three letter-by-letter (LBL) dyslexics is reported. All three patients showed a facilitatory effect of increased N size with low letter-confusability words, but no N size effect with high confusability words. This exactly replicates previous observations by Arguin, Fiset, and Bub (2002) in another LBL dyslexic. A facilitatory N size effect requires parallel letter processing and the word recognition performance of normal readers is unaffected by letter confusability. The present fin

The incidence and nature of letter orientation errors in reading disability.

Letter orientation confusions (reversals) in the reading and writing of 10-year-old children with and without reading disability were investigated to determine whether reading disability is associated with letter orientation errors and to identify the nature of the errors. In a variety of tasks measuring letter orientation confusions in reception (reversal detection and recognition) and production (controlled writing, copying), individuals with reading disability made more orientation confusions than average readers. Orientation errors were more frequent for reversible than for nonreversible i…

Nomenclature of cryptorchidism.

Despite the fact that cryptorchidism is a common congenital anomaly, our understanding of this problem is hampered by confusion and uncertainty regarding diagnosis and treatment. A major factor producing this confusion is the lack of a common terminology that describes to the reader (or listener) exactly what the author means by a given term. Herein is proposed a classification system that, if adopted and utilized, could ameliorate this problem.

[A case of multiple brain calcifications associated with hypoparathyroidism].

We report a case of extensive cerebral calcifications associated with hypoparathyroidism in a 82-year-old woman who has been thyroidectomized and received an antiepileptic treatment for 26 years after an episode of coma followed by confusion. We discuss the frequent confusion in terminology used for describing such cases, emphasizing the necessary distinction between Fahr's disease (extensive cerebral calcifications without abnormal phosphocalcium metabolism) and cerebral calcinosis associated with hypoparathyroidism.

Complex partial status epilepticus: case report and proposal for diagnostic criteria.

A child had complex partial status epilepticus after resection of a craniopharyngioma. The status epilepticus was manifested by confusion and documented electroencephalographically by continuous focal seizure patterns in the right temporal region. Complex partial status epilepticus is an electroclinical syndrome of prolonged or repetitive complex partial seizures (with continuous interictal confusion) accompanied by electroencephalographic seizure patterns, which are either focal (usually temporal lobe) or secondarily generalized from a focal pacemaker.

The effects of emotional content on source monitoring in fibromyalgia patients.

Source confusion refers to a person's failure to distinguish whether an event has been actually seen or simply imagined. Nevertheless, prior research has demonstrated a reduction of source confusion for negative arousing information. According to the emotional-congruence effect, this emotional benefit is likely observed in patients suffering from chronic pain. This hypothesis was tested on 15 patients suffering of fibromyalgia syndrome (FMS) and 15 healthy women. In a source-monitoring task, participants had to decide whether positive, negative, and neutral words were imagined or seen with a p…

Working memory in children: tracing age differences and special educational needs to parameters of a formal model.

Parameters of a formal working-memory model were estimated for verbal and spatial memory updating of children. The model proposes interference though feature overwriting and through confusion of whole elements as the primary cause of working-memory capacity limits. We tested 2 age groups each containing 1 group of normal intelligence and 1 deficit group. For young children the deficit was developmental dyslexia; for older children it was a general learning difficulty. The interference model predicts less interference through overwriting but more through confusion of whole elements for the dysl…

Subclinical encephalopathy in chronic obstructive pulmonary disease.

BACKGROUND: Clinical and experimental evidence suggests that chronic obstructive pulmonary disease (COPD) is associated with a variety of mental symptoms that range from cognitive slowing to mental confusion and dementia. PURPOSE: To test the hypothesis that COPD leads to cognitive impairment in the absence of acute confusion or dementia. METHOD: The global cognitive status of 30 patients with COPD without dementia or acute confusion and 34 controls was assessed with a Brazilian version of the Mini-Mental State Exam (MMSE). RESULTS: The MMSE scores were significantly lower in the patient gr…

Intraoperative pacemaker rate changes associated with the rest mode.

Anesthesiologists frequently are required to provide perioperative management for patients with cardiac rhythm management devices. Here, I describe a case in which, despite efforts to ensure that the anesthesia care team understood the pacemaker and its settings, unanticipated pacing events occurred that created confusion about the status of the pacemaker in a pacemaker-dependent patient. The confusion was created by a relatively new function found in many current pacemakers, a rest mode.

Management of perinatal loss of a twin.

The death of a twin during pregnancy or around birth gives rise to a bewildering confusion of thoughts and feelings that can impede mourning and disturb the bereaved mother's care of a surviving twin. Every effort should be made to give the parents and siblings an experience of the dead baby. Photographs especially can reduce confusion and assist reality testing and thus facilitate the grieving process and improve the care of the surviving twin.
