An 11-year-old boy gradually developed headache, vomiting and diplopia over a period of 1 month. Repeated examinations of head CT scan revealed an arachnoid cyst in the right middle cranial fossa and bilateral subdural effusion of enlarging size. Papilledema was absent on admission, but it became evident after 1 week, and lumbar puncture disclosed very high pressure (800 mmH2O) of the cerebrospinal fluid. This report concerns eight patients with noncolloid neuroepithelial cyst of the lateral ventricle, including four surgical and four conservative cases. Of the surgical patients, two had headache, one had orbital pain, and one had seizures. In the conservative group there were no symptoms due to the cysts and no changes in computed tomography scan during follow-up for an average of 1.4 years. Aneurysmal bone cysts are very rare in the calvaria. We are reporting a 23 year old white male with a very large right frontal aneurysmal bone cyst and a large follow-up period. The MRI findings--a multiloculated expansive tumor with fluid levels inside--are almost pathognomonic for this lesion. Radical surgery was curative for this patient.
