Dynamic FDG PET/CT imaging with diuresis demonstrates an enterovesical fistula in a lymphoma patient with repeated colon diverticulitis.

A 43-year-old male patient with follicular B-cell lymphoma was referred for a FDG PET/CT scan due to severe left lower abdominal pain to rule out recurrent cancer. These FDG PET/CT images and previous FDG PET/CT images 5 months ago both revealed an air bubble in the urinary bladder on the CT images. He had a recurrent urinary tract infection history for 6 months. A list-mode dynamic data acquisition with diuresis intravenous injection revealed linear FDG activity extending from the upper-left portion of the bladder to a soft tissue mass in the lower-left pelvic region. An enterovesical fistula

[The ambulatory medical treatment of colonic diverticulitis. An open clinico-endoscopic-histological study with rifaximin, a nonaminoglycoside enteric antibiotic].

Twenty patients with mild acute diverticulitis of the colon were treated with rifaximine 400 mg b.i.d. per os per 10 days. From a clinical point of view, spontaneous and evoked abdominal pain, diarrhoea, nausea, leukocytosis and hyperthermia resulted in marked significant reduction at the end of treatment, as compared to the basal data (P less than 0.01). A significant reduction of the peridiverticular oedema, mucosal redness, submucosal haemorrhage and diffuse granular pattern was observed endoscopically at the end of treatment (P less than 0.05). Only a trend toward degrees of vasodilation a…

[Conservative treatment of simple, uncomplicated diverticulitis. Preliminary study for evaluating the effectiveness of a method of topical therapy: rectal perfusion].

Transit times and distribution modalities of an oily mixture used as a vehicle for drugs administered by rectal perfusion in topical therapy in patients with uncomplicated diverticulitis of the colon have been examined. X-ray study makes it possible to state that the technique is mainly indicated in descending colon and sigmoid localisations, segments in which the oily mixture distributes homogeneously and massively in all patients treated.

Possible underestimation of non-operative management success in perforated diverticulitis with distant free air : Re: Cirocchi R, Matteucci M, Mari GM et al. (2026) The role of non-operative management (NOM) in perforated diverticulitis: a systematic review. Langenbecks Arch Surg 411:79.

The review reported a 27.8% success rate for non-operative management in patients with distant free air and, on that basis, suggested that early surgery should be strongly considered. When we examined the published data, we were unable to reproduce this estimate. Recalculating from the information given in the review, the numerator appeared to include only studies that reported this outcome, whereas the denominator also included patients from studies in which it was not reported; on a like-for-like basis the success rate is closer to 73% than to the reported 27.8%. Several values in the distan

Elective Colectomy Versus Observation Following Initial Nonoperative Success in Hinchey Ib/II Diverticulitis: A Systematic Review and Meta-Analysis.

Introduction/PurposeDiverticulitis with abscess (Hinchey Ib or II) is often initially managed nonoperatively with antibiotics and/or percutaneous drainage (PCD). For patients who respond successfully to this approach, it remains controversial whether an elective colectomy (EC) should routinely follow. This systematic review and meta-analysis evaluate and compare the outcomes of two strategies in this population: EC vs continued observation after successful nonoperative management.MethodsWe systematically searched PubMed, Embase, Web of Science, and the Cochrane Library through August 15, 2024,…

Cost-Effectiveness Analysis of Early Colectomy vs Percutaneous Drain Placement with Interval Colectomy for Complicated Diverticulitis with Abscess Formation.

BACKGROUND: The optimal timing of nonemergent colectomy for complicated diverticulitis remains controversial. Early colectomy is associated with increased rates of stoma formation and length of stay, whereas interval colectomy prolongs disease management and is subject to repeat diverticulitis flares. We hypothesized that early colectomy is the more cost-effective strategy compared with interval colectomy, yielding lower total cost and improved patient outcomes. STUDY DESIGN: A tree-based decision analysis model was constructed for a reference patient with complicated diverticulitis with asso…

Early Computed Tomography Imaging and Clinical Outcomes in Hospitalized Patients with Acute Diverticulitis: A Propensity-Matched Multicenter Cohort Study.

Background/Objectives: Computed tomography (CT) is the gold-standard diagnostic modality for acute diverticulitis, yet the association between early CT imaging and short-term clinical outcomes among hospitalized older adults has not been evaluated in a large multicenter propensity-matched cohort. We assessed whether early CT abdomen/pelvis (within 24 h of admission) was associated with improved short-term outcomes compared with delayed or absent CT among adults aged ≥65 years hospitalized with acute diverticulitis. Methods: We conducted a retrospective cohort study using the TriNetX US Collabo…

Damage control surgery and open abdomen in Hinchey III-IV acute diverticulitis: current practice from a nationwide Italian survey.

INTRODUCTION: Damage control surgery (DCS) with staged reconstruction may offer an alternative to Hartmann's procedure in selected unstable patients with generalized peritonitis from acute diverticulitis, but its adoption in routine practice is uncertain. METHODS: A nationwide web-based survey explored management of Hinchey III-IV acute diverticulitis, institutional use of open abdomen, and knowledge and application of DCS. Categorical data were summarized as n (%). The survey was endorsed by Italian Society of Emergency and Trauma Surgery (SICUT). RESULTS: Among 175 respondents, 71.4% repor…

Outpatient Management of Acute Sigmoid Diverticulitis: Education and a Clinical Pathway Reduced Antibiotic Use Without Compromise in Clinical Outcomes.

BACKGROUND: European studies have demonstrated that acute uncomplicated sigmoid diverticulitis can be managed without antibiotics in select patients. However, routine use of antibiotics remains prevalent for this disease in the United States. OBJECTIVE: This study aimed to evaluate the effectiveness of a quality improvement initiative implemented to reduce antibiotic use in patients with acute uncomplicated sigmoid diverticulitis discharged from the emergency department. DESIGN: Retrospective study comparing preintervention and postintervention periods. SETTINGS: A single academic instituti…

The early biomarker of immature granulocyte count in predicting right-side colonic complicated acute diverticulitis: a retrospective cohort study.

OBJECTIVE: Early identification of complicated acute diverticulitis(cAD) is especially significant for clinical physician and surgeon to reduce the antibiotic usage and the risk of emergency surgery. This study was aimed to investigate the significance of immature granulocyte(IG) count in early prediction for right-side(Rt-side) cAD. METHODS: The patients with Rt-side colonic acute diverticulitis was enrolled between January, 2019 and March, 2024, and divided into complicated and simple acute diverticulitis group(cAD and sAD). The data about demographic, clinical and laboratory parameters wer…

Long-term outcomes and risk factors for diverticulitis recurrence after a successful laparoscopic peritoneal lavage in Hinchey III peritonitis.

PURPOSE: Recently, treatment of Hinchey III diverticulitis by laparoscopic peritoneal lavage has been questioned. Moreover, long-term outcomes have been scarcely reported. Primary outcome was to determine the recurrence rate of diverticulitis after a successful laparoscopic peritoneal lavage in Hinchey III diverticulitis. Secondary outcomes were identification of associated risk factors for recurrence and elective sigmoidectomy rate. METHODS: A retrospective cohort study in a tertiary referral center was performed. Patients with Hinchey III diverticulitis who underwent a successful laparoscop…

Long-term urogenital assessment after elective laparoscopic sigmoid resection for diverticulitis: a comparison between central and peripheral vascular resection.

AIM: Increasing attention has been given to postoperative gastrointestinal functional outcome and quality of life after sigmoid resection for diverticulitis. Conversely, very little has been described about postoperative urogenital functional outcome and even less about its potential relationship to the type of vascular approach. The aim of this study was to evaluate whether central ligation of the inferior mesenteric artery (IMA) compared with peripheral dissection could impair urinary and sexual function in the long term. METHOD: Patients undergoing elective laparoscopic sigmoid resection f…

The Microbial Ecosystem Distinguishes Chronically Diseased Tissue from Adjacent Tissue in the Sigmoid Colon of Chronic, Recurrent Diverticulitis Patients.

Diverticular disease is commonly associated with the older population in the United States. As individual's age, diverticulae, or herniation of the mucosa through the colonic wall, develop. In 10-25% of individuals, the diverticulae become inflamed, resulting in diverticulitis. The gut ecosystem relies on the interaction of bacteria and fungi to maintain homeostasis. Although bacterial dysbiosis has been implicated in the pathogenesis of diverticulitis, associations between the microbial ecosystem and diverticulitis remain largely unstudied. This study investigated how the cooperative network …

Sequential pancreatitis and diverticulitis following elective anterior lumbar disc replacement: a case report of a rare postoperative domino effect.

INTRODUCTION: Anterior lumbar disc replacement (LDR) is a motion-preserving alternative to fusion for degenerative disc disease but carries risk of vascular and visceral injury. We report the first documented case of a sequential "domino effect" of acute postoperative pancreatitis followed by perforated diverticulitis after LDR surgery. CASE PRESENTATION: A 66-year-old man with diabetes underwent elective L2-L3 LDR via a left anterior retroperitoneal approach. Although initially stable, he developed acute pancreatitis on postoperative day (POD) 1, confirmed by elevated enzymes and CT findings…

National Trends in Hospital Admissions, Interventions, and Outcomes for Early-Onset (Age <50 years) Diverticulitis From 2005 to 2020.

BACKGROUND: Little is known about the burden and outcomes of diverticulitis in patients younger than 50 years. This knowledge gap hinders the development of effective management strategies and preventive measures for this population. OBJECTIVE: This study aimed to analyze national trends in hospitalizations, interventions, and outcomes for early-onset diverticulitis (age younger than 50 years) in comparison to standard-onset diverticulitis (age 50 years or older) cohorts. DESIGN: Retrospective cohort study. SETTINGS: A survey-weighted, national sample extracted from the National Inpatient S…

Perforated Duodenal Diverticulitis: A Case Report of a Rare Surgical Entity Treated by Roux-en-Y Deriving Intestinal Patch.

Perforated diverticulitis is a rare but serious complication associated with a significant mortality rate. Although many cases of conservative treatment have been reported, surgery remains the mainstay for perforated duodenal diverticulitis. We report a rare case of a 55-year-old female who presented with epigastric pain without fever. Computed tomography revealed a 3 cm perforated duodenal diverticulum of the D2 part of the duodenum with a localized abscess. After the failure of conservative treatment, we performed a deriving intestinal patch completed by cholecystectomy and biliary decompres…
