Pseudoaneurysm of the Pancreaticoduodenal Artery Associated with Duodenal Diverticulitis.

BACKGROUND: Duodenal diverticula tend to be asymptomatic; however, patients may develop duodenal diverticulitis. CASE PRESENTATION: A 66-year-old Caucasian man presented to our emergency room with a two-day history of right-sided abdominal pain, chills, tachycardia, nausea, and emesis. His WBC, lactic acid, and bilirubin were elevated. CT-scan revealed an inflammatory process involving the gallbladder, the duodenum and ascending colon, a mesenteric soft tissue mass, and a diverticulum of the second portion of the duodenum. He was admitted, antibiotics were started, and he improved clinically

[A case of papillary renal cell carcinoma mimicking a hemorrhagic renal cyst].

A right renal cyst was found in a 69-year-old man with stage IV chronic kidney disease on abdominal ultrasonography performed to investigate a right upper abdominal swelling. Aspiration cytology of the cyst revealed no malignancy, but malignancy could not be ruled out on magnetic resonance imaging because of the cyst's wall thickness and heterogeneous contents. At one-year of follow-up, emergent abdominal surgery was performed due to incidental perforation of ascending colon diverticulitis. At that time, cystic fenestration was performed because the large renal cyst obstructed the operative pr

Small-bowel perforation caused by fish bone.

A diagnosis of small-bowel perforation, caused by a sharp or pointed foreign body, is rarely made preoperatively because the clinical symptoms are usually nonspecific and can mimic other surgical conditions, such as appendicitis and diverticulitis. We report the case of a 62-year-old woman who experienced severe pain in the right iliac fossa and fever for about five days before arrival at our hospital. The presumptive diagnosis was acute purulent appendicitis and an emergency appendectomy was planned. Swelling and erythema were noted in a segment of the small bowel in the lower right abdomen.

[Septic shock caused by perforation of sigmoid diverticulum into the Retzius space. A case report].

An uncommon complication of the sigmoid diverticulitis personally observed is described. A woman, 59 years old, presented a septic shock caused by perforated sigmoid diverticulum into the space of Retzius. The diagnosis was carried out by an X-ray of the abdomen that suspected the presence of air in the retroperitoneum and then by barium enema showing a perforation of the sigmoid colon. During laparotomy a perforation of sigmoid diverticulum in to the space of Retzius was observed. So the space of Retzius was opened and then pus and watery exudate containing bubbles of gas were found extended

[Diverticulosis of the sigmoid colon from the radiological point of view (author's transl)].

Diverticulosis is one of the most common diseases of the colon. Its importance arises from complications. In countries of the Western hemisphere the incidence is 10-20% beyond the age of 50. Among the natives of Africa and South-Eastern Asia it is practically unknown. It is suggested that the disease is caused by low residue diet, modern living conditions. and constitutional disorders. 500 barium enemas revealed 13,4% patients with diverticula including a 5-year-old child and a 32-year-old woman. The incidence of diverticulosis among patients over 40 years of age is 19,6%. The sex distribution

Laparoscopic surgery for fistulas that complicate diverticular disease.

BACKGROUND: Diverticular disease is complicated by colovesical and colovaginal fistulas in 4-20% of patients. Laparoscopic surgery is usually reserved for selected cases of uncomplicated disease. The aim of this study was to assess the efficacy and effectiveness of laparoscopic surgery in the treatment of those patients. METHODS: Eighteen patients, 15 with colovesical fistulas and three with colovaginal fistulas, were operated on laparoscopically. Prospectively collected data, associated with technical feasibility, short-term outcome and effectiveness, were analysed. RESULTS: Twelve sigmoide

Changing patterns in the surgical treatment of diverticular disease.

This paper includes a brief historic summary of the surgical aspects of diverticular disease and of corresponding developments in the Massachusetts General Hospital from 1911 to the present. The 350 cases observed in 1974-1983 are compared with 338 seen in the previous decade. Major trends include a decrease in hospital admissions for diverticular disease but a sustained number of operations; increased severity of the disease in hospitalized patients manifested by an increased percentage of patients with immunosuppression or serious other diseases (p less than 0.001), an increased number with

[Reflexions on the surgical treatment of diverticular sigmoiditis. Apropos of 62 cases].

The authors report 62 cases of operated diverticular sigmoiditis, note the indications for surgery and the method adopted. Apart from acute complications comparison of the clinical, laboratory and radiological data permit one to decide on a cold operation for prophylactic purposes. For acute generalised or localised peritonitis, the authors remove the lesions straight away when local and general conditions permit it. They emphasis the severity of the complications and the dealy with which these patients are still too often admitted on a surgical unit, which explins the persistently poor progno

Giant Colonic Diverticulum: Case report of a rare complication of a common disease.

INTRODUCTION AND IMPORTANCE: Giant Colonic Diverticulum (GCD) is a rare but potentially life-threatening complication of diverticular disease, defined as a diverticulum larger than 4 cm, most commonly arising in the sigmoid colon. Its rarity could lead to diagnostic delays and mismanagement. CASE PRESENTATION: A 64-year-old Caucasian female presented with persistent abdominal pain and abdominal swelling. Examination revealed a palpable mass in the left flank. A computed tomography (CT) scan demonstrated an 11x10x14 cm air-filled cavity adjacent to the sigmoid colon, initially misdiagnosed as

Foreign body-induced Colovesical fistula: A case report.

BACKGROUND: Colovesical fistula (CVF) is a rare condition characterized by an abnormal connection between the colon and the urinary bladder. While diverticular disease is the most common cause, this report describes a unique case of CVF resulting from an ingested foreign body. CASE PRESENTATION: A 78-year-old woman with a history of COPD and chronic lymphocytic leukemia presented with prolonged urinary symptoms, including dysuria, frequency, hematuria, and a history of pneumaturia and fecaluria. A CT scan with rectal contrast revealed thickening of the sigmoid colon, contrast material extendi

Morphologic pattern of myenteric neural plexus in colonic diverticular disease. A whole-mount study employing histochemical staining for acetylcholinesterase.

BACKGROUND: Diverticular disease (DD) of the colon is a frequent clinical problem because 30-50% of the population over the age of 60 years in western communities are affected by DD. Although certain clinical, physiological and biochemical studies have shown that the origin of DD may be neurogenic, the mechanism of DD pathogenesis is still not clear. METHODS: The aim of the present study has been to assess the morphologic pattern of the myenteric nerve plexus (MNP) in diverticulous sigmoid colon (DSC) comparing the structural organization in DSC (n=10) to relatively normal sigmoid colon (rNSC

Use of a pediatric colonoscope improves the success of total colonoscopy in selected adult patients.

BACKGROUND: Colonoscopy using a standard adult colonoscope can be difficult or impossible when there is a fixed, angulated sigmoid colon or stricture. It is sometimes possible to traverse such segments using a smaller-diameter, more flexible pediatric colonoscope. METHODS: For this prospective study, one endoscopist started 645 consecutive, elective colonoscopies with a standard adult Olympus CF-100L colonoscope (52% women, 48% men). There were 36 examinations (5.6%) in which the adult colonoscope could not be passed through a fixed, angulated sigmoid colon (n = 34) or a sigmoid colonic stric

[Emergency colonic surgery. Value of intervention in delayed emergency].

OBJECTIVES: Currently, there is a trend towards emergency colectomy in patients requiring immediate care for lesions involving the large bowel. However, the therapeutic indications have not been fully identified. We therefore retrospectively evaluated our experience with 123 emergency situations. METHODS: Among 745 cases requiring colonic surgery from January 1985 to August 1992, 123 were seen in emergency for intestinal obstruction (n = 69; 56.1%), peritonitis (n = 41; 33.3%) or haemorrhage (n = 13; 10.6%). The mean age of the patient was 71 years and 50% were over 75. The left colon was inv

[Epiploic appendagitis: an uncommon and nonsurgical cause of abdominal pain].

BACKGROUND: Epiploic appendagitis (AE) is a rare cause of acute abdomen and is often misdiagnosed as other common causes of acute abdomen, such as acute appendicitis, cholecystitis or diverticulitis due to its low incidence and its nonspecific clinical picture. This study presents the clinical course of AE and typical radiological features for an early and correct diagnosis in order to emphasize the importance of an early and correct diagnosis of AE. METHODS: This is a retrospective review of 43 patients diagnosed with AE between June 2010 and September 2022 at the Charité - University Hospit

The Hartmann procedure.

In 1923, Hartmann described an operation for cancer of the rectosigmoid area in high-risk patients not amenable to abdominoperineal resection. In the period 1972 to 1984 at the Queen Elizabeth Hospital in Montreal interest in the Hartmann procedure, with resection, end-colostomy and suture closure of the distal colon intraperitoneally, was renewed. The main indication for the procedure was diverticulitis, with cancer second. The author reviews the experience with 64 patients (average age 80 years) who underwent the Hartmann procedure. Obstruction, perforation and abscesses were the commoner in

Operations on the colon without preoperative oral antibiotic therapy.

The ideal preparation of the intestine prior to elective colonic resection has been a controversial subject. With the development of new and more effective antibiotics, many modalities of intestinal preparation preoperatively have been used in an effort to reduce the number of infectious complications. Mechanical cleansing of the intestine, when accompanied by perioperative parenteral antibiotics, is adequate preparation prior to resection of the intestine. One hundred consecutive operations upon the colon by one surgeon were the subject of this retrospective study. Approximately 70 per cent o

Colonic diverticulosis in Hong Kong: distribution pattern and clinical significance.

This is a retrospective study on the prevalence of diverticulosis in Hong Kong Chinese adults. Eight-hundred and fifty-eight consecutive barium enema examinations over a period of 18 months (January 1995-June 1996) were analysed. Results show that the prevalence of diverticulosis in our community is 25.1% with no significant difference between male and female adults. The prevalence is lower than Western countries but higher than in Asia. Moreover, the peak prevalence is at the 50-79 years age group with lower prevalence in the older age groups. This may be explained by the rapid rise in preval

Elective one-stage abdominal operations after percutaneous catheter drainage of pyogenic liver abscess.

During the past 10 years, 15 patients have had percutaneous catheter drainage (PCD) of pyogenic liver abscesses (PLA) at a major teaching hospital. Five PLA were related to biliary tract disease, two were secondary to colonic diverticulitis, two developed after abdominal surgery, and the remaining were associated with hepatic trauma, gastric ulcer, Crohn's ileitis, and colon cancer. Two abscesses were cryptogenic. Mean diameter of PLA was 8 cm and ranged from 2-14 cm. Three patients had multiple PLA. All patients were initially treated by PCD without major complications. However, one patient r

Role of protective stoma after primary anastomosis for generalized peritonitis due to perforated diverticulitis-DIVERTI 2 (a prospective multicenter randomized trial): rationale and design (nct04604730).

BACKGROUND: Traditionally, patients with peritonitis Hinchey III and IV due to perforated diverticulitis were treated with Hartmann's procedure. In the past decade, resection and primary anastomosis have gained popularity over Hartmann's procedure and recent guidelines recommend Hartmann's procedure in two situations only: critically ill patients and in selected patients with multiple comorbidity (at high risk of complications). The protective stoma (PS) is recommended after resection with primary anastomosis, however its interest has never been studied. The aim of this trial is to define the
