The aim of this case report was to present the outcomes of minimally invasive surgical treatment of a rare and atypical form of an elbow joint luxation, known as 'divergent dislocation of the elbow' in a dog. Initially, a closed reduction of the dislocated elbow joint was performed, then surgical treatment was decided. A small incision was made on the caudal side of the ulna, then a 2 mm Kirschner wire was inserted from the caudal aspect of the proximal ulna to the radial neck. Following that, a 20 mm, 2.4 mm cortical screw was placed, securing the luxated radius to the ulna. All the procedure…

INTRODUCTION: Posttraumatic elbow arthritis in young adults is a rare yet challenging condition, presenting primarily with progressive pain and stiffness that limits functional activities. Achieving a functional, low-pain elbow joint with future treatment options is essential for younger, active patients. Unlike total elbow arthroplasty (TEA), which is prone to loosening in young patients, interposition arthroplasty (IA) offers bone preservation, pain reduction, and improved joint function.
CASE PRESENTATION: A 22-year-old male presented with four years of right elbow pain and stiffness follo…

Peripheral nerve compression syndromes at the elbow include Cubital Tunnel Syndrome (CuTS), Radial Tunnel Syndrome (RTS), Posterior Interosseous Syndrome (PINS), Pronator Teres Syndrome (PTS), and Anterior Interosseous Nerve Syndrome (AINS). These conditions, though rare compared to carpal tunnel syndrome, can cause debilitating motor and sensory deficits. This review aims to consolidate the latest evidence about treatments to improve therapeutic outcomes and provide clinical recommendations. Conservative treatment is usually first-line and includes physical therapy, splinting, nonsteroidal an…

AIMS: The management of fractures of the medial epicondyle is one of the greatest controversies in paediatric fracture care, with uncertainty concerning the need for surgery. The British Society of Children's Orthopaedic Surgery prioritized this as their most important research question in paediatric trauma. This is the protocol for a randomized controlled, multicentre, prospective superiority trial of operative fixation versus nonoperative treatment for displaced medial epicondyle fractures: the Surgery or Cast of the EpicoNdyle in Children's Elbows (SCIENCE) trial.
METHODS: Children aged se…

BACKGROUND: Pneumatic tourniquets are frequently employed in extremity surgeries, aiming to enhance intraoperative visibility and minimize blood loss. Although their benefits and drawbacks have been extensively studied in lower limb operations, their impact on upper limb procedures, particularly elbow surgery, remains poorly understood. This study investigates the advantages and risks associated with pneumatic tourniquet used in elbow surgery.
METHODS: A retrospective analysis was conducted on 183 patients who underwent elbow surgery for fractures between January 2019 and September 2023. Pati…

INTRODUCTION: Surgical treatment of ulnar tunnel syndrome is controversial, with no consensus on the ideal procedure. Nevertheless, in-situ decompression seems to provide faster recovery with less morbidity than neurolysis-transposition. The aim of this retrospective study was to validate a new percutaneous technique using ultrasonography.
METHODS: A retrospective 2-center study included 40 patients with McGowan stage 1 or 2 ulnar nerve entrapment at the elbow treated by a new limited ultrasound-guided percutaneous technique. Patients with McGowan stage 3, osteoarthritis at the elbow or insta…

BACKGROUND: The reasons for malpractice litigation in elbow surgery are not well understood. The aim of this study is to report the most frequently litigated surgeries of the elbow and analyze the reasons for litigation and case outcomes.
METHODS: A retrospective review of the Westlaw legal database was performed, and all federal and state jurisdiction litigation cases involving the elbow from 2000 to 2023 were queried. Cases were excluded if they did not involve an orthopedic surgeon, nor primary elbow injury or procedure. Cases were reviewed for demographic information, surgical procedure b…

PURPOSE: Traumatic elbow instability (subluxation and dislocation) represents a challenging clinical entity that can be complicated by persistent intraoperative instability after attempted stabilization or recurrent postoperative instability. Both static and dynamic supplemental stabilization procedures have been described for cases where fracture fixation and ligament repair fail to restore stability. There is a paucity of prior studies involving ulnohumeral cross-pinning (UCP), which is a type of static supplemental stabilization. Our purpose was to assess complications and outcomes after UC…

Terrible triad injuries of the elbow are complex injuries which can result in long term complications and significant disability. They must be identified correctly, and managed appropriately in order to maximise functional outcomes. A clear understanding of the bony and ligamentous anatomy is essential to plan appropriate surgical reconstruction to provide elbow stability. Urgent reduction of the elbow, followed by 3-dimensional imaging and surgical repair or replacement of the injured structures is the mainstay of treatment in the majority of cases. This review presents a summary of the relev…

Although pitching-related injuries in the overhead athlete have been studied extensively, injuries associated with windmill pitching are not as clearly elucidated. Windmill pitching produces high forces and torques in the upper extremity, and studies have shown it creates similar shoulder and elbow joint loads to those reported in baseball pitchers. Studies have shown that the windmill pitching motion generates high levels of biceps activation with an eccentric load, placing the biceps at increased risk for overuse injuries. Although the American Orthopaedic Society for Sports Medicine publish…

Individuals with C5 or C6 spinal cord injury (SCI) have paralysis of the triceps brachii, and the subsequent loss of elbow extension makes it impossible to reliably use their hands above shoulder level because of the inability to hold the elbow extended against gravity. For persons with cervical SCI, elbow extension can be restored with both tendon and nerve transfers. Elbow extension is necessary for dressing, eating, wheelchair locomotion, pressure relief maneuvers, independent transfers, and reaching objects above shoulder level. Deltoid-to-triceps and biceps-to-triceps tendon transfers hav…

Reconstruction of unstable osteochondritis dissecans lesions of the capitellum using fresh osteochondral allograft transplantation from the capitellum has the advantages of restoring hyaline cartilage, matching the native radius of curvature, and avoiding the donor-site morbidity encountered with osteochondral autograft transfer. This technical note describes the indications and contraindications, pertinent anatomy, and surgical technique of open osteochondral allograft transplantation using fresh distal humerus allograft for the treatment of unstable osteochondritis dissecans lesions of the c

Tennis elbow (lateral epicondylitis or lateral elbow tendinopathy) is a self-limiting condition in most patients. Surgery is often offered to patients who fail to improve with conservative treatment. However, there is no evidence to support the superiority of surgery over continued nonoperative care or no treatment. New evidence also suggests that the prognosis of tennis elbow is not influenced by the duration of symptoms, and that there is a 50% probability of recovery every three to four months. This finding challenges the belief that failed nonoperative care is an indication for surgery. In…

Tennis elbow, a common musculoskeletal disorder also known as lateral epicondylitis, causes pain and tenderness on the outside of the elbow. Although it is frequently linked to repetitive motions, such as those in racquet sports, it can affect people in a variety of occupations and activities. Tennis elbow can be treated conservatively with physiotherapy, which focuses on pain management, functional recovery, and recurrence prevention. The goal of this review is to give a thorough overview of the physiotherapy methods used to treat tennis elbow. In order to determine the most effective treatme…

BACKGROUND: Elbow fractures are common in children. While Kirschner wire (K-wire) is the most commonly used fixation material in children, medial entry pins may be needed for fracture stability. This study aimed to assess ulnar nerve instability by ultrasonography in children.
METHODS: We enrolled 466 children aged 2 months to 14 years between January 2019 and January 2020. There were at least 30 patients in each age group. Ulnar nerves were observed under the ultrasound equipment with the elbow fully extended and flexed. If ulnar nerves were subluxated or dislocated, they were considered to …
