Anterior elbow dislocation with potential compartment syndrome: a case report.

Anterior elbow dislocation is an infrequent lesion, usually produced by direct trauma to the proximal ulna after a fall on the elbow in flexion, and is often associated with soft tissue injuries. The authors report a case of a complex injury produced by a high-energy trauma in the right arm of a 65-year-old patient. His limb was trapped inside an industrial spin-dryer, resulting in a closed anterior elbow dislocation, diaphyseal ulnar shaft, radial styloid process fractures, and an associated compartment syndrome. The injury mechanism and its treatment are described to better manage the soft t

Early mobilization of isolated ulnar-shaft fractures.

A consecutive series of 21 isolated ulnar-shaft fractures were treated with early mobilization. All the fractures healed with good function and without serious complications. As long as the radioulnar joints are intact, this type of fracture can safely be treated with immediate mobilization or a short period in a below-elbow cast.

Comparison of Functional and Cosmetic Outcomes According to Fracture Level in Gartland Type III Pediatric Supracondylar Humerus Fractures.

BACKGROUND: Supracondylar humerus (SCH) fractures in children have been traditionally categorized according to the Wilkins-modified Gartland classification scheme, which is solely based on the degree of displacement. As this classification does not consider fracture patterns in the coronal or sagittal plane, the relationship between the fracture pattern and prognosis in SCH fractures remains unclear. Therefore, the purpose of this study was to evaluate the relationship between the fracture level and prognosis of pediatric SCH fractures. METHODS: Medical records and radiographs of 786 patients

The management of distal humeral fractures in older patients.

The number of fractures of the distal humerus in older adults is rising due to an ageing population and an increasing incidence of fragility fractures. The management is based on the patient's preference, the pattern of the fracture, the quality of the bone and soft-tissue, the patient's comorbidities and potential for rehabilitation, as well as balancing the risk of complications such as infection, nonunion, instability, or loosening. Nonoperative treatment remains appropriate for a few low-demand or medically unfit patients. Open reduction and internal fixation (ORIF) or arthroplasty, either

Huckstep intramedullary nailing for non-union of humeral shaft fractures.

BACKGROUND: Various techniques of internal fixation for non-union of humeral shaft fractures have been attempted, leading to union rates of between 50 and 90% with persisting non-union causing pain and disability. Some of these techniques have led to shoulder and elbow dysfunction. METHODS: Eight patients treated with Huckstep nail fixation for humeral shaft non-union were reviewed and the rate of union was determined. Elbow and shoulder function were assessed as well as the presence of pain or disability. RESULTS: Union was achieved in seven of the eight patients (87.5%). Four patients had

Risk factors for the displacement of fractures of both bones of the forearm in children.

Forearm fractures in children have a tendency to displace in a cast leading to malunion with reduced functional and cosmetic results. In order to identify risk factors for displacement, a total of 247 conservatively treated fractures of the forearm in 246 children with a mean age of 7.3 years (sd 3.2; 0.9 to 14.9) were included in a prospective multicentre study. Multivariate logistic regression analyses were performed to assess risk factors for displacement of reduced or non-reduced fractures in the cast. Displacement occurred in 73 patients (29.6%), of which 65 (89.0%) were in above-elbow ca

Bipolar Disruption of the Lateral Ulnar Collateral Ligament of the Elbow: A Case Report.

CASE: The lateral ulnar collateral ligament (LUCL) is one of the primary stabilizers of the elbow. Disruption typically occurs from the humeral origin and may be because of an elbow dislocation or fracture/dislocation. If not identified and properly managed, posterolateral rotatory instability may result from LUCL insufficiency. We describe the case of a patient with bipolar LUCL disruption consisting of an avulsion fracture of the crista supinatoris and a soft-tissue avulsion from the lateral epicondyle. CONCLUSION: Bipolar disruption of the LUCL is a previously unreported injury pattern tha

[Ultrasound diagnosis in trauma: elbow and hand].

Ultrasonography with high-frequency transducers nowadays plays an important role in diagnosing post-traumatic complaints of the elbow and hand tissue in children as well as in adults. We see it as an addition to clinical examination, standard X-ray, and other radiological methods. As a noninvasive procedure we can investigate post-traumatic changes and postoperative pain, detect foreign bodies, and also use it for postoperative metal removal. The possibility of imaging cartilage structures with ultrasound also enables us to diagnose post-traumatic injuries in children's elbows such as supracon

Sagittal Plane Residual Deformity in Pediatric Type II Supracondylar Humerus Fractures.

INTRODUCTION: The incidence of residual deformity in the sagittal plane of the humerus (RDSPH) after nonoperative management of type II supracondylar humerus fractures (SCHFs), and the effects of such deformity on the overall arc of motion (AOM) of the elbow, are unknown. Our purpose was to analyze data collected prospectively on a large cohort of type II SCHF's to establish the incidence and extent of RDSPH, and the effects of the deformity on the elbow function, to further support our previously published recommendations on the treatment of type II SCHF. METHODS: The clinical data and radio

Traumatic cubitus valgus consequent of distal humeral fracture: Two case studies from the Holocene Later Stone Age in southern Africa.

OBJECTIVES: Distal fractures of the humerus and their complications have rarely been described or analysed in the palaeopathological literature. The objective of this study was to evaluate two cases of distal humeral fracture with associated cubitus valgus observed in two individuals from the context of the Later Stone Age (LSA) in southern Africa. MATERIALS: Skeletal remains of two individuals. A middle-aged female radiocarbon dated to c.160 BP and a middle-aged male radiocarbon dated to c.2 300 BP. METHODS: Remains were macroscopically and radiographically assessed for injury. RESULTS: Bo

Midterm results with a bipolar radial head prosthesis: radiographic evidence of loosening at the bone-cement interface.

BACKGROUND: Metal prostheses are useful for restoring elbow and forearm stability when the radial head cannot be fixed after a fracture. Because the anatomy of the radial head is difficult to reproduce with a prosthesis, two different options have been proposed: a bipolar prosthesis with a fixed stem and a mobile head, and a monoblock prosthesis with a smooth stem that is intentionally fixed loosely in the neck of the radius. One concern with a fixed-stem implant with a mobile head has been the risk of osteolysis. The purpose of this study was to evaluate radiographic changes reflecting or sug

Magnetic resonance imaging of the elbow: update on current techniques and indications.

The utility of magnetic resonance imaging of the elbow has noticeably improved over the past decade. Advances in equipment and refinement of pulse sequences has allowed for superior visualization of many pathologic processes involving the elbow joint and its surrounding structures. At this time, magnetic resonance imaging is most valuable in detecting occult fractures, osteochondritis dissecans, collateral ligament tears, nonossified intra-articular loose bodies, and soft tissue masses. It may also aid in the diagnosis of tendinous injuries, compression neuropathies, and synovial disorders. Th

An Unusual Terrible Triad Variant Associated with an Essex-Lopresti Injury.

Essex-Lopresti injuries and terrible triad injuries of the elbow are rare injuries that typically result from high-energy trauma such as falling from a height or a motor vehicle collision. However, the combination of an Essex-Lopresti injury and terrible triad injury is unique and poses a significant challenge for treatment as these injuries are independently associated with poor functional outcomes if they are not acutely diagnosed. We describe a case of a 19-year-old who presented with an unusual variant of a terrible triad injury associated with an Essex-Lopresti injury. The patient had a d

[Late ulnar nerve paresis in adults after neglected treatment for fracture of the capitulum with part of the trochlea in children].

Etio-pathomorphogenesis of compression syndromes of the ulnar nerve strained over the medial side of the valgus elbow is presented on the basis of follow-up of 11 patients. Valgus deformity of the elbow resulted from neglecting of surgical reduction of the displaced capitulum and part of the trochlea in the childhood. Cartilage of the articular surface of the displaced fragment facing injury site of the humeral condyle impedes healing of the fracture and disturbs the growth of the lateral epiphysis. Thus valgus deformity is accompanied by pseudoarthroses and various deformities of the articula

Radial Head Fractures.

BACKGROUND: Radial head fractures are common elbow injuries in adults and are frequently associated with additional soft tissue and bone injuries. METHODS: A literature search was performed and the authors' personal experiences are reported. RESULTS: Mason type I fractures are treated non-operatively with splinting and early mobilisation. The management of Mason type II injuries is less clear with evidence supporting both non-operative treatment and internal fixation. The degree of intra-articular displacement and angulation acceptable for non-operative management has yet to be conclusively

Use Of POCUS for the Paediatric Patient with an Undifferentiated Upper Limb Injury.

A 2-year-old girl presented to the Paediatric Emergency Department following an unwitnessed injury to her left arm while playing at nursery limiting further examination. On assessment she was reluctant to use her left arm and further examination was difficult. In cases of unwitnessed and undifferentiated elbow injuries point of care ultrasound (POCUS) can be used to evaluate for elbow joint effusion, fracture, or radial head subluxation, also known as nursemaid's elbow. Pulled elbow is a commonly encountered paediatric injury but based on the history and examination findings it may not always

Short- to mid-term clinical outcomes and survivorship of the Nexel total elbow arthroplasty in Japanese patients: a single-center retrospective study.

BACKGROUND: Total elbow arthroplasty (TEA) has become a well-established surgical option for advanced elbow disorders. The Nexel prosthesis (Zimmer Biomet, Warsaw, IN, USA) has been associated with mechanical complications and implant instability in previous international reports. However, no clinical outcomes have been published for Japanese patients. This study aims to evaluate the short- to mid-term clinical outcomes of the Nexel TEA in a Japanese cohort. METHODS: We retrospectively reviewed 31 elbows in 30 patients (6 men, 24 women; mean age 73.0 years) treated with the Nexel prosthesis b
