Erythema multiforme as a rare skin manifestation during pembrolizumab treatment: a case report and literature review.

We report the case of a 77-year-old man with recurrent oral squamous cell carcinoma who developed erythema multiforme (EM) following his third dose of pembrolizumab (PBZ). The patient presented with a widespread skin rash characterized by erythematous papules and plaques that evolved into targetoid lesions, with no mucosal involvement or systemic symptoms. The treatment with a tapering dose of prednisolone resolved the rash without interrupting PBZ therapy. The report reviews the existing literature on PBZ-induced EM as a rare cutaneous toxicity associated with immune-checkpoint inhibitors (IC…

A case of mixed connective tissue disease with usual interstitial pneumonia and methotrexate-induced erythema multiforme.

Mixed connective tissue disease (MCTD) is a rare autoimmune disorder characterized by overlapping features of various connective tissue dise … hich led to bullous drug eruptions on the palms, lips, and soles, diagnosed as methotrexate-induced erythema multiforme. Following the discontinuation of methotrexate, treatment with mycophenolate mofetil and prednisolone resulted in the resolution of fever and synovitis within 1 month. Over 6 months, there was significant improvement in Raynaud's phenomenon and scle …

Rowell Syndrome in Nigeria: Systemic Lupus Erythematosus Presenting as Recurrent Erythema Multiforme in a Young Woman.

Dear Editor, Systemic lupus erythematosus (SLE) is a heterogeneous autoimmune disease characterized by diverse patterns of auto-antibody production with multi-organ affectation. Cutaneous involvement, either alone or in association with other systemic illnesses, is one of its most common manifestations (1). Dermatologic disorders like malar and discoid rashes are quite suggestive of SLE. However, the occurrence of non-specific skin lesions like erythema multiforme (EM) in patients with SLE (Rowell syndrome) can rarely occur (1). In such patients, a diagnosis of SLE may be missed or delayed in …

Erythema multiforme with reference to atypical presentation in an HIV-positive patient following antiretroviral therapy discontinuation.

Erythema multiforme (EM) is a skin disease caused by numerous potential triggering agents. It is characterized by symmetrically distributed, round, erythematous iris lesions, with remarkable tendency of recurrence. The purpose of this randomized retrospective study was to summarize our experience concerning the etiology, clinical variants and management of EM. We present 12 EM patients, along with laboratory, clinical and histopathologic analysis, and an overview of the treatment used. According to clinical presentation and histopathologic analysis, there were six cases of EM major, five cases…

Bullous erythema multiforme after treatment with Malarone, a combination antimalarial composed of atovaquone and proguanil hydrochloride.

We report on a Kenyan woman who developed bullous erythema multiforme, in association with Malarone treatment.

Detection and genotyping of human herpes simplex viruses in cutaneous lesions of erythema multiforme by nested PCR.

A subset of erythema multiforme (erythema multiforme) is associated with herpes simplex virus (HSV) infection; viral cultures of erythema multiforme lesions are, however, usually negative and viral antigens difficult to identify. Polymerase chain reaction (PCR) has been used to demonstrate the association, hence, is currently the only available sensitive diagnostic means for HSV-associated erythema multiforme. A nested PCR, which could simultaneously detect and genotype HSV in erythema multiforme lesions and in clinical swab specimen was developed using the DNA polymerase gene of HSV as target…

The dermal type of erythema multiforme: a rare variant of Stevens-Johnson syndrome or cases of clinical misclassification?

Since 01.04.90 the Dokumentationszentrum schwerer Hautreaktionen (dZh) in Freiburg has registered cases of severe skin reactions like erythema exsudativum multiforme majus, Stevens-Johnson syndrome and toxic epidermal necrolysis in Germany. With the largest study so far of histological slides from patients included in this registry we were able to show that the epidermal type of erythema multiforme described by Orfanos et al. is the histopathological correlated of these severe skin reactions. Except two biopsies all of the specimens taken from the registered patients showed histological charac…

Increased adherence to keratinocytes of peripheral blood mononuclear leucocytes of a patient with drug-induced erythema multiforme.

The pathogenetic mechanisms involved in the development of drug-induced erythema multiforme (EM) are still largely unknown. The observation that epidermal keratinocytes (KC) in EM express intercellular adhesion molecule-1 (ICAM-1) points to a putative role for T-cell/KC adhesion in the pathogenesis of EM. In this study, the binding of peripheral blood mononuclear leucocytes (PBML) from a patient with carbamazepine-induced EM and of normal control PBML to autologous and heterologous KC was investigated, using two different binding assays. Patient PBML obtained at the time of disease (t0) showed…

Sorafenib-Induced Erythema Multiforme Major and Severe Hepatic Failure in Metastatic Hepatocellular Carcinoma: A Case Report.

Sorafenib, a kinase inhibitor, is known to cause skin toxicity, which sometimes leads to treatment interruption or drug dose reduction. Erythema multiforme (EM) is one of these dermatologic toxicities induced by sorafenib. We report the case of a 28-year-old male with hepatocellular carcinoma (HCC). Two months after surgery, the patient presented with multiple metastases to the retroperitoneal lymph nodes and lungs. Therefore, systemic therapy with sorafenib was indicated. While receiving the medication, the patient presented signs compatible with EM. The signs occurred on the torso and then s…

Reactive infectious mucocutaneous eruption due to COVID-19 with erythema-multiforme-like lesions and myeloid cells.

Reactive infectious mucocutaneous eruption (RIME) is a recently described entity in which there is prominent mucositis, most commonly involving the oral and urogenital mucosa, secondary to a variety of pathogens. There is typically minimal cutaneous involvement in RIME. This contrasts with erythema multiforme (EM) in which characteristic targetoid lesions predominate, usually in isolation (EM minor), but in a subset of cases, with severe mucositis (EM major). While the histopathologic features of RIME have not been as well defined, those of EM are characterized by epidermal apoptosis and inter…
