Contact heat-evoked potentials as a useful means in patients with Guillain-Barré syndrome.

Few objective methods have been utilized to identify the small myelinated fiber impairment causing neuropathic pain in Guillain-Barré syndrome (GBS). In this study, contact heat-evoked potentials (CHEPs) were applied to study the nociceptive pathway in GBS. Sixty GBS patients and fifty healthy controls were enrolled. The 60 GBS patients were divided into two subgroups presenting with or without subjective lower limb paresthesia (21/39). CHEPs were recorded at Cz and Pz with a peak thermal stimuli of 47 °C applied to the skin of the leg above the internal malleolus (AIM) and of the waist at the…

Vagus somatosensory-evoked potentials are prolonged in patients with multiple sclerosis with brainstem involvement.

Vagus somatosensory-evoked potentials (VSEP) were proposed as a neurophysiological indicator of brainstem dysfunction based on prolonged latencies found in Alzheimer's dementia and Parkinson's disease. We now aimed at a further confirmation of this view independent from neurodegenerative diseases and hypothesized that VSEP in multiple sclerosis with brainstem affection show prolonged latencies, too. In 15 patients with multiple sclerosis according to McDonald and 15 healthy controls after stimulation of the auricular branch of the vagus nerve at the tragus (electrical square impulses, impulse …

Tongue's motor evoked potentials in the diagnosis of primary lateral sclerosis (PLS): preliminary report.

BACKGROUND: Primary Lateral Sclerosis (PLS) is an adult-onset neurodegenerative disorder due to a selective loss of precentral pyramidal neurons. Our purpose was to evaluate preferential impairment of pyramidal tract to bulbar muscles in patients with PLS and identify a valuable electrophysiological method to help clinicians in the differential diagnosis from Amyotrophic Lateral Sclerosis (ALS). MATERIALS AND METHODS: We recorded Motor Evoked Potentials (MEPs) from tongue's and anterior tibialis muscles in six patients with PLS and compared the results, in terms of Central Motor Conduction Ti…

Nociceptive pathway function is normal in cervical dystonia: a study using laser-evoked potentials.

Cervical dystonia (CD) is often associated with pain in the neck muscles, though the mechanisms underlying pain in this condition are still largely unknown. The aim of this study was to assess laser pain rating and CO(2) laser-evoked potentials (LEPs) in CD patients with pain in the posterior neck region. We assessed the N2/P2 LEP complex and laser pain rating in a group of 20 CD patients and in 21 normal subjects. In 11 of the 20 CD patients (group I), the N2/P2 complex was recorded after stimulation of the skin overlying the right and left deltoid muscles (painless and non-dystonic). In the …

Ocular vestibular evoked myogenic potentials in response to air-conducted sound in Ménière's disease.

BACKGROUND: Currently, Ménière's disease is predominantly diagnosed through clinical criteria. Additional standard vestibular testing, such as nystagmography, can show variable responses. In the last decade, the cervical vestibular evoked myogenic potential (VEMP) has shown to be of additive value in diagnosing Ménière's disease. In this study, the results of the ocular VEMP (oVEMP) in response to air-conducted sound will be discussed. OBJECTIVES: To evaluate possible changes of the oVEMP in a large group (n = 37) of patients with Ménière's disease. MATERIAL AND METHODS: In 55 subjects witho

Audiogram of a stranded Blainville's beaked whale (Mesoplodon densirostris) measured using auditory evoked potentials.

Quantifying and understanding the impact of anthropogenic sound on marine mammals has been the focus of many researchers both in laboratory settings as well as in the field. This study presents the audiogram of a sub-adult Blainville's beaked whale that stranded in Hawaii. The hearing measurements were conducted using the non-invasive auditory brainstem response technique. A total of 11 sinusoidally amplitude modulated tones were tested ranging from 5.6 to 160 kHz. The audiogram data indicated that the region of best hearing was found between 40 and 50 kHz with thresholds below 50 dB. This fre

Laser-evoked potentials: prognostic relevance of pain pathway defects in patients with acute radiculopathy.

The radicular pain syndrome is a major problem in public health care that can lead to chronic back and leg pain in 30%. Ischalgia and back pain are the most prominent signs of dorsal root affection. Until now, no clinical or neurophysiological test procedure exists that evaluates the function of the dorsal root and predicts the prognosis of patients suffering from RPS. We have recently demonstrated that laser-evoked potentials (LEP) are able to demonstrate dorsal root damage. With this study, we investigated 54 patients with acute radicular symptoms and compared LEP parameters (side to side di…

Alarm criteria for motor-evoked potentials: what's wrong with the "presence-or-absence" approach?

STUDY DESIGN: Combined prospective and retrospective. OBJECTIVE: Evaluate 2 published criteria for interpreting motor-evoked potentials (MEP) in response to repetitive transcranial electrical stimulation (rTES) during surgery. SUMMARY OF BACKGROUND DATA: There is controversy regarding how to interpret MEPs elicited by rTES. Many centers warn the surgical team only if the MEP is lost entirely ("Presence-or-Absence" method). Alternatively, we monitor the stimulus energy needed to elicit a minimal evoked EMG response; significant increases in this energy reflect impending motor tract injury and…

Objective assessment of cervical spinal cord injury levels by transcranial magnetic motor-evoked potentials.

BACKGROUND: The neurologic examination serves as the optimal method to record the level of spinal cord injury (SCI). However, this test is subject to interexaminer variability. To address this shortcoming, we describe a technique that uses transcranial magnetic motor-evoked potentials (tcMMEPs) and dermatomal somatosensory-evoked potentials (d-SSEPs) to more accurately measure the precise level of SCI. METHODS: Two groups of subjects were studied: (1) complete cervical SCI (n = 10) and (2) neurologically intact volunteers (n = 10). Two additional patients were evaluated: one with a cervical c…
